(2) 혈청학적시험을 위해 상업용 진단 Kit와 본 실험실의 간접면역형광항체법 (IFA)으로 수집된 시험대상 집단의 혈청에서 Dengue 바이러스에 대한 항체 보유율을 조사. (3) 혈청학적 실험결과를 종합하여 분석하여 실험실 진단지침 및 감시체계를 구축. ○ 연구결과 - 본 조사는 상업용 kit와 본 실험실에서 정립된 간접형광항체법 (IFA)을 이용하여 조사되었다. 뎅기열로 의심되어 국내 병원에 입원하였던 25명의 혈청에서 9건 (36%)이 MAC-ELISA kit에서 IgM 양성반응을 보였고, IFA에서는 13건 (52%)이 1:32이상의 IgG 양성반응을 보였다. 일반 건강 검진자 가운데 뎅기열 감염 확률이 높은 482건의 원양어선 선원들의 혈청에서는 46건 (9.5%)이 IFA법에서 항체 양성으로 확인되었으며, 여행안내자 35명의 조사에서는 항체 양성자가 없었다. IFA법의 정립을 위해 수집한 뎅기열 유행지역의 베트남 현지인의 혈청에서는 총 33건 중에서 15건(45.5%)이 항체 양성으로 확인되어 현지인들의 높은 항체 양성율을 보였다. - 조사결과로 미루어 조사되지 않은 더 많은 감염자가 있을 것으로 추측할 수 있으며 해외교류가 증가함에 따라 뎅기 감염자는 증가할 것으로 생각되며 고가의 진단kit를 대체 할 수 있는 진단법 정립이 요구되었으며, 이 대상에서 시험되지 못한 유행지역의 외교관, 선교사, 교민 등의 조사가 요구되며 재감염에 대한 조치가 필요함을 알 수 있었다.

To determine the presence and extent of antibody against dengue virus in Korea, we initiated a serosurvey of the suspected dengue patients, seamen, and Vietnamese. This study provides the establishment of the diagnosis and surveillance systems of dengue in Korea for the control of disease. ○ Methods (1) To determine the presence and extent of antibody against dengue virus in Korea, serum specimens of travelers and long-term overseas stayers were collected with the help of related institutes. (2) Serum specimens have been tested for the dengue serodiagnosis, using a commercial kit (IgM capture enzyme-linked immunosorbent assay, MAC-ELISA) and indirect immunofluorescent antibody (IFA) test. For IFA test, the dengue virus serotypes (DEN-2, DEN-3, DEN-4) were infected in vero cells. Infected cell cultures were harvested and assayed for dengue virus by IFA test. Positive sera were titrated. (3) By the analysis of this study, the laboratory-based dengue diagnosis and surveillance system were established in Korea. ○ Results During 2001, our laboratory received 25 samples from returning travelers who had suspected dengue. Sera of nine (36%) patients, returning from south-east Asia, were positive by IgM-capture-ELISA, indicating current or recent infections. A total of 482 sera were studied from seamen during August through December 2001, 46 (9.5%) were seropositive for IFA using a reciprocal titre ${\geq}32$ as positive. Of 33 Vietnamese sera for dengue serodiagnosis, 15 (48,.5%) were antibody positive by the IFA test. Since travelers to tropical areas in south-east Asia have high antibody titer against dengue virus, these data highlight the need for the laboratory-based dengue surveillance system in Korea.
