Devices, systems, and methods for suturing of body lumens allow the suturing of vascular puncture sites located at the distal end of a percutaneous tissue tract. An elongated articulated foot near a distal end of a shaft is inserted through the penetration and actuated so that the foot extends along the lumenal axis. The foot carries suturing attachment cuffs, and needles are advanced from the shaft through the vessel wall outside of the penetration and into engagement with the needle cuffs after the foot has been drawn proximally up against the endothelial surface of the blood vessel. The cross-section of the shaft within the tissue tract can be minimized by laterally deflecting the needles as they leave the shaft, while tapered depressions within the foot can guide the advancing needles into engagement with the cuffs. The cuffs lockingly engage the needles and can be withdrawn proximally along the needle paths and through the tissue tract so as to form a loop of suture across the puncture. The articulating foot may be realigned with the shaft and withdrawn proximally through the tissue tract without dilating the tissue tract.

A method for suturing a puncture in a tissue, the method, comprising: providing a device that includes a suture having a first end and a pre-tied knot spaced from said first end, the pre-tied knot being disposed about a first needle of the device at a point spaced back from the distal end of the device; advancing said first end of said suture through the tissue using a second needle; advancing said first end of said suture through said pre-tied knot using said first needle following advancing said first end of said suture through the tissue using the second needle; and tensioning said suture across said puncture formed in said tissue, wherein said first needle is provided with a distal tissue penetrating tip, and wherein advancing said first end of said suture through said pre-tied knot includes pulling said suture with said distal tissue penetrating tip of said first needle. 2. The method of claim 1, further comprising: providing a shaft, said first needle advanceable along said shaft, said pre-tied knot being releasably attached to said shaft; and advancing said shaft through a tissue tract of a patient body. 3. The method of claim 2, wherein providing a suture having a first end and a pre-tied knot spaced from the first end, the pre-tied knot pre-arranged on the device, further including the pre-tied knot being pre-arranged on the device, further including the pre-tied knot being pre-arranged about the first needle. 4. A method for suturing a puncture in a tissue using a device, the method comprising: inserting at least a portion of a suture through a tissue tract of a patient body and through the tissue using a second needle; inserting at least a portion of a first needle through the tissue tract; advancing a first end of the suture with said first needle through a pre-tied knot of the suture following inserting said first needle through the tissue tract, the pre-tied knot being disposed about the first needle of the device at a point spaced back from the distal end of the device; and tensioning the first end of the suture and a second end of the suture adjacent the pre-tied knot to form a knot affixing a loop of suture across the puncture. 5. The method of claim 4, further comprising: providing a shaft, said first needle and said second needle being advanceable along said shaft, said pre-tied knot being releasably attached to said shaft; and advancing said shaft through a tissue tract of a patient body. 6. The method of claim 5, wherein advancing the first end of the suture through the pre-tied knot includes pulling the first end of the suture with the first needle. 7. A method for closing an opening in tissue using a suturing device, the method comprising: inserting at least a portion of a suture through a tissue tract and through a tissue wall using a second needle; advancing a first end of the suture with a first needle through a pre-tied knot of the suture following inserting the at least a portion of the suture, the second needle, and the first needle through the tissue wall, the pre-tied knot being initially disposed about the first needle of the suturing device; and tensioning the first end of the suture and a second end of the suture adjacent the pre-tied knot to form a knot affixing a loop of the suture across the opening. 8. The method of claim 7, wherein the first needle comprises a distal tip, and wherein advancing said first end of said suture through said pre-tied knot includes pulling said suture with said distal tip of said first needle. 9. The method of claim 8, wherein said distal tip is a distal tissue penetrating tip. 10. The method of claim 9, wherein inserting the first needle through the tissue wall further comprises inserting the first needle through a first tissue tract and advancing the first end of the suture with the first needle through a pre-tied knot of the suture further comprises retracting the first needle through the first tissue tract. 11. The method of claim 10, wherein inserting the second needle through the tissue wall further comprises inserting the second needle through a second tissue tract. 12. The method of claim 11, wherein the first needle fixedly captures the first end of the suture. 13. The method of claim 12, wherein the first needle fixedly attaches to the first end of the suture. 14. The method of claim 13, wherein the second needle fixedly captures the first end of the suture. 15. The method of claim 14, wherein the second needle fixedly attaches to the first end of the suture. 16. The method of claim 15, wherein the first needle and the second needle substantially simultaneously fixedly attach to the suture. 17. The method of claim 16, wherein the first needle and the second needle substantially simultaneously fixedly attach to the suture via a suture link. 18. The method of claim 7, further comprising: providing a shaft of said suturing device, said first needle advanceable along said shaft, and pre-tied knot being releasably attached to said shaft; and advancing said shaft through a tissue tract of a patient body.
