The bone anchor receiver of claim 1, further comprising a closure. 3. The bone anchor receiver of claim 2, wherein the closure and the tool attachment structure are configured such that when a tool is engaged with the tool attachment structure, the closure is insertable into the channel. 4. The bone anchor receiver of claim 3, wherein when the closure is inserted and the tool is removed, the receiver top surface and the upright arm lateral side outwardly facing surfaces remain uncovered. 5. The bone anchor receiver of claim 2, wherein the closure has a mating guide and advancement structure on an exterior surface thereof configured to directly mate with a guide and advancement structure on the upright arms. 6. The bone anchor receiver of claim 2, wherein the closure is configured to directly contact the rod and lock the rod in the rod-receiving channel. 7. The bone anchor receiver of claim 1, wherein each of the upright arms has a guide and advancement structure formed on the inwardly facing surface thereof. 8. The bone anchor receiver of claim 1, wherein when the rod is locked within the rod-receiving channel, a top surface of the rod is disposed below each of the horizontal radiused tool attachment structure upper edge downwardly facing surfaces. 9. The bone anchor receiver of claim 1, wherein the second planar surface portions are coplanar with respect to the receiver base and opposed upright arm lateral side outwardly facing surfaces. 10. The bone anchor receiver of claim 1, wherein the first and second planar surface portions are faceted with respect to the receiver outwardly facing front and back surfaces and the lateral side outwardly facing surfaces. 11. The bone anchor receiver of claim 1, further comprising a shank, the shank having an upper end portion positionable within a lower cavity of the receiver base, the cavity communicating with the channel and a bottom surface of the receiver base through a lower opening. 12. The bone anchor receiver of claim 11, wherein the shank is pivotal with respect to the receiver. 13. The bone anchor receiver of claim 11, wherein the shank is cannulated.
