An excision guide is positioned on an articular surface, wherein the excision guide includes a guide head and a guide sleeve disposed on the guide head, wherein the guide head includes a contact surface configured to locate the excision guide relative to the articular surface. A guide pin may be advanced through the guide sleeve and secured to the articular surface, wherein the guide sleeve is configured to receive the guide pin therethrough and position the guide pin at an angle β relative to an axis generally normal and central to a defect on the articular surface, wherein angle β is less than 90 degrees.

A system for repairing a defect on a portion of an articular surface of a patient's glenoid, said system comprising: a guide pin to be secured into said articular surface of said glenoid proximate to said defect:an excision device comprising: a cannulated shaft to be advanced over and rotated about said guide pin; andone or more cutters having a generally arcuate shape to form a generally hemi-spherical primary excision site in a portion of said glenoid articular surface having a diameter generally corresponding to a diameter of an implant, wherein all of said one or more cutters extend outward from said cannulated shaft and are generally aligned in a single plane extending along a longitudinal axis of said cannulated shaft; anda guide including: a guide arm; anda guide head including: an upper portion and a lower portion; anda passageway including at least one interior surface, said at least one interior surface extending through said guide head from said upper portion to said lower portion at an angle β less than 90 degrees relative to an axis generally normal and central to said guide head, wherein said at least one interior surface is to receive said guide pin and extends through said guide head such that an entry point of said guide pin into said glenoid articular surface is offset from said axis generally normal and central to said guide head and such that said excision device, when advanced over said guide pin, forms said generally hemispherical primary excision site having a center that is generally coaxial with a center of said guide head and said axis. 2. The system of claim 1, wherein said passageway extends to periphery of the guide head. 3. The system of claim 1, wherein said guide head is releasably coupled to said guide arm. 4. The system of claim 1, wherein said guide head has a height Ht that corresponds to a height H of said implant configured to be received in said primary excision site. 5. The system of claim 1, wherein said guide head has a radius Rt that corresponds to a radius Ri of said implant configured to be received in said primary excision site. 6. The system of claim 1, further comprising an impact device comprising a proximal end and a distal end, wherein the proximal end includes a striking surface and said distal end is configured to be received in and extend through an impact slot extending through said guide head from said upper portion to said lower portion. 7. The system of claim 6, wherein said impact device includes a chisel. 8. The system of claim 6, wherein said impact slot is configured to arrange said impact device at an angle γ relative to the guide arm, wherein angle γ is in the range 0 degrees to 45 degrees. 9. The system of claim 6, wherein said proximal end of said impact device is configured to be disposed generally parallel to said guide arm when distal end of said impact device is received in said impact slot. 10. The system of claim 1, further comprising an impact slot disposed generally between said passageway and said guide arm, said impact slot defining an opening through said guide head from said upper portion to said lower portion. 11. The system of claim 1, wherein said passageway further includes a guide sleeve defining said at least one interior surface, said guide sleeve configured to receive said guide pin therethrough. 12. The system of claim 1, wherein an interior surface of said lower portion of said guide is recessed relative to a periphery of said guide head. 13. The system of claim 1, wherein at least a portion of said lower portion of said guide is configured to contact said primary excision site. 14. The system of claim 1, further comprising said implant configured to be received within said primary excision site centered about said axis such that at least a portion of a load bearing surface of said implant is generally flush with a remaining portion of said glenoid articular surface at least partially surrounding said implant. 15. The system of claim 14, wherein said guide head has a height Ht that corresponds to a height H of said implant. 16. The system of claim 14, wherein said guide head has a radius Rt that corresponds to a radius Ri of said implant. 17. The system of claim 14, wherein said one or more cutters have a radius that corresponds to a radius Ri of said implant.
