본 과제의 연구목표는 기존의 공기전도형 보청기가 가지는 난점인 음향궤환에 의한 하울링과 음향 전달과정에서의 왜곡현상을 극복할 수 있도록 중이 이소골에 부착되는 전자 트렌스듀서를 갖는 인공중이를 구현함으로써 중등이상의 전도성 및 감음성 난청 등에도 효과적으로 응용가능한 이식형 보청기를 개발하기 위한 것이다. 이를 위하여 초소형, 고효율 인공중이용 전자 트랜스듀서와 소형의 고성능 음성처리용 체외기 및 체내기를 개발하며, 개발된 시스템을 이식하는데 필요한 생체측정과 생체적합성 확보에 대한 연구 그리고 트랜스듀서를 이소골에 이식하기
본 과제의 연구목표는 기존의 공기전도형 보청기가 가지는 난점인 음향궤환에 의한 하울링과 음향 전달과정에서의 왜곡현상을 극복할 수 있도록 중이 이소골에 부착되는 전자 트렌스듀서를 갖는 인공중이를 구현함으로써 중등이상의 전도성 및 감음성 난청 등에도 효과적으로 응용가능한 이식형 보청기를 개발하기 위한 것이다. 이를 위하여 초소형, 고효율 인공중이용 전자 트랜스듀서와 소형의 고성능 음성처리용 체외기 및 체내기를 개발하며, 개발된 시스템을 이식하는데 필요한 생체측정과 생체적합성 확보에 대한 연구 그리고 트랜스듀서를 이소골에 이식하기 위한 적절하면서도 안전한 임상적 시술절차에 관한 연구를 수행하였으며 그 내용을 구체적으로 요약하면 다음과 같다.
1. 이시중이용 전자 트랜스듀서의 개발 먼저, 중이 이소골에 설치할 수 있는 작은 크기를 가지며 공급전류에 대한 진동효율이 높고 외부의 강한 환경자장에 의한 잡음 상쇄 및 생체 보호가 가능한 차동 플로팅매스형 (DFMT: differential floating mass type) 전자 트랜스듀서를 개발하였다. 개발된 트랜스듀서는 코일의 내부에같은 극끼리 마주보도록 부착한 2개의 영구자석을 비접촉으로 위치시킨 후, 탄성체를 이용하여 코일과 자석을 지지하도록 하는 구조를 가졌으며, 코일에 인가되는 전류에 대하여 트랜스듀서 전체가 진동하는 일체형의 형식을 가진다. 개발된 트랜스듀서는 코일내부에 영구자석이 위치하므로 누설 자속을 최소화 할 수 있어서 진동효율이 매우 높고, 같은 극끼리 마주 보도록 부착한 2개의 영구자석을 사용하므로 외부의 환경자기장이존재하는 곳에서도 자기장의 영향을 받지 않고 안정된 성능을 발휘하도록 하였다. 트랜스듀서에 사용되는 탄성체는 기존에 사용하던 실리콘 대신탄성이 우수한 Polyimide를 이용하였으며 반도체 마이크로머시닝 기법을 이용하여 제작함으로써 향후 동일한 성능의 트랜스듀서를 대량 생산가능하도록 하였다. 한편, 공급되는 가청 주파수의 음성전류에 대하여 트랜스듀서에서 발생되는 진동력을 계산함으로써 개발된 차동 플로팅매스형 트랜스듀서가 이식중이에 사용되기 위해 필요한 설계상수를 추출하였고 전기-기계적 모델링을 통한 트랜스듀서의 진동특성을 해석하였다. 그리고 청각 모델에트랜스듀서의 모델을 삽입하여 해석함으로써 이식중이에 의해 내이로 전달되는 진동력의 크기를 계산하였다. 이러한 과정을 거쳐 설계, 제작된 인공중이용 차동 플로팅매스형 전자 트랜스듀서는 진동실험 및 사체를 이용한 이소골 진동실험을 통해 가청주파수대역에서 매우 우수한 진동특성을 가짐을 확인하였고 이소골 내에 이식 가능한 크기인 지름 1.8mm, 길이 2mm의 크기로 제작되었을 때 중등 고도이상의 난청자에게 필요한 100dB SPL에 해당하는 진동력을 충분히 생산해 낼 수 있음을 확인하였다.
2. 음성처리용 체외기 및 체내기의 개발 이식형 인공중이 시스템은 중등고도이상의 난청역치를 가지는 전도성, 감음신경성 및 혼합형 난청 등 모든 난청자들에게 사용 가능한 특징을 갖는다. 그러므로 실제적인 난청자의 청각 특성을 고려하여 모든 형태의 난청자들에게 사용할 수 있도록 신호처리회로를 설계하였다. 먼저 이소골에 전달되는 음압에 해당되는 기계적 진동을 발생시키기 위하여 진동 트랜스듀서의 효율, 체외기와 체내기간의 coupling 계수 및귀의 생리적 특성이 감안된 시스템의 필요이득을 해석하였다. 이를 통하여 난청자의 난청정도에 맞는 주파수 대역별 필요이득을 계산하였고 신호처리회로의 설계에 이용하였다. 설계된 신호처리회로는 가청주파수 대역을 크게 2개의 대역으로 나눈 후 각 대역에서의 이득을 조정 가능하도록 설계하였다. 그리고 현재 기존의 보청기의 신호처리회로에 사용되는 programmable chip을 활용하여 인공중이에 적용함으로써 컴퓨터를 이용하여 간단히 난청자의 청각역치에 주파수별 이득을 맞추기만 하면 이식형 청각보조기에 필수적인 요소인 AGC 및 압축작용이 가능하도록 설계하였다. 인공중이는 몸 속에 이식되는 부위인 체내기에 전원이 없기 때문에 효율적?기로의 전력전달 효율을 개선시켰다. RF 변조단은 Colpitts발진 회로를 이용하였고 carrier frequency는 5MHz 정도이다. 한편 안정된 FM발진 및 회로의 소형화를 위하여 4046B PLL chip을 이용하여 발진회로를 설계하여 이용하였다. 이와 같은 과정을 통하여 소비전류 5mA미만, 왜율 5% 미만, 트랜스듀서 최대구동전류가 3mA 정도인 우수한 성능의 인공중이용 체외기 및 체내기 회로를 설계, 제작하였다.
3. 생체계측 및 임상적 연구 먼저 몸 속에 이식되는 부분에 대한 생체적합성의 확보를 위하여 체내개의 외부에 생체적합성이 검증된 물질을 이용하여 코팅을 하는 방법을 사용하였다. 코팅물질로는 반투명의 Poly ethylene-vinyl acetate(60% ethylene)과 폴리우레탄을 이용하였다. 제작된 체내기를 코팅물질로 코팅한 후 기니픽과 삽살개를 이용하여 중, 장기의 생체실험을 실시하였고 조직검사를 통하여 생체에 주는 영향에 대한 평가를 실시하였다. 그리고 제작된 트랜스듀서와 체외기 및 체내기를 사람의 사체샘플을 이용하여 진동실험을 실시함으로써 개발된 인공중이 시스템의 성능을 평가하였다. 한편 기니픽과 삽살개를 이용하여 직접 이식실험을 실시함으로써 사람에게 이식시의 문제점에 대하여 고찰하였고 수술절차에 대한 연구 및 데이터를 획득하였다. 이식 실험 후에는 이식에 사용된 기니픽과 삽살개에 대한 뇌간 청성 반응 검사(ABR: auditory brainstem response)를 이용하여 이식후의 청각의 상태를 검증하였다. 이와 같은 과정을 통하여 proto-type의 이식형 인공중이 시스템의 시제품을 성공적으로 개발하였다. 삽살개를 이용한 동물 이식실험을 통하여 개발된 이식형 인공중이 시스템의 성능의 우수성을 확인하였다본 연구진에 의해 개발된 이식형 인공중이 시스템은 국내에서는 최초이기 때문에 이의 사업화를 위하여 인공중이를 사업품목으로 하는 벤처기업 인증을 획득하였으며 식품의약품 안정청에 신개발 의료기기로 등록신청을 하였다. 또한, 사람에게 직접 적용하는 임상실험을 실시하기 위한식품의약품 안정청의 허가를 얻기 위한 절차를 진행중이다.

The goal of the current R&D is an implantable middle ear device that includes an electromagnetic transducer attached to the auditory ossicles, thereby overcoming the weaknesses of conventional hearing ability due to conductive distortion and the howling effect caused by audio feedback for those with mild to severe conductive or sensorineural hearing impairments. Accordingly, a miniaturized high-efficiency electromagnetic transducer was developed along with a hearing aid consisting of internal and external devices for high-performance audio signal processing. In addition, appropriate and safe procedures for the surgical implantation of the proposed transducer in the middle ear were investigated, along with several biomedical measurement and bio-compatibility tests of the internal device in the human body. A differential floating mass type(DFMT) electromagnetic transducer has been developed that is enough small to be attached to the ossicles and its vibration efficiency is superior to the conventional floating mass type(FMT). The major advantages of the developed transducer include its ability to protect the human ear through reducing any noise generated by a strong environmental magnetic field. The proposed transducer has two permanent magnets facing each other with the same polarity within a cylindrical loop coil. The coil and magnet are both supported by an elastic body, as such, the whole transducer vibrates when a signal current is flowing through the coil. The proposed transducer also has a very high vibration efficiency because the permanent magnet is located inside the coil, thereby minimizing any loss of magnetic flux. Thee lastic body used in the transducer is made of polymide, rather than silicon, and is manufactured using micro-machining technology, therefore, it is easy to maintain a consistent quality in the case of mass production. By calculating the vibration force in terms of the current within the audible frequency range, design criteria have been extracted that enable the proposed DFMT transducer to be altered to suit the individual patient, while the vibration characteristics are analyzed using an electrical and mechanical model of the transducer. Furthermore, the vibration force transmitted from the transducer to the inner ear can be calculated using the hearing organ model incorporated in the proposed transducer. From vibration experiments using the developed transducer in an unloaded condition and attached to the ossicles of a human ear cadaver, the proposed DFMT transducer was confirmed to exhibit excellent vibration characteristics in the audio frequency region. When the diameter and length of the proposed transducer was 1.8mm and 2mm, respectively, enabling it to beimplanted in the auditory ossicles, a vibration force of 100dB SPL could be produced, which is the required input level for a severely hearing impaired person.
