A needleless injector comprising a body at a proximal end,a first shaft extending distally from the body,an injection shaft extending from the body to a distal end of the injection shaft and a shaft terminus, wherein the injection shaft is axially moveable along a longitudinal axis of the first shaft,an injection orifice at the distal end of the injection shaft in fluid communication with a fluid chamber at the proximal end, the injection orifice having a diameter in the range from about 0.001 to 0.05 inches, the injection orifice being located on the injection shaft at a proximal location relative to the shaft terminus, and the injection orifice being positioned for fluid ejection in a lateral direction relative to a longitudinal axis of the injection shaft,a pressure source in communication with the fluid chamber, andan extendable tissue tensioner located at the distal end of the injection shaft proximal to the injection orifice,wherein, the tissue tensioner is located at a length-wise position along the longitudinal axis of the injection shaft on the distal end that is the same as a length-wise position of the injection orifice such that when the distal end comprising the tissue tensioner and injection orifice is placed within a urethral lumen and the tissue tensioner is extended, tissue of the urethra is tensioned and the injector is capable of ejecting fluid from the injection orifice to penetrate tissue under tension. 2. The injector of claim 1, wherein the tissue tensioner comprises a vacuum. 3. The injector of claim 1, wherein the tissue tensioner comprises a balloon. 4. The injector of claim 1, comprising an injector head at the distal end of the injection shaft, the injector head comprising a flat surface, the flat surface comprising the injection orifice and a vacuum orifice. 5. The injector of claim 4, wherein the injector head comprises multiple injection orifices having diameters in the range from about 0.001 to 0.05 inches. 6. The injector of claim 1, wherein the tissue tensioner comprises a retractable tissue tensioner that retractably extends from the injection shaft away from the injection orifice. 7. The injector of claim 1, comprising an optical component comprising an eyepiece at the proximal end and an optical device extending from the eyepiece to a location at the distal end of the injection shaft. 8. The injector of claim 1, comprising a balloon at the distal end of the injection shaft, the balloon being located distal from the injection orifice relative to the proximal end. 9. The injector of claim 8, comprising an additional balloon located at the distal end of the injection shaft proximal from the injector orifice. 10. The injector of claim 1, comprising a flexible injection shaft. 11. The injection of claim 1, wherein the injector is pre-primed in a sterilized package. 12. A needleless injector comprising a body at a proximal end,a first shaft extending distally from the body,a flexible shaft extending from the body to a distal end of the flexible shaft having a shaft terminus, wherein the flexible shaft is axially moveable along a longitudinal axis of the first shaft,an injection orifice at the distal end of the flexible shaft in fluid communication with a fluid chamber at the proximal end, the injection orifice being located on the flexible shaft at a proximal location relative to the shaft terminus, and the injection orifice being, positioned for fluid ejection in a lateral direction relative to a longitudinal axis of the flexible shaft,an extendable tissue tensioner located at the distal end of the flexible shaft proximal to the injection orifice,a pressure source in communication with the fluid chamber, wherein the needleless injector is capable of ejecting fluid as a pressurized stream from the injection orifice so the pressurized stream of fluid can pass through a surface of tissue of a urinary tract and become dispersed within tissue of a lower urinary tract as droplets. 13. The injector of claim 12, comprising an optical device for viewing at a distal end of the injector. 14. The injector of claim 13, wherein the optical device comprises an eyepiece at the proximal end and flexible optical fiber extending from the eyepiece to a location at the distal end of the injection shaft. 15. The injector of claim 12, wherein the injector is pre-primed in a sterilized package. 16. The injector of claim 12, comprising a priming mechanism capable of filling an injection lumen by low pressure flow of fluid from the fluid chamber. 17. The injector of claim 16 wherein the body comprises a fluid reservoir,a syringe chamber,a pressure source, anda plunger adapted to the syringe chamber, the plunger cooperative with the pressure source for pressurizing the syringe chamber,wherein the priming mechanism produces low pressure flow by the pressure source moving the plunger to cause fluid to flow from the syringe chamber to the injection lumen. 18. The injector of claim 1 wherein the injection orifice is capable of ejecting fluid in a direction that includes a directional component away from the longitudinal axis of the injection shaft at the location of the ejection. 19. The injector of claim 1 wherein the injection orifice is moveable relative to the first shaft rotationally around a longitudinal axis or lengthwise along a length of the first shaft. 20. A needleless injector comprising a body at a proximal end,a first shaft extending distally from the body,an injection shaft extending from the body to a distal end of the injection shaft, wherein the injection shaft is axially moveable along a longitudinal axis of the first shaft,an injection orifice at the distal end of the injection shaft in fluid communication with a fluid chamber at the proximal end, the injection orifice directed away from the longitudinal axis of the injection shaft to provide for lateral fluid ejection from the injection orifice,a pressure source in communication with the fluid chamber, anda tissue tensioner located at the distal end of the injection shaft proximal to the injection orifice, wherein:the injection orifice ejects fluid in a direction that includes a directional component away from a longitudinal axis of the injection shaft at the location of the ejection,the tissue tensioner is located at a length-wise position on the distal end of the injection shaft that is the same as a length-wise position of the injection orifice such that the device is capable of ejecting fluid from the injection orifice to penetrate tissue under tension by the tissue tensioner, andthe needleless injector is capable of ejecting fluid from the injection orifice at a pressure to cause the fluid to pass through a tissue surface and become dispersed within tissue as droplets. 21. The injector of claim 20 wherein the injection orifice is moveable relative to the first shaft rotationally around a longitudinal axis or lengthwise along a length of the first shaft. 22. The injector of claim 20 wherein the injection orifice is directed away from the longitudinal axis of the injection shaft at a location of the injection orifice. 23. The injector of claim 1 wherein the pressure source is capable of pressurizing fluid within the fluid chamber to an injection pressure to cause the fluid to pass through a surface of a urethra and become dispersed transurethrally within prostate tissue when ejected from the injection orifice located within a urethra of a male patient. 24. The injector of claim 23 wherein the injection pressure is 2000 pounds per square inch or greater. 25. The injector of claim 24 wherein the injection orifice is directed away from a longitudinal axis of the injection shaft at the location of the injection orifice. 26. The injector of claim 24 wherein the injection shaft extends from the proximal end to a shaft terminus at the distal end,an injection lumen extends within the injection shaft from the proximal end to the distal end, to a location on a proximal side of the shaft terminus, andthe injection orifice is located on a proximal side of the shaft terminus. 27. The injector of claim 26 wherein the injection orifice is directed away from a tangent of a longitudinal axis of the injection shaft at the location of the injection orifice, andthe pressure source is capable of pressurizing fluid within the fluid chamber to an injection pressure to cause the fluid to pass through a surface of a urethra and become dispersed transurethrally within prostate tissue when ejected from the injection orifice located within a urethra of a male patient. 28. The injector of claim 27 wherein the tissue tensioner comprises an inflatable balloon. 29. The injector of claim 12 wherein the injection pressure is 2000 pounds per square inch or greater. 30. The injector of claim 12 wherein the injection pressure is 2000 pounds per square inch or greater, andwhen the distal end comprising the tissue tensioner and injection orifice is placed within a urethral lumen and the tissue tensioner is extended, tissue of the urethra is tensioned at a location of fluid ejected from the injection orifice. 31. The injector of claim 30 wherein the tissue tensioner comprises an inflatable balloon. 32. The injector of claim 20 wherein the tissue tensioner comprises an inflatable balloon. 33. The injector of claim 12 wherein the tissue of a urinary tract is tissue selected from a urethra, a bladder, and a prostate. 34. The injector of claim 1, wherein at least a portion of the length of the injection shaft is positioned within a lumen of the first shaft.
