본 연구의 목표는 흰쥐 관절염 모델에서 수반되는 통증의 발생기전을 규명하고자 전기생리학적 연구와 함께 새로운 관절통 측정 방법 및 장치를 개발하여 관절염 연구의 기초를 공고히 하고 또한 관절염시 신경계의 변화를 관찰하여 관절염이 어떠한 염증매개물질에 의하여 유도되는지를 확인함으로써 통증을 조절할 수 있는 방법을 개발하는데 도움을 줄 수 있는 실험적 증거들을 확보하는 것을 목표로 하였다. 관절염 및 관절통의 조절기전을 규명하기 위해 다음과 같은 연구를 하였다. 1)동물 관절통 모델에서 새로운 관절통 측정 방법 및 장치 개발, 2) 관
본 연구의 목표는 흰쥐 관절염 모델에서 수반되는 통증의 발생기전을 규명하고자 전기생리학적 연구와 함께 새로운 관절통 측정 방법 및 장치를 개발하여 관절염 연구의 기초를 공고히 하고 또한 관절염시 신경계의 변화를 관찰하여 관절염이 어떠한 염증매개물질에 의하여 유도되는지를 확인함으로써 통증을 조절할 수 있는 방법을 개발하는데 도움을 줄 수 있는 실험적 증거들을 확보하는 것을 목표로 하였다. 관절염 및 관절통의 조절기전을 규명하기 위해 다음과 같은 연구를 하였다. 1)동물 관절통 모델에서 새로운 관절통 측정 방법 및 장치 개발, 2) 관절의 NK-1수용체가 통증의 발생 및 유지에 관여하는지를 알아보기 위하여 NK-1 수용체 길항제인 L-703,606을 이용한 행동학적 검사 및 조직학적 실험, 3) 관절통에 관여하는 구심성 신경의 역할을 알아보기 위한 전기생리학적 연구, 4) 척수에서 통증과 관련된 neuropeptide의 발현정도와 signaling pathway를 알아보기 위하여 c-fos, SP, CGRP의 발현에 대한 면역조직학적 연구, 5) prostaglandin을 합성하는데 필요한 COX의 역할을 규명하기 위해 in vivo 및 in vitro model에서 cyclooxygenase 역할 연구, 6) 관절의 NMDA 수용체 관여 여부 및 통증과 c-fos의 상관관계를 규명하기 위한 행동학적 연구 및 면역조직학적 연구, 7) 여러 가지 진통제 및 항염증제를 사용한 행동학적 연구 및 조직학적 연구를 시행하였다. 본 연구를 통해 관절염의 동물 모델에서 체중 부하의 감소는 통증을 나타내는 지표로 확인되었으며, 객관적인 통증의 수치화가 가능하며 이후의 관절통 연구에 활용할 수 있는 측정방법과 장치를 개발하였다. 이 방법과 장치는 미국 특허를 출원해 놓은 상태이다. 이 장치를 이용한 행동학적 검사에서 NK-1 수용체와 NMDA 수용체가 관절통증의 발생에 특히 관여하는 것으로 나타났다. 그리고 전기생리학적 연구에서 슬관절 수용체를 지배하는 제 2군 구심성 신경섬유의 민감화가 관절통의 발생에 중요한 역할을 함을 규명하였다. 염증성 질환에 투여한 indomethacin, piroxicam, MK801, capsaicin 등은 형태학적 통증 차단의 지표인 척수 후각의 c-fos 신경원 수와 척수와 DRG의 SP, CGRP면역 염색 반응도를 저하시켰으며, 관절염과 연관된 통증 유발시 기전이 다른 통증 억제제가 c-fos 단백질의 발현이나 신경 펩타이드의 발현에 미치는 효과가 서로 다름을 알 수 있었다. 세포수준에서는 만성 염증으로 인한 COX 활성에는 COX-2가 중요하며 이에 대한 보상은 COX-1이 주도적으로 할 가능성을 시사하고 있다. 본 연구는 관절염 및 관절통을 부작용 없이 치료할 수 있는 약물을 개발하는데 필요한 기초적인 자료를 제공하였을 뿐만 아니라 다른 염증성 질환의 발생기전에 대한 연구와 질환의 치료에도 직접적인 도움을 줄 것으로 기대된다.

The aims of this project were 1) to reveal the mechanism of arthritic pain accompanying arthritis and 2) to provide new method and apparatus that could help the study of arthritis and arthritic pain and 3) to study which inflammatory mediators were involved in the induction of arthritis by examining the changes in the nervous tissue from periphery to spinal cord following arthritis and 4) to obtain the experimental evidences for the development of intervention which could control of arthritic pain. To study the mechanism of arthritic pain accompanying arthritis, the serial experiments were conducted as follows: 1) the experiment that provide a more practical, objective and quantitative method and apparatus to assess arthritic pain and to examine whether NMDA receptors in the knee joint played a role in the generation of the arthritic pain and the accompanying Fos expression in the spinal dorsal horn, 2) the behavioral and immunohistochemical study for the role of NK-1 receptors in the induction and maintenance of arthritic pain, 3) electrophysiological study to examine the role of afferent fiber innervating the knee joint, 4) the study of effects of various anti-inflammatory drugs, such as cyclooxygenases(COX) inhibitors, indomethacin, piroxicam, and capsaicin, in the experimental arthritis model, the changes of number of c-fos immunoreactive neurons, substance P-and CGRP-immunoreactive neurons in dorsal horn and DRG. 5) the study of the roles of COX, which are the rate-limiting enzyme for prostanoids in nervous system, using in vivo and in vitro model, 6) behavioral and immunohistochemical study, using various analgesic drugs and anti-inflammatory drug. The device developed in this project is a reliable and practical measure for assessing arthritic pain quantitatively, which can be applied in the following arthritic study. This device and method had been applied for american patent. In the behavioral and immuohistochemical study using this device, the activation of NMDA receptors in the knee joint contributing to the generation of arthritic pain was involved in Fos protein expression in spinal neurons. This study revealed that sensitization of group II fiber innervating the knee joing contributes the induction of arthritic pain. Indomethacin and piroxicam which is an inhibitors of COX, significantly reduced the expression of c-fos proteins and desensitized nociceptive primary afferents at the early time, and capsaicin, a pungent algesic substance, decreased the level of c-fos protein, SP and CGRP over a wider time in dorsal horn and DRG. Chronic inflammation induced COX-2 expression and COX-2 played an important role in maintenance and development of pain and edema. The predominance of COX-2 in chronic inflammation was interpreted and analyzed in this study by studying the roles and regulation of COX-1 and COX-2 in neurons. These results contribute to clarifying the pathological mechanism, and increase the possibility of prevention and therapeutic approach without unwanted complication to control arthritis and arthritic pain.
