Implants are disclosed which employ various means for adjusting and maintaining the size of an orifice to which they are attached. Systems permit the implants to be implanted using minimally invasive procedures and permit final adjustments to the circumference of the implants after the resumption of normal flow of anatomic fluids in situ. Methods are disclosed for using the implants to treat heart valve abnormalities, gastroesophageal abnormalities, anal incontinence, and the like.

The adjustable implantable device of claim 1, wherein the annular dimension can be adjusted to provide leaflet coaptation of an anatomic orifice. 3. The adjustable implantable device of claim 1, wherein the adjustable member is configured to non-uniformly adjust an annular dimension of the prosthetic implant device. 4. The adjustable implantable device of claim 1, wherein said prosthetic implant device further comprises a tube formed into an annular shape and wherein said prosthetic implant device is attached to the tissue adjacent the anatomic orifice or lumen by securing said tube directly or indirectly to said tissue. 5. The adjustable implantable device of claim 1, wherein the said adjustment tool comprises a distal end and a proximal end, said distal end being removably coupled to the adjustable member and extending therefrom such that adjustments to the prosthetic implant device can be made from the proximal end. 6. The adjustable implantable device of claim 5, wherein said adjustments of the prosthetic implant device can be performed after resumption of physiologic flow in the annulus or lumen. 7. The adjustable implantable device of claim 5, wherein said adjustment tool provides direct circuitry to effect adjustment of the adjustable prosthetic implant device. 8. The adjustable implantable device of claim 5, wherein said adjustment tool provides electronic conduit leads to effect adjustment of the adjustable prosthetic implant device. 9. The adjustable implantable device of claim 1, wherein said adjustment tool provides electronic conduit leads to activate adjustment of the prosthetic implant device. 10. The adjustable implantable device of claim 1, wherein a distal end of the adjustment tool is mechanically attached to an interface location of the implant device. 11. The adjustable implantable device of claim 1, wherein the adjustable member includes an assembly operative for adjusting the prosthetic implant device from the first dimension to the second dimensions by rotation of the adjustment tool when connected thereto. 12. The adjustable implantable device of claim 1, wherein the implant device comprises an annuloplasty ring. 13. An adjustable implantable device for an anatomic orifice or lumen in a mammalian body comprising: an implantable implant body comprising a prosthetic implant device having a rotatable adjustable member operative to adjust the annular dimension or shape of the implant body over a functional range when attached to an adjustment tool, the adjustable member including a first gear having a first rotational axis and a second gear having a second rotational axis engaged with the first gear, wherein the first rotational axis traverses the second rotational axis, the implant body configured to be attached to tissue within a mammalian body adjacent the anatomic orifice or lumen; andan adjustment tool having a proximal end and a distal end, the distal end configured to be releasably attached to the adjustable member for rotation of the first and second gears when received intracorporeal of the mammalian body with the proximal end arranged extracorporeal of the mammalian body;wherein the distal end of the adjustment tool is remotely operative when attached to the adjustable member to initiate adjustment of the implant body by rotation of the adjustable member, in situ, over the functional range while the proximal end of the adjustment tool is arranged extracorporeal. 14. The adjustable implantable device of claim 13, wherein the functional range is operative to provide leaflet coaptation of an anatomic orifice. 15. The adjustable implantable device of claim 13, wherein said implant body further comprises a tube formed into an annular shape and wherein said implant body is attached to the tissue adjacent the anatomic orifice or lumen by securing said tube directly or indirectly to said tissue. 16. The adjustable implantable device of claim 13, wherein the adjustment tool is remotely operative by engagement of the proximal end for adjusting the implant body over the functional range. 17. The adjustable implantable device of claim 13, wherein the adjustment tool comprises an elongated member. 18. The adjustable implantable device of claim 13, wherein the adjustable implant body comprises a plurality of corrugated sections. 19. The adjustable implantable device of claim 13, wherein the adjustable member includes an assembly operative for adjusting the prosthetic implant device over the functional range by rotation of the adjustment tool when connected thereto. 20. The adjustable implantable device of claim 13, wherein the implant device comprises an annuloplasty ring. 21. An adjustable implantable device for an anatomic orifice or lumen in a mammalian body, comprising: an adjustable implant body having an adjustable annular dimension, the implant body including a rotatable adjustable member adapted to adjust the annular dimension of the implant body from a first dimension to one of a range of second dimensions upon rotation when attached to an adjustment tool, the adjustable member including a first gear having a first rotational axis and a second gear having a second rotational axis engaged with the first gear, wherein the first rotational axis traverses the second rotational axis, the implant body configured to be attached to tissue within a mammalian body adjacent the anatomic orifice or lumen; andan adjustment tool configured to activate adjustment of the implant body from the first dimension to one of the range of second dimensions by rotation of the first and second gears of the adjustable member, the adjustment tool having an end configured for releasable attachment to the adjustment member;wherein, when the adjustment tool is attached within a mammalian body to the adjustable member of the implant body, the adjustment tool being operative by rotation thereof to initiate adjustment of the annular dimension of the implant body over the range from the first dimension to the second dimensions in situ; andwherein the adjustment tool is detachable from the adjustable member after adjustment of the implant body in situ and is withdrawable from within the mammalian body. 22. The adjustable implantable device of claim 21, wherein the implant body comprises an annuloplasty ring. 23. An adjustable implantable device for an anatomic orifice or lumen in a mammalian body comprising: an adjustable implant body having a rotatable adjustable member operative to adjust an annular dimension of the implant body from a first dimension to one of a range of second dimensions, the adjustable member including a first gear having a first rotational axis and a second gear having a second rotational axis engaged with the first gear, wherein the first rotational axis traverses the second rotational axis, the implant body configured to be attached to tissue within a mammalian body adjacent the anatomic orifice or lumen; andan adjustment tool having a proximal end and a distal end, the distal end configured to be releasably attached to the adjustable member for rotation of the first and second gears;wherein the distal end of the adjustment tool is operative to initiate adjustment of the implant body by rotation of the adjustable member, in situ, over the range of second dimensions when arranged intracorporeal of the mammalian body while the proximal end of the adjustment tool is arranged extracorporeal. 24.
