In certain embodiments, a vial adaptor for removing liquid contents from a vial comprises a housing and a bag. In some embodiments, an initial volume of a regulating fluid can be contained within the bag. In some embodiments, the regulating fluid flows into the vial as liquid is removed from the vial via the adaptor, thereby regulating pressure within the vial. In some embodiments, a vial adaptor is coupled with the vial in order to remove the liquid. In some embodiments, as the liquid is removed from the vial via the adaptor, the bag expands, and in other embodiments, the bag contracts.

The adaptor of claim 1, wherein the first and second filters are configured to filter air passing between the regulator channel and ambient. 10. The adaptor of claim 1, wherein the regulator channel is configured to permit ambient air to flow into the regulator channel as medicinal fluid is withdrawn from the vial, thereby equalizing pressure within the vial. 11. The adaptor of claim 1, wherein the port further comprises a one-way valve configured to permit ambient air to flow into the regulator channel and to prevent fluid from flowing out of the regulator channel to ambient. 12. The adaptor of claim 1, further comprising a medical connector interface configured to couple the adaptor with a medical device, wherein an interior of the medical device is in fluid communication with the extractor channel when the medical device is coupled to the medical connector interface. 13. The adaptor of claim 1, further comprising a regulating enclosure portion comprising an interior volume in fluid communication with the regulator channel, the regulating enclosure portion configured to regulate pressure within the vial when fluid is withdrawn from the vial while the adaptor is coupled with the vial. 14. The adaptor of claim 13, wherein the regulating enclosure portion is positioned outside the vial when the adaptor is coupled with the vial. 15. The adaptor of claim 1, wherein the piercing member comprises a substantially cylindrical sheath. 16. The adaptor of claim 1, wherein the piercing member comprises a tip configured to facilitate insertion of the piercing member into the vial. 17. The adaptor of claim 1, wherein the second filter is capable of prohibiting the passage of liquids or vapors. 18. The adaptor of claim 1, wherein the second filter is capable of preventing liquids or vapors from passing from the vial to the exterior of the adaptor. 19. The adaptor of claim 1, wherein the first and second filters are capable of preventing liquids or vapors from passing from the vial to the exterior of the adaptor. 20. The adaptor of claim 1, wherein the first and second filters prevent liquid from exiting the adaptor. 21. A method of withdrawing medicinal fluid from a vial having an interior filled with medicinal fluid, the method comprising: providing a pressure regulating adaptor for coupling with the vial, the adaptor comprising: a housing portion configured to couple with the vial, the housing portion comprising at least a portion of a regulator channel configured to permit regulating fluid to pass therethrough and at least a portion of an extractor channel configured to permit medicinal fluid to pass therethrough;a piercing member that includes a regulator aperture and through which at least a portion of the regulator channel extends;a port disposed between the regulator channel and ambient;a first filter in fluid communication with the interior of the vial when the adaptor is coupled with the vial, wherein the first filter is configured to filter ambient air passing through the port; anda second filter in fluid communication with the regulator channel, wherein the second filter is connected to the housing portion and spatially separated from the first filter, and wherein the second filter is separated from the interior of the vial by at least one membrane when the adaptor is coupled with the vial;coupling the pressure regulating adaptor to the vial such that the extractor channel is in fluid communication with an interior of the vial; andwithdrawing medicinal fluid from the vial. 22. A method of manufacturing a pressure regulating adaptor for coupling with a vial having an interior filled with medicinal fluid, the method comprising: providing a pressure regulating adaptor comprising a housing portion configured to couple with the vial, the housing portion comprising at least a portion of a regulator channel configured to permit regulating fluid to pass therethrough and at least a portion of an extractor channel configured to permit medicinal fluid to pass therethrough, wherein the adaptor further comprises a port disposed between the regulator channel and ambient;providing a first filter;connecting the first filter to the housing portion, wherein the first filter is in fluid communication with the interior of the vial when the adaptor is coupled with the vial, and wherein the first filter filters air passing through the port;providing a second filter; andconnecting the second filter to the housing portion, wherein the second filter is in fluid communication with the regulator channel, and wherein the second filter is spatially separated from the first filter. 23.
