본 연구의 최종 개발 목표는 동양인 체형 및 생활양식에 적합한 인공무릎관절을 상품화하는 것이다. 최종 목표를 달성하기 위해 본 과제는 크게 4개의 세부과제로 구성되었다. 제1세부과제는 인공무릎관절의 설계 및 평가에 필요한 요소기술의 확립, 시술기기의 제작, 제2세부과제는 금속재료의 개발 및 제조공정 확립을 통한 시제품 완성, 제3세부과제는 초고분자량 폴리에틸렌 재료의 마모 및 접착특성을 향상시키는 기술개발 그리고 제:t세부과제는 소요되는 모든 재료의 생체적합성 검증을 담당하였다. 제1세부과제 본 세부과제에서는 유한요소해석을 기반으로 생체 역학적인 인공무릎관절을 설계하고 이를 위한 수술용 instrument를 제작하였으며 simulator를 이용하여 인공무릎관절의 생체역학적 성능평가를 수행하였다. 한국형 인공무릎관절의 최적 설계를 위해 정상인 무릎관절에 대한 3차원 수치해석 모델을 구축하였고 이에 대한 형상 설계변수를 설정하여 유한요소해석을 수행하였다. 이를 통하여 patellar component의 정적해석을 통해 형상 type을 결정하였고 충격하중 하에서의 응답특성 연구를 수행하여 tray part의 형상 설계변수를 결정하였으며 접촉면에 대한 응력해석과 운동성을 분석하여 PE part 및 femoral component의 곡률변수를 결정하였다. 최종적으로 최적 형상의 시제품을 제작하였다. 본 제품에 대한 생체역학적 성능평가를 위해 본 연구에서 dynamic knee simulator를 제작하였다. 본 기기는 5축의 자유도를 갖고 있으며 사용자가 원하는 압축 하중곡선과 굴곡각을 모사할 수 있다. 이를 이용하여 개발된 인공무릎관절의 마모 및 운동성을 예측할 수 있고 추가적으로 I-scan 압력센서를 사용하여 접촉면에 대한 연속적인 압력분포를 분석할 수 있다. 이는 차후에 다양한 제품에 대한 기계적 특성의 정량화가 가능하고 좀 더 개선된 제품 개발에 활용할 수 있을 것으로 사료된다. 위탁과제 1 본 연구의 목적은 컴퓨터 3차원 영상과 시뮬레이터를 이용하여 인공관절의 기계적 성능개선을 위한 설계를 지원한다. 이를 위해 무릎 관절의 연속적인 2차원 CT 영상 데이터로부터 영상처리와 컴퓨터 그래픽스의 요소 기술을 활용하여 관절을 3차원 형태로 실측기준으로 시각화하며, 가상공간에서 인공관절 치환시 운동 해석을 위한 시뮬레이션 환경을 PC 기반에서 구축한다. 아울러 모의시술 프로그램을 통하여 조이스틱과 마우스를 이용하여 인공관절의 절단과 치환을 가상으로 실행할 수 있도록 한다. 시뮬레이터 설계 지원 및 계측 데이터 수집 그리고 관절 운동과 부하 변하에 따른 관절의 접촉면과 부를 실시간으로 영상처리를 할 수 있음으로써 마모 예측을 시각화한다. 결과적으로 보행 pattern에 따른 운동에 대한 관절 접촉면 해석을 통하여 마모 및 운동 범위를 예측하여 인공관절 model 변경에 대한 근거를 제시하며 아울러 실측기준으로 재구성된 관절의 가상 치환을 통하여 치환 위치의 정합성 판단에 대한 근거를 제시 할 수 있다. 제2세부과제 본 연구의 목적은 인공무릎관절에서 femoral component와 tray 제작에 사용되는 생체 이식 용 Co-Cr 합금 개발 및 가공공정을 개발함에 있다. 생체 이식용 합금 개발을 위해 주조 제품의 열처리 조건을 확립하고 potentiodynamic, potentiostatic test에 의해 내식성을 평가하였다. 무시멘트 접합기술을 획득은 Ti-In 합금의 화학적-열적 처리에 표면개질 처리를 통해 이룰 수 있고 in vitro 시험을 통해 Ti-In 합금의 생체활성 연구를 수행하였다. 그리고 본 연구에서 개발한 Co-Cr 합금을 사용하여 정밀주조공정을 통한 한국형 인공무릎관절을 제작하였다. Co-Cr-Mo 합금의 용체화 처리는 임플란트의 내식성과 이와 밀접한 관계가 있는 생체적합성에 악영향을 끼치지 않으면서도 기계적 특성을 향상시키는 방법으로서 장기간 지속적으로 높은 하중을 받는 임플란트에 보다 높은 안정성을 부여할 수 있는 필수적인 공정이라 사료된다. 본 연구에서? 사용하고 있는 Ti-6Al-4V 합금과 비교했을 때 더욱 미세한 다공성 조직을 나타내어 치밀하고 얇은 다공질 층을 형성하기 때문에 물리적으로 안정하여 보다 높은 골결합강도를 나타낸다. 또한 유독성 원소를 포함하지 않을 뿐 아니라 우수한 생체활성을 가지고 있어 Ti-6Al-4V 합금을 대체할 수 있는 유력한 생체용 합금으로서 기대된다. 제3세부과제 제3세부과제에서는 인공관절의 수명향상을 위하여 폴리에틸렌과 뼈 시멘트와의 접착력을 향상시키는 연구와 마모 성능이 개선된 폴리에틸렌 재료의 개발을 위한 연구를 독립적으로 수행한 후, 결과들을 종합하여 궁극적으로 슬개골과 경골부에서 금속지지체를 쓰지 않고 전부 고분자 재료로 된 인공관절을 개발하는 것을 목적으로 하였다. UV/오존법. UV법, 감마선조사 등으로 폴리에틸렌 표면에 PMMA를 그래프팅시켜 뼈 시멘트와의 접착력 시험을 수행한 결과 계면이 아닌 뼈시멘트 자체가 파괴될 정도로 접착력을 크게 향상시켰다. 폴리에틸렌을 peroxide를 이용한 화학적 가교를 통해 가교가 내마모성을 향상시킬 수 있음을 확인하였으며, 감마선 조사와 열처리들 통해 가교도와 결정화도를 증가시키는 방법으로 내마모성 향상 가능성을 제시하였으며 내크립성을 크게 향상시킬 수 있었다 결론적으로 금속 지지체없이 고분자재료만으로된 인공관절의 설계가 가능함을 제시하였다. 제4세부과제 본 세부 과제에서는 인공무릎관절의 요소 재료인 Ti-4V-6Al 합금과 Co-Cr 합금 및 초고분자량 폴리에틸렌에 대하여 적정한 가공 후 상태에서 생체 적합성 검사를 시행하였다. 시행한 검사로는 가토를 이용한 생체 피부, 근육, 골격내의 이식 시험을 직접 수행하였고, 세포 독성 시험, 감작성 시험, 급성 독성 시험, 자극성 시험, 유전 독성 시험 등은 용출물을 이용한 시험관내 검사로 시행하였다. 이러한 검사를 통하여 본 재료의 생체 적합성을 확인하였다. 따라서, 상기 시료를 이용한 인공무릎관절 제작은 생체 적합성 면에서 적절하리라 판단된다.

This final objective of this project was to develope an total knee implant for Oriental populations. This project consists of 4 sub-projects. Sub-project 1 includes development of fundamental technologies for implant design and evaluation as well as manufacturing of instruments. Sub-project 2 focuses on the development of a new metal alloy and its manufacturing process, thus producing a real prototype. Sub-project 3 deals with UHMWPE, in which the enhancement of wear and adhesion properties of the material was achieved. In sub-project 4, all the animal and clinical evaluation were peformed on the all the finally processed materials. Sub-project 1 The optimal design of the knee implant was performed using FEM analysis. All the geometrical parameters of implants were examined under both static and impact loading situations. The contact stress analysis was also performed to minimize wear of UHMWPE. Based on the analysis as well as clinical evaluations on conventional implants, the curvature of PE and femoral components were determined. The instruments were designed and modified several times to satisfy clinicians demands for convenience. In order to evaluate the biomechanical performances of the implant, we developed a dynamic knee simulator which has 5-axis of freedom, thus simulates rolling and sliding as well as rotation of knee. Using I-scan technology, we were able to analyze the continuous contact pressure on PE surfaces on real time basis. In co-operation with Hong-Ik University, we developed a comprehensive 3D graphical computer user interfaces to visualize the knee surface model and a force-controlled dynamic knee simulator to improve the mechanical performance of artificial knee joint design. A PC-based simulated replacement program for artificial knee joint was developed. From consecutive CT images we construct the knee surface model based on actual data in 3-D virtual space by applying various image processing techniques. Through this program we test and visualize the simulated joint motion according to the angle data around the mechanical axis and also can on a virtual replacement surgery using device such as mouse or joystick. To quantify the mechanical performance of artificial knee joint design we construct the force controlled dynamic knee simulator. The pressure is measured from I-scan system and simulator visualize the pressure distribution on the joint contact surfaces under loading condition during walking cycle. We provide a computer user interface for performing 3-D spatial manipulation and reasoning tasks which might be a basis for the general purpose 3D surgical simulation environment. With interactive visualization of the load distribution on the joint contact surfaces during simulated gait cycle we can evaluate the long-term implant wear pattern and the geometric interaction of femoral and tibial design. Sub-project 2 Subproject 2 was focused on the developement of a new Co-Cr alloy and its manufacturing process for knee implant. The heat treatment conditions for casted alloy materials and their corrosion resistance properties were evaluated using potentiodynamic and potentiostatic tests. Furthermore, the surface treatment technology of Ti-In alloy was developed and its biocompatibility was evaluated with in vitro tests. The implant was produced through precision casting technology for the first time in Korea. The defects due to the casting process was minimized. The alloy also showed a better mechanical properties. The new Ti-In alloy was more stable and better biocompatibility as well as excellent bone adhesion characteristics. Sub-project 3 This sub-project was aimed at developing an polymer material for the longer life span and for the all-polymer type artificial joint without a metal stem. In this end we carried out both of studies on the improvement of adhesion between ultra-high molecular weight polyethylene (UHMWPE) and bone cement and on the enhancement of the wear resistance of UHMWPE. A considerable improvement of the adhesion resulted from grafing PMMA onto the UHMWPE surface by ozon/UV method, UV exposure, or gamma-irradiation. Polyethylene chemically crosslinked with peroxide showed that crosslinking is an promising method to improve the wear resistance of UHMWPE. A strong probability of enhancing wear resistance of UHMWPE was proposed by the increase in crosslink and crystallinity via gamma-irrradiation and heat treatment which enhanced creep resistance of UHMWPE. In conclusion, there is a good possibility of the design of the artificial joint composed of only polymer components without metal stem. Sub-project 4 All the materials used for knee implants, metal alloy as well as polymer, and the effect of the manufacturing process on body were evaluated through in vitro and in vivo tests. For animal experiments, rabbit model was developed and all the necessary biological tests, required for KFBA approval, were performed based on an International Standards (ASTM). The biocompatibility of all the materials was proved. We are sure that the tests results will guarantee the KFDA approval in a near future.
