Biomarkers are collected and used to determine biological propensities of a patient, to determine the efficacy of medical devices, to select and administer therapeutic agents, to select medical devices, to make adjustments to medical devices, and/or to adjust surgical techniques. An apparatus includes a port to draw a biological fluid (e.g., a mist) from a surgical site. The apparatus includes a sensor having a cantilevered beam. The beam includes substances selected to attract certain biomarkers as the fluid is communicated across the beam. The same apparatus or another apparatus is used to administer a therapeutic agent based at least in part on collected biomarker data. The therapeutic agent delivery apparatus may include a device that is also used to create a wound at a surgical site. For instance, a harmonic surgical instrument may be used to both collect biomarkers and administer a therapeutic agent (e.g., gene therapy using sonoporation).

The apparatus of claim 1, wherein the biomarker collection assembly comprises a suction pump, wherein the suction pump is integral with the handpiece. 3. The apparatus of claim 1, wherein the biomarker collection assembly comprises a suction pump, wherein the end effector further comprises an active element operable to create a wound in tissue when the active element is activated, wherein the suction pump is configured to create suction in the conduit in response to activation of the active element. 4. The apparatus of claim 3, wherein the active element comprises a ultrasonic blade. 5. The apparatus of claim 1, wherein the biomarker collection assembly comprises a capture vessel removably coupled with the handpiece, wherein the capture vessel is configured to store biomarkers drawn in through the conduit. 6. The apparatus of claim 1, wherein the one or more substances comprise one or more of an antibody or an amino acid. 7. The apparatus of claim 1, wherein the biomarker collection assembly further includes one or both of a communication port or a graphical user interface configured to communicate biomarker data. 8. The apparatus of claim 1, wherein the beam is rotatable about the beam shaft, wherein the beam shaft is rotatably fixed within the conduit such that the beam shaft is non-rotatable relative to the conduit. 9. The apparatus of claim 1, wherein the beam is rotatably fixed to the beam shaft such that the beam is non-rotatable relative to the beam shaft, wherein the beam shaft is rotatable within the conduit. 10. An apparatus, comprising: (a) a body;(b) a shaft extending distally from the body, wherein the shaft includes a distal end;(c) a conduit extending through the shaft;(d) an end effector positioned at the distal end of the shaft, wherein the end effector comprises a ultrasonic blade, wherein the end effector is in fluid communication with the conduit, wherein the end effector further comprises a clamp pad, wherein the clamp pad is pivotable relative to the ultrasonic blade, wherein the clamp pad comprises one or more openings in fluid communication with the conduit, wherein the one or more openings comprises one or more variable orifices, wherein each orifice is configured to transition between a closed state and an open state in response to a control signal; and(e) a therapeutic agent administration assembly in fluid communication with the conduit, wherein the therapeutic agent administration assembly is operable to administer a therapeutic agent through the end effector. 11. The apparatus of claim 10, wherein the ultrasonic blade includes at least one opening in fluid communication with the conduit. 12. The apparatus of claim 10, wherein the ultrasonic blade includes at least one recess in fluid communication with the conduit. 13. The apparatus of claim 10, further comprising a sheath disposed about a portion of the ultrasonic blade, wherein the sheath and the ultrasonic blade together selectively open a gap in fluid communication with the conduit when the ultrasonic blade is in an activated state, wherein the gap is closed when the ultrasonic blade is in a non-activated state. 14. The apparatus of claim 10, wherein each opening is associated with a respective cell, wherein each cell defines a reservoir containing the therapeutic agent, wherein each cell is configured to drive the therapeutic agent from the cell and out through the corresponding opening in response to a control signal. 15. The apparatus of claim 10, wherein the body comprises a handpiece. 16. An apparatus, comprising: (a) a body;(b) a shaft extending distally from the body, wherein the shaft includes a distal end;(c) an end effector positioned at the distal end of the shaft, wherein the end effector comprises an ultrasonic blade;(d) a biomarker collection assembly in fluid communication with the end effector, wherein the biomarker collection assembly is configured to draw in biomarkers associated with one or more conditions including one or both of thermal damage to tissue or coagulation of blood through the end effector and store or process the biomarkers; and(e) a biomarker response assembly, wherein the biomarker response assembly is operable to change one or both of a frequency of energy applied through the end effector or a power level of energy applied through the end effector, based at least in part on biomarker data from the biomarker assembly associated with one or more conditions including one or both of thermal damage to tissue or coagulation of blood. 17. The apparatus of claim 16, further comprising a conduit extending through the shaft, wherein the conduit is in fluid communication with one or both of the biomarker collection assembly or the biomarker response assembly, wherein the end effector further comprises a port in fluid communication with the conduit, wherein the port is positioned adjacent to the ultrasonic blade.
