Cardiac signal separation is employed to detect, monitor, track and/or trend ischemia using cardiac activation sequence information. Ischemia detection may involve sensing composite cardiac signals using implantable electrodes, and performing a signal separation that produces one or more cardiac activation signal vectors associated with one or more cardiac activation sequences. A change in the signal vector may be detected using subsequent separations. The change may be an elevation or depression of the ST segment of a cardiac cycle or other change indicative of myocardial ischemia, myocardial infarction, or other pathological change. The change may be used to predict, quantify, and/or qualify an event such as an arrhythmia, a myocardial infarction, or other pathologic change. Information associated with the vectors may be stored and used to track the vectors.

A system, comprising: a housing configured for implantation in a patient;a plurality of implantable electrodes configured for sensing a plurality of composite signals associated with an unknown number of signal sources;a memory provided in the housing and configured to store a baseline angular orientation of a cardiac signal vector representative of the patient's baseline ischemia status; anda processor provided in the housing and coupled to the electrodes and the memory, the processor configured to: open a window associated with an ST-T segment within which the plurality of composite signals are sensed for each of a plurality of cardiac cycles;perform recurring source separation operations by which the plurality of composite signals sensed within the window are separated according to their respective sources to produce windowed cardiac signal vectors indicating a dominant angular orientation of repolarization wavefronts over the plurality of cardiac cycles; anddetect a deviation in the dominant angular orientation of the windowed cardiac signal vectors relative to the baseline angular orientation as an indication of a change in the patient's ischemia status relative to the patient's baseline ischemia status. 2. The system of claim 1, wherein the processor is configured to center the window on the ST-T segment of each of the plurality of cardiac cycles. 3. The system of claim 1, wherein the processor is configured to determine a change in one or more ST-T segment features of the plurality of composite signals sensed within the window indicative of the change in the patient's ischemia status. 4. The system of claim 1, wherein the processor is configured to determine a change in one or more ST-T segment features of the plurality of composite signals sensed within the window indicative of an elevation or a depression of the ST-T segment. 5. The system of claim 1, wherein: the memory is configured to store a baseline magnitude of a cardiac signal vector representative of the patient's baseline ischemia status; andthe processor is configured to detect a deviation in angular orientation and a deviation in magnitude of the windowed cardiac signal vectors relative to the baseline angular orientation and baseline magnitude, respectively, as an indication of the change in the patient's ischemia status relative to the patient's baseline ischemia status. 6. The system of claim 1, wherein the processor is configured to predict myocardial infarction based on the detected deviation in angular orientation. 7. The system of claim 1, wherein the processor is configured to implement a confirmation procedure to confirm that candidate windowed cardiac signal vectors comprise a cardiac signal. 8. The system of claim 1, wherein the processor is configured to: implement a first confirmation procedure that attempts to confirm that the windowed cardiac signal vectors comprise a cardiac signal; andif the first confirmation procedure is unsuccessful, implement a second confirmation procedure to that attempts to confirm that the windowed cardiac signal vectors comprise a cardiac signal, the second confirmation procedure differing from the first confirmation procedure. 9. A system, comprising: a plurality of electrodes configured for sensing a plurality of composite signals associated with an unknown number of signal sources including physiologic and environmental noise sources;a memory configured to store baseline data of a cardiac signal vector representative of the patient's baseline ischemia status; anda processor coupled to the electrodes and the memory, the processor configured to: open a window associated with an ST-T segment within which the plurality of composite signals are sensed for each of a plurality of cardiac cycles;perform recurring source separation operations by which the plurality of composite signals sensed within the window are separated according to their respective sources to produce windowed cardiac signal vectors indicating a feature of repolarization wavefronts over the plurality of cardiac cycles; anddetect a deviation in the repolarization wavefront feature of the windowed cardiac signal vectors relative to the baseline data as an indication of a change in the patient's ischemia status relative to the patient's baseline ischemia status. 10. The system of claim 9, wherein the deviation in the repolarization wavefront feature detected by the processor comprises one or more of a phase angle change of the windowed cardiac signal vectors, a magnitude change of the windowed cardiac signal vectors, a variance change of the windowed cardiac signal vectors, a rate of change of phase angle of the windowed cardiac signal vectors, a rate of change of magnitude of the windowed cardiac signal vectors, and a rate of change of variance of the windowed signal vectors. 11. The system of claim 9, wherein the deviation in the repolarization wavefront feature detected by the processor comprises one or more of a power spectral density change of a phase of the windowed cardiac signal vectors and a power spectral density change of a magnitude of the windowed cardiac signal vectors. 12. The system of claim 9, wherein the deviation in the repolarization wavefront feature detected by the processor comprises one or more of a trajectory change of the windowed cardiac signal vectors, a temporal profile change of the windowed cardiac signal vectors, and a rate of change of a temporal profile of the windowed cardiac signal vectors. 13. The system of claim 9, wherein the deviation in the repolarization wavefront feature detected by the processor comprises one or more of a trend of a phase angle of the windowed cardiac signal vectors, a trend of a magnitude of the windowed cardiac signal vectors, a trend of a variance of the windowed cardiac signal vectors, and a trend of a temporal profile of the windowed cardiac signal vectors. 14. The system of claim 9, wherein the processor comprises a processor of a patient-external device or system. 15. The system of claim 9, wherein the processor comprises a processor of a remote networked device or system. 16. The system of claim 9, wherein the processor is configured to predict myocardial infarction based on the detected deviation in the repolarization wavefront feature. 17. The system of claim 9, wherein the processor is configured to implement a confirmation procedure to confirm that candidate windowed cardiac signal vectors comprise a cardiac signal. 18. The system of claim 9, wherein the processor is configured to: implement a first confirmation procedure that attempts to confirm that the windowed cardiac signal vectors comprise a cardiac signal; andif the first confirmation procedure is unsuccessful, implement a second confirmation procedure to that attempts to confirm that the windowed cardiac signal vectors comprise a cardiac signal, the second confirmation procedure differing from the first confirmation procedure. 19. A system, comprising: a plurality of electrodes configured for sensing a plurality of composite signals associated with an unknown number of signal sources including physiologic and environmental noise sources;a memory; anda processor coupled to the electrodes and the memory, the processor configured to: open a first window associated with a P-QRS segment within which the plurality of composite signals are sensed for each of a plurality of cardiac cycles;open a second window associated with an ST-T segment within which the plurality of composite signals are sensed for each of the plurality of cardiac cycles;perform recurring source separation operations by which the plurality of composite signals sensed within the first and second windows are separated according to their respective sources to produce first and second windowed cardiac signal vectors representing a dominant angular orientation of respective depolarization and repolarization wavefronts over the plurality of cardiac cycles;determine a relationship between the angles of the first windowed vectors relative to the second windowed vectors; anddetect a change in the patient's ischemia status based on the relationship between the angles of the first windowed vectors relative to the second windowed vectors. 20. The system of claim 19, wherein the processor is configured to: determine whether or not the ST-T segment vectors lie at angles greater than those of the P-QRS segment vectors; anddetect a change in the patient's ischemia status based on the determination. 21. The system of claim 19, wherein the processor is configured to: determine whether the ST-T segment vectors lie at angles less than those of the P-QRS segment vectors; anddetect worsening of the patient's ischemia status in response to determining that the ST-T segment vectors lie at angles less than those of the P-QRS segment vectors. 22. The system of claim 19, wherein the processor is configured to implement a confirmation procedure to confirm that candidate windowed cardiac signal vectors comprise a cardiac signal.
