본 연구에서는 수지상 세포 (DC)의 배양기술을 확립하기 위하여 마우스 골수 및 사람의 혈액으로부터 DC를 분화, 배양시키는 기술을 발전시키고자 하였다. 이를 위해 지금까지 보고된 기술을 조사 분석하고 본 실험실의 조건에 적합한 대량 배양 기술을 확립하는 것을 첫 번째 목표로 하였다. DC는 그 배양조건이 까다로운 편이면서 장기간의 배양이 어려워 다양한 실험을 수행하는데 필요한 충분한 양을 순도 높게 얻는 것이 용이하지 않았으나 최근에는 전구세포로부터 cytokine의 존재 하에서 분화를 유도하여 비교적 다량을 얻는 기술이 확립되어

DC는 그 배양조건이 까다로운 편이면서 장기간의 배양이 어려워 다양한 실험을 수행하는데 필요한 충분한 양을 순도 높게 얻는 것이 용이하지 않았으나 최근에는 전구세포로부터 cytokine의 존재 하에서 분화를 유도하여 비교적 다량을 얻는 기술이 확립되어 가고 있다. 본 연구를 통해서 이것이 가능하다는 것을 확인하였으며, 이들을 이용한 종양 치표용 DC 백신의 개발이 가능함을 입증하였다. 본 연구는 이러한 기초 위에서 마우스의 골수와 사람의 혈액에서 DC를 분리 배양하고 이들의 특성을 분석하기 위하여 수지상 세포 생산 및 면역 표현형, 항원 탐식작용, 혼합 림프구 반응, 항원 선택적 세포독성 T 림프구의 생산 및 자연살해 세포의 활성 유도에 관한 실험을 하였으며, 마우스 종양모델에 응용하여 성공적으로 종양 예방백신으로 활용 가능함을 입증하였다. 이때에 종양항원을 가해주는 다양한 방법을 연구하여 암세포-DC의 동시배양 그리고 necrosis를 유도한 종양세포와 DC의 동시 배양법 등을 연구하여 이러한 방법들이 실제 종양항원의 정체를 알 수 없는 임상조건에서도 활용 가능한 방법임을 입증하였다. 나아가 DC가 보여주는 항종양 면역에 관여하는 면역 효능세포의 역할에 대해서도 연구하여 종양 특이적 CTL의 활성화가 중요한 기준이 될 수 있으며, 또한 자연살해세포로 알려진 NK cell의 활성화도 중요한 항암 면역 지표로 작용할 수 있음을 보여 주었다. 또한 IL-12의 생성이나 chemokine 수용체의 발현도 항암 면역 유도에 있어서 좋은 지표가 될 수 있음을 보여 주었다. 나아가 DC의 세포사멸을 유도하는 기전을 연구하여 장차 DC의 생존을 높이는 방법을 개발하는데 도움이 될 수 있는 정보를 다양하게 얻을 수 있었다. 그러나 본 연구에서 사용된 DC 백신은 이미 확립된 종양에 대한 치료 효과 측면에서는 그 효과가 미미하여 실제로 임상에서 필요한 기술을 확보하는데는 더 많은 연구가 필요하다는 점을 보여 주었다. 실제로 다양한 종양에 적용 가능한 항암 효능을 갖는 DC 백신을 만들기 위해서는 항암 cytokine의 병용, 기타 화학요법이나 외과적 수술이 병행되어야 함을 많은 연구자들이 지적하고 있어, DC를 활용한 완벽한 치료용 백신의 개발은 아직 시간이 필요한 것으로 생각되고 있다. 한편 제2세부과제에서는 인체내에서 강력한 면역 반응을 유도 할 수 있는 수지상 세포를 만들며 이 세포들이 항원 전달세포로서의 기능을 수행 할 수 있는가를 확인하며 기존에 알려진 수지상 세포보다 좀더 강력한 면역 반응을 유도 할 수 있는 새로운 형태의 수지상 세포를 만드는 것이 일차적인 목표였다. 본 연구는 기존의 알려진 방법인 GM-CSF, SCF, TNF-α, IL-4, CD40L를 이용한 수지상 세포 생산 및 면역 표현형, 항원 탐식작용, 혼합 림프구 반응, peptide 항원을 이용한 항원 선택적 세포독성 림프구의 생산에 관한 실험을 하여 최종적인 기능적 결과까지 확인 할 수 있었다. 최근 쥐의 비장에서 Th-1 형태의 단백질인 interferon-gamma를 직접 생산하며 CD8의 면역 표지자를 보이는 수지상 세포를 확인하였고 이를 이용한 면역 반응유도에서 림프절로 이동하지 않고 주사된 부위에서 직접 면역 반응을 유도 할 수 있는 수지상 세포가 발견되었다. 이러한 연구는 매우 중요한 발견으로 생각되어 지고 있으나 아직 인간에서의 존재는 확인되지 않았다. 본 연구에서는 인간의 CD34 양성의 모세포에서 GM-CSF와 interferon-gamma를 이용하여 수지상 세포를 만들었으며 이 세포들은 CD8, CD40, CD54, CD80, CD83, CD86와 HLA를 표현하며 CD1a, CD3, CD4와 CD14를 표현하지?고 이러한 결과는 세계적으로 처음인 결과이다. 이러한 강력한 기능을 갖는 수지상 세포는 앞으로 진행할 면역 요법의 임상연구에 주요한 수단을 제공 할 것이다. 최근까지 분자 생물학 기법을 이용하여 미세 잔류암을 진단하고자하는 시도는 계속되어지고 있다. 미세 잔류암의 혈액내 존재 확인으로 예후에 대한 좀더 좋은 예측을 하려는 시도이며 이를 대상으로한 면역 치료가 임상적으로 좋은 결과를 기대할 수 있을 것이다. 유방조직에서만 분비되는 단백질인 hMAM의 존재가 알려졌고 이를 이용한 RT-PCR기법으로 새로운 단백질을 표적으로 한 시험을 다양한 병기의 유방암환자를 대상으로 실시하였다. 환자에서만 혈액내에서 hMAM의 RT-PCR에서 양성 반응을 보였으며 비교적 병기별로 양성율의 차이를 보였다. 정상인에서 발견되지 않는다는 점과 질병의 진행상태에 따라 양성율이 높아진다는 점이 미세 잔류암의 진단을 위한 목적으로 합리적일 것이라는 결론이다. 본 연구의 목적이 미세 잔류암의 조기 진단 및 이를 대상으로 한 효과적인 면역 치료로 실제 생존율의 향상을 이루고자 하는 것에 부합되는 결과라 하겠다. 위의 결과의 바탕으로 인체에 적용 가능한 강력한 기능을 갖는 수지상 세포를 이용하여 거시적으로 관해된 유방암에서 미세 잔류암의 확인하거나, 치료에 반응하지 않는 경우 수지상 세포를 이식하여 장기적 생존율의 향상을 이루고자하는 것이 앞으로 진행될 임상 실험 계획이다.

In this study we wanted to develop and establish culture technique for dendritic cells (DCs) generated and differentiated from mouse bone marrow and human peripheral blood cells. To this end, we studied the published culture techniques and tried to establish our own mass culture conditions of DCs. Though it had been difficult to generate DCs in vitro with high purity in a large amount enough to do experiments since long-term in vitro culture was not easy, recently it has been reported that they can be induced to differentiate from precursor cells in the presence of cytokines. We confirmed in this project that primary DCs can be cultured in the presence of combination of cytokines. Based on these conditions, efficacy of DC vaccine for tumor immunotherapy could be tested in mouse tumor model. We studied the immunophenotype on DCs, antigen uptake/processing, mixed lymphocyte reaction by them, and induction of specific cytotoxic T lymphocyte and natural kiler cell activity followed by immunization with DC vaccine. They were finally proved to successfully protect mice against tumor challenge. Some strategies to charge tumor antigen(s) on DC, such as tumor-DC co-cultivation or necrotic tumor cell onto DC, were studied in detail. Our results suggested that the approach would to be useful for designing tumor vaccine using DC when the information about tumor antigens is limited. In addition, it has been shown in the study on the role of immune effector cells in the antitumor immune response that activity of cytotoxic T lymphocytes as well as natural killer cells induced by DC vaccination was valuable parameter for determining antitumor efficiency of DCs. Our experiments also showed that IL-12 production or expression of specific chemokine receptors after antigen-loading on DCs could be used to detect DC maturation, resulting in an enhanced stimulation of antitumor immune responses. Furthermore, prolonged survival of DCs by CD40 ligation has been observed in the experiment of glucocorticoid-induced DC apoptosis mechanisms. However therapeutic effect of our DC vaccine in tumor-bearing mice, which may be relevant to a better planning of clinical trials, was little bit disappointing. Therefore further intensive studies deserve to be followed. In fact it has been reported that DC vaccine combined with cytokines with antitumor effect after surgery or chemotherapy would be even more effective compared to DC vaccine alone. Based on these informations it is anticipated that we can design improved DC vaccine in the near future. The primary goals of second part of the project are generation of dendritic cells which have potent immunostimulatory function including antigen presentation and more potent dendritic cells compared with previously known dendritic cells in human. We studied the immunophenotype, antigen processing, mixed lymphocyte reaction and generation of specific cytotoxic lymphocyte with peptide antigen with GM-CSF, SCF, TNF-α, IL-4, CD40L from CD34+ stem cells. Recently, new type of dendritic cells was studied which were isolated in mouse spleen and had function of interferon-gamma production and CD8 molecule on the surface. Still this type of dendritic cells was not discovered in human. We generated this new type of dendritic cells from human CD34+ stem cells with GM-CSF and interferon-gamma. These type of dendritic cells expressed CD8, CD40, CD54, CD80, CD83, CD86 and HLA systems and did not express CD1a, CD3, CD4 and CD14 and produced IL-12 and interferon-gamma. We confirmed functions of antigen presentation with this type of dendritic cells. In human, the generation of CD8+ dendritic cells is first result in the world. This potent dendritic cells can introduce immunotherapeutic modality in the clinical trials. Recently, molecular techniques were applied for detection of micro-residual disease. This means that early detection of systemic disease can be used to decide further therapy after conventional treatment modalities, especially immunotherapy. The mammaglobin is a novel protein that is secreted from the mammarian epithelium of normal breast tissue as well as malignant breast cancers. In order to ascertain the prognostic value of mammaglobin gene in breast cancer patients, we measured the expression of human mammaglobin (hMAM) by RT-PCR method in various stages of breast cancer patients. In contrast to healthy volunteers, hMAM transcripts were detected in the peripheral blood of breast cancer patients. The frequency of hMAM expression in peripheral blood was correlated with the clinical stages of disease. The clinical relevance of hMAM RT-PCR-based tumor cell detection in the peripheral blood of breast cancer patients should be considered further therapies including dendritic cell based immunotherapy. Based on basic research, we can design a clinical protocol on dendritic cell-based immunotherapy for micro-residual and refractory breast cancer patients in order to improve long term survival.
