A CRM system enhances intracardiac electrogram-based arrhythmia detection using a wireless electrocardiogram (ECG), which is a signal sensed with implantable electrodes and approximating a surface ECG. In one embodiment, an intracardiac electrogram allows for detection of an arrhythmia, and the wireless ECG allows for classification of the detected arrhythmia by locating its origin. In another embodiment, the wireless ECG is sensed as a substitute signal for the intracardiac electrogram when the sensing of the intracardiac electrogram becomes unreliable. In another embodiment, a cardiac signal needed for a particular purpose is selected from one or more intracardiac electrograms and one or more wireless ECGs based on a desirable signal quality. In another embodiment, intracardiac electrogram-based arrhythmia detection and wireless ECG-based arrhythmia detection confirm with each other before indicating a detection of arrhythmia of a certain type.

A cardiac rhythm management (CRM) system, comprising: an implantable medical device including: a sensing circuit including an electrogram sensing circuit to sense one or more intracardiac electrograms and a wireless electrocardiogram (ECG) sensing circuit to sense one or more subcutaneous ECGs;a processing circuit to receive and process at least one signal selected from the one or more intracardiac electrograms and the one or more subcutaneous ECGs based on a selection signal; anda signal selection circuit coupled to the processing circuit, the signal selection circuit including a signal quality assessment circuit and a selection signal generator, the signal quality assessment circuit adapted to produce quality parameters by analyzing each of the one or more intracardiac electrograms and the one or more subcutaneous ECGs during a specified cardiac rhythm including a normal sinus rhythm or a cardiac arrhythmia, the quality parameters each being a measure of quality of one cardiac signal of the one or more intracardiac electrograms and the one or more subcutaneous ECGs, the quality parameters including at least a first quality parameter generated by analyzing morphology of one of the one or more intracardiac electrograms and at least a second quality parameter generated by analyzing morphology of one of the one or more subcutaneous ECGs, the selection signal generator adapted to produce the selection signal based on at least a comparison between the at least the first quality parameter and the at least the second quality parameter; anda plurality of implantable subcutaneous electrodes coupled to the sensing circuit. 2. The system of claim 1, wherein the plurality of implantable subcutaneous electrodes comprises at least one electrode incorporated onto the implantable medical device. 3. The system of claim 1, wherein the one or more intracardiac electrograms comprises at least one atrial electrogram, and further comprising at least one lead including one or more atrial electrodes, coupled to the sensing circuit, for sensing the at least one atrial electrogram. 4. The system of claim 1, wherein the one or more intracardiac electrograms comprises at least one ventricular electrogram, and further comprising at least one lead including one or more ventricular electrodes, coupled to the sensing circuit, for sensing the at least one ventricular electrogram. 5. The system of claim 1, wherein the signal quality assessment circuit comprises a signal-to-noise ratio measurement circuit to measure a signal-to-noise ratio of each cardiac signal of the one or more intracardiac electrograms and the one or more subcutaneous ECGs. 6. The system of claim 1, wherein the signal selection circuit further comprises a command receiver to receive an external command, wherein the selection signal generator is adapted to produce the selection signal based on at least one of the quality parameters and the external command. 7. The system of claim 6, further comprising an external system communicatively coupled to the implantable medical device, the external system including a user input to receive the external command. 8. The system of claim 1, wherein the processing circuit comprises: an arrhythmia detection circuit to detect an episode of arrhythmia; anda morphology-based arrhythmia classification circuit, coupled to the arrhythmia detection circuit, to classify the arrhythmia based on a template morphology represented by template parameters associated with morphological features detected from the at least one signal selected from the one or more intracardiac electrograms and the one or more subcutaneous ECGs during a known rhythm. 9. The system of claim 8, wherein the morphology-based arrhythmia classification circuit comprises a template generator including: a morphological feature detector to detect the morphological features;a morphological parameter generator, coupled to the morphological feature detector, to produce the morphological parameters; anda template parameter generator, coupled to the morphological feature detector, to produce the template parameters. 10. The system of claim 8, wherein the arrhythmia detection circuit includes an atrial tachycardia detector to detect an episode of atrial tachycardia, and the morphology-based arrhythmia classification circuit is adapted to confirm each atrial tachycardia detection based on a template morphology represented by template parameters associated with morphological features detected from the at least one signal selected from the one or more intracardiac electrograms and the one or more subcutaneous ECGs during one of a normal sinus rhythm and a known atrial tachycardia. 11. The system of claim 8, wherein the arrhythmia detection circuit includes an ventricular tachycardia detector to detect an episode of ventricular tachycardia, and the morphology-based arrhythmia classification circuit is adapted to confirm each ventricular tachycardia detection based on a template morphology represented by template parameters associated with morphological features detected from the at least one signal selected from the one or more intracardiac electrograms and the one or more subcutaneous ECGs during one of a normal sinus rhythm and a known ventricular tachycardia. 12. The system of claim 8, wherein the arrhythmia detection circuit includes an ventricular tachycardia detector to detect an episode of ventricular tachycardia, and the morphology-based arrhythmia classification circuit is adapted to classify each detected ventricular tachycardia as one of ventricular tachycardia and supraventricular tachycardia based on a template morphology represented by template parameters associated with morphological features detected from the at least one signal selected from the one or more intracardiac electrograms and the one or more subcutaneous ECGs during a known ventricular tachycardia. 13. A method for operating a cardiac rhythm management (CRM) system, the method comprising: sensing a plurality of cardiac signals using an implantable medical device and a plurality of implantable subcutaneous electrodes, the cardiac signals including one or more intracardiac electrograms and one or more subcutaneous electrocardiograms (ECGS) during a specified cardiac rhythm including a normal sinus rhythm or a cardiac arrhythmia:producing quality parameters by analyzing each of the one or more intracardiac electrograms and the one or more subcutaneous ECGs, the quality parameters each being a measure of quality of one cardiac signal of the plurality of cardiac signals using the implantable medical device, the quality parameters including at least a first quality parameter generated by analyzing morphology of one of the one or more intracardiac electrograms and at least a second quality parameter generated by analyzing morphology of one of the one or more subcutaneous ECGs;selecting at least one cardiac signal from the plurality of cardiac signals based on at least a comparison between the at least the first quality parameter and the at least the second quality parameter using the implantable medical device; anddetecting an arrhythmia based on the selected at least one cardiac signal using the implantable medical device. 14. The method of claim 13, wherein producing the quality parameters comprises measuring a signal-to-noise ratio of each cardiac signal of the plurality of cardiac signals, and selecting the at least one cardiac signal further comprises selecting at least one cardiac signal from the plurality of cardiac signals based on at least the signal-to-noise ratios. 15. The method of claim 14, further comprising receiving an external command transmitted to the implantable medical device from an external programmer, wherein selecting the at least one cardiac signal further comprises selecting at least one cardiac signal from the plurality of cardiac signals based on at least one of the quality parameters and the external command. 16. The method of claim 13, further comprising classifying the detected arrhythmia based on the selected at least one cardiac signal. 17. The method of claim 16, wherein classifying the detected arrhythmia comprises: detecting predetermined morphological features from the selected at least one cardiac signals during the detected arrhythmia;producing arrhythmic morphological parameters associated with the predetermined morphological features detected during the detected arrhythmia; andclassifying the detected arrhythmia based on the arrhythmic morphological parameters. 18. The method of claim 17, further comprising: detecting the predetermined morphological features from the selected one or more cardiac signals during a known rhythm; andproducing template morphological parameters associated with the predetermined morphological features detected during the known rhythm,wherein classifying the detected arrhythmia comprises comparing the arrhythmic morphological parameters to the template morphological parameters. 19. The method of claim 17, wherein selecting the at least one cardiac signal comprises selecting at least one of the one or more intracardiac electrograms and at least one of the one or more subcutaneous ECGs. 20. The method of claim 17, wherein classifying the detected arrhythmia comprises classifying the detected arrhythmia as one of a supraventricular tachycardia and a ventricular tachycardia.
