There are disclosed various embodiments of surgical tacks for use in surgical procedures. The tacks generally include a head and a barrel portion extending distally from the head. Preferably the head and the barrel portion define a throughbore for receipt of a drive instrument. A thread on the head is provided to engage threads in the installation tool. A tissue thread is provided on the barrel portion to engage tissue. Distal and proximal surfaces of the tissue thread may be oriented at various angles relative to the barrel portion. There is also disclosed an insertion instrument to insert one or more tacks as well as a method of use. There is further disclosed a model device for use in explaining the operation of the instrument.

A surgical instrument for insertion of surgical tacks into tissue comprising: a handle portion;an elongate tubular portion extending distally from the handle portion;a rod mounted within the elongate tubular portion and having a plurality of surgical tacks received therealong, a distal end of each of the surgical tacks is tapered toward the distal end for forming an atraumatic transition with the tip of the rod for insertion of the tack; andan actuator system for rotating the tacks relative to the tubular portion, the rod having a pointed distal end projecting distally beyond the tubular portion during insertion to pierce tissue and facilitate initial insertion of the tack into tissue. 2. A surgical instrument according to claim 1, wherein each of the surgical tacks comprises a throughbore configured to receive the rod therethrough. 3. A surgical instrument according to claim 2, wherein the throughbore comprises a non-circular shape for a cross-section of the throughbore. 4. A surgical instrument according to claim 1, wherein the rod comprises a non-circular shape for a cross-section of the rod. 5. A surgical instrument according to claim 1, wherein the elongate tubular portion includes an inner thread. 6. A surgical instrument according to claim 5, wherein the inner thread of the elongate tubular portion is configured to engage a drive thread on each of the plurality of surgical tacks. 7. A surgical instrument according to claim 6, wherein a distal end of the inner thread is positioned flush with a distal end of the elongate tubular portion to facilitate reengagement of the inner thread with the drive thread of the surgical tacks after installation. 8. A surgical instrument according to claim 6, wherein the inner thread of the elongate tubular portion extends at least partially along the length of the elongate tubular portion. 9. A surgical instrument according to claim 6, wherein the inner thread is integrally formed with the elongated tubular portion. 10. A surgical instrument according to claim 1, further including a spring configured to bias the surgical tacks distally. 11. A surgical instrument configured to apply tacks to hernia mesh and tissue, the surgical instrument comprising: a handle portion;an elongate tubular portion extending distally from the handle portion;a drive rod rotatably mounted within the elongate tubular portion;an actuator operatively associated with the drive rod such that actuation of the actuator rotates the drive rod relative to the tubular portion; anda plurality of surgical tacks, wherein a distal section of the drive rod is sufficiently elongate so as to receive multiple surgical tacks therealong and the distal section of the drive rod has a pointed distal end to facilitate initially piercing tissue, a distal end of each of the surgical tacks is tapered toward the distal end for forming an atraumatic transition with the tip of the rod to avoid traumatizing the tissue and damaging the mesh. 12. A surgical instrument according to claim 11, wherein the surgical tacks comprise a throughbore configured to receive the drive rod therethrough. 13. A surgical instrument according to claim 12, wherein the throughbore comprises a non-circular shape for a cross-section of the throughbore. 14. A surgical instrument according to claim 11, wherein the elongate tubular portion has a thread at least partially along an inner surface thereof configured to engage a drive thread formed on an outer surface of the surgical tacks. 15. A surgical instrument according to claim 11, wherein the surgical tacks comprise a head portion and a barrel portion, the barrel portion having a thread formed on an outer surface thereof that is engageable with tissue. 16. A surgical instrument comprising: a handle portion;an elongate tubular portion extending distally from the handle portion;a drive rod rotatably mounted within the elongate tubular portion;an actuator operatively associated with the drive rod such that actuation of the actuator rotates the drive rod relative to the tubular portion; anda plurality of surgical tacks, wherein a distal section of the drive rod is sufficiently elongate so as to receive multiple surgical tacks therealong and the distal section of the drive rod has a pointed distal end to facilitate initially piercing tissue, each surgical tack include a head portion and a barrel portion, the barrel portion having a thread formed on an outer surface thereof that is engageable with tissue. 17. A surgical instrument according to claim 16, wherein the surgical tacks comprise a throughbore configured to receive the drive rod therethrough. 18. A surgical instrument according to claim 17, wherein the throughbore comprises a non-circular shape for a cross-section of the throughbore. 19. A surgical instrument according to claim 16, wherein the elongate tubular portion has a thread at least partially along an inner surface thereof configured to engage a drive thread formed on an outer surface of the surgical tacks.
