세포, 실험동물 그리고 당뇨환자의 신조직에서 TonEBP(tonicity enhancer binding protein), AR(aldose reductase), HSP 70(heat shock protein 70) 및 여러 소디움 운반체와 소디움 통로의 변화를 조사하여 당뇨병성 신증의 병리기전을 규명하고자 하였다.
■ RNAi를 이용한 TonEBP knockdown permanant cell line 제작에 실패하여 고혈당 자극에서 TonEBP의 역할 규명에는 성과가 없었다.
■ MDCK세포를 밀집시켜 배양한 후 장기간
세포, 실험동물 그리고 당뇨환자의 신조직에서 TonEBP(tonicity enhancer binding protein), AR(aldose reductase), HSP 70(heat shock protein 70) 및 여러 소디움 운반체와 소디움 통로의 변화를 조사하여 당뇨병성 신증의 병리기전을 규명하고자 하였다.
■ RNAi를 이용한 TonEBP knockdown permanant cell line 제작에 실패하여 고혈당 자극에서 TonEBP의 역할 규명에는 성과가 없었다.
■ MDCK세포를 밀집시켜 배양한 후 장기간 높은 tonicity, 요소, betaine에 노출시킨 결과 신장과 유사한 고장성 조건하에서는 TonEBP 단백양의 변화보다는 세포내 분포의 변화가 TonEBP activity의 주된 조절기전이었고 요소와 betaine이 TonEBP activity에 중요한 영향을 미친다는 사실을 알 수 있었다. 이는 향후 당뇨 병성 신증에서 세포수준의 병리기전을 규명하기 위한 적절한 세포배양모델로 활용될 수 있겠다.
■ 자연발생적인 제 2형 당뇨 동물모델인 OLETF rat는 요단백양과 사구체여과율이 증가하여 말기신부전으로 진행하기 전단계의 당뇨병성 신증에 합당한 소견을 보였다. 이 단계에서 NHE3, NKCC2, NCC는 단백발현에 변화가 없었고 ENaC β subunit는 증가, γ subunit는 감소 소견을 보였으며 AR과 HSP70는 변화가 관찰되지 않았다. 당뇨병성 신증의 진행에는 대사성 기전보다는 혈역학적 기전이 더 중요한 역할을 하는 것으로 사료된다.
■ 대조군 IgA 신병증 환자 4명과 질환군 당뇨병성 신증 환자 3명의 신장조직에서 NKCC2, NCC, ENaC subunit의 변화를 비교하였다. NKCC2, NCC의 발현은 감소하였고 ENaC β와 γ subunit는 모두 발현이 증가하였다. 사람의 신장조직에서 소디움 수송체와 소디움 통로의 면역조직염색이 가능해져 향후 후속 연구들에 도움이 될 수 있겠다. 동물실험과 다른 실험결과는 차후 더 많은 당뇨병성 신증 환자의 신장조직을 확보하여 검증이 필요하겠다.
■ MDCK 세포주를 이용한 신장세포를 모방한 세포배양모델을 개발함으로서 향후 당뇨병성 신증의 연구에 유용한 연구수단을 제공하였다.
■ 말기신부전 전 단계의 당뇨병성 신증 동물모델에서 집합관의 소디움 통로 단백양 의 변화가 당뇨병성 신증의 고혈압 발생과 진행에 관여한다는 단서를 제공하였다.
■ 최초로 당뇨병성 신증 환자의 신장조직에서 여러 소디움 수송체와 소디움 통로의 면역조직염색을 성공적으로 시행함하였다.

To elucidate the metabolic and hemodynamic pathogenesis of diabetic nephropathy in the cellular and molecular levels, we examined expressional changes of the molecules including TonEBP(tonicity enhancer binding protein), AR(aldose reductase), HSP 70(heat shock protein 70), sodium transporters and channels in cell culture, animal model and kidney tissues of diabetic patients.
■ We made plasmid producing siRNA against TonEBP. However, we failed to observe TonEBP silencing effect in the cells by stable transfection.
■ Confluent monolayers of MDCK cells were grown under hypertonicity, high urea, and betaine for more than 5 days. As a results, betaine and urea cause cytoplasmic shift of TonEBP and downregulates TonEBP activity. Nucleocytoplasmic redistribution is a major regulatory mechanism of TonEBP activity in the kidney. This is a cell culture model mimicking renal medulla.
■ Spontaneously diabetogenic OLETF rat showed findings of overt diabetic nephropathy. At 40 weeks of age, they showed increased proteinuria and GFR. No significant differences in the expression of NHE3, NKCC2 and NCC were observed. Expression in the β subunit of ENaC was increased, that of γ subunit was decreased. Expression of AR and HSP70 was not changed. From these results, we suggest that the hemodynamic rather than metabolic pathogenesis will play more important role in the progression of diabetic nephropathy.
■ We compared the expression of NKCC2, NCC, ENaC between the kidneys of 4 IgA nephropathy patients and those of 3 diabetic nephropathy patients. NKCC2 and NCC showed decreased immunostaining, β and γ subunit of ENaC showed decreased immunostaining in the diabetic kidneys. Successful immunohistochemical staining of sodium transporters and channels in the human renal tissues could provide a useful tool to future study. Different results in the expressional pattern of sodium transporters and channels between human and animal need further research.
■ Development of cell culture model mimicking renal medulla using MDCK cell line will provide a useful tool in the study of pathogenesis of diabetic nephropathy in the cellular level.
■ Expressional changes of ENaC in the animal model of overt diabetic nephropathy may be related to hypertension and an altered sodium balance, which will provide a clue into the progression of diabetic nephropathy.
