유전과 생활환경의 상호작용이 비만관련 대사증후군에 미치는 영향을 규명하며, 대사성 질환을 예방하기 위한 genome-based lifestyle interaction 전략을 구축하고 예방효능을 입증하여 국제공동연구를 통하여 국내 인간게놈과 임상영양학 분야를 국제적 수준으로 발전시키고자 함.
1차년도에 대사증후군 관련 주요 후보유전자다형성을 탐색하여 이중 영향을 주는 유전 자 SNP를 선정하고 생활환경 및 생화학검사를 통해 유전배경과 생활환경의 상호작용이 대사증후군에 미치는 영향을 규명하였음. 2차년도에 공복혈당장애/당뇨병환자(
유전과 생활환경의 상호작용이 비만관련 대사증후군에 미치는 영향을 규명하며, 대사성 질환을 예방하기 위한 genome-based lifestyle interaction 전략을 구축하고 예방효능을 입증하여 국제공동연구를 통하여 국내 인간게놈과 임상영양학 분야를 국제적 수준으로 발전시키고자 함.
1차년도에 대사증후군 관련 주요 후보유전자다형성을 탐색하여 이중 영향을 주는 유전 자 SNP를 선정하고 생활환경 및 생화학검사를 통해 유전배경과 생활환경의 상호작용이 대사증후군에 미치는 영향을 규명하였음. 2차년도에 공복혈당장애/당뇨병환자(PPARr, APM1), 비만인(AMP1, PLIN), 고콜레스테롤혈증자(APOE) 별로 생활요법을 실시하여 유전자형에 따른 효과를 관찰하였음. 3차년도에 상기 결과를 바탕으로 일반인에게 무료 건 강검진을 실시하여 위험정도를 알려주고, 대사성질환 예방 및 개선을 위한 유전위험인자에 따른 생활요법을 제시하였음. 상기 연구결과는 세계저명학술지 및 학술대회에 발표하여 관련 학계 및 업계에 자료를 제공하였음.
1) APOA5 gene 중 promoter 1131 부분에서 염기 T가 C로 변이될 경우 식사섭취 패턴이 비슷하더라도 공복 및 식후 중성지방 및 유미지립, 초저밀도지단백질(VLDL)의 대사가 지연되고, 염증성지표를 비롯한 스트레스지표가 높게 나타났고, 특히 고지방식사시 저지방 식사시와 비교하여 chylomicron TG의 제거가 훨씬 더 지연되는 것으로 나타나, C allele을 가진 경우 한국인에서 심혈관질환 위험도를 높임을 밝힘(Am J ClinNutr 2005, J Am Coll Nutr 2006),
2) adiponectin gene SNP의 +276위치에 있는 G allele이 한국인에서 환경적인 요소들과 독립적으로 심혈관계질환 위험에 유의적인 영향을 미치는 요소임을 밝혔고(Am J Clin Nutr 2005, Metabolism 2006), 비당뇨 심혈관질환자에서 높은 혈중 adiponectin 농도는 복부비만 정도나 흡연여부에 관계없이 동맥경화증의 위험을 줄일 수 있음을 밝힘(Clin Chim Act 2006).
3) 과체중/비만인에게 12주간 300kcal/d의 열량을 제한시 PLIN 유전자다형성(7개 SNPs)에 따라 체지방 및 유리지방산 농도에 미치는 영향이 달랐으며, 특히 혈중 유리지방산 농도가 지질과산화물 변화를 야기시키는 것으로 나타남(Int J Obes 2006).
4) 대사적 문제를 가진 비만인은 정상적 대사를 보이는 비만인과 비교하여 높은 내장지방 면적 및 더 좋지 않은 대사지표양상을 보였고, 12주간 300kcal/d의 식사제한 결과 혈중지질 및 염증성지표가 더 개선된 것으로 나타났음(Int J Obes 2006).
5) Lp-PLA2 유전자의 V279F에서 F allele가 항동맥경화 인자로 작용하고 Lp-PLA2가 심혈관계 질환의 발병 및 동맥경화 진행에 영향을 미치는 중요한 염증성 지표임을 입증할 수 있었음(J Clin Endocrinol metab 2006).
6) 비당뇨/비비만인에서 RAGE 유전자 G82S의 S minor allele 군에서 sRAGE, 염증성지표, 산화적 스트레스, 인슐린저항성 등 심혈관질환 위험요소가 높은 것으로 나타남(Metabolisml, accepted). 이밖에 2004년 6월 한국 노화학회 및 한일 공동 심포지움을 비롯한 세계적으로 유명한 국제 학술대회인 American Heart Association에 상기의 결과를 발표(2004년 2편, 2005년1편, 2006년 11월 1편 예정)하였음.
본 연구결과는 9편의 세계저명학술지와 5편의 국제학술대회에 발표하여 관련 학계/업계에 귀중한 자료를 제공하였음. 이를 통해 대사중후군/심혈관계 위험에 미치는 환경과 유 전의 상호작용을 과학적으로 규명하였으며, 위험 대상자의 조기선별 및 맞춤형 생활요 법 제안의 근거를 제시하였음.

To determin effects of gene-life environment interaction on obesity related metabolic syndromes and to develop strategy for prevention of metabolic diseases through genome-based lifestyle intervention
In the first year, we searched major candidate hub genes related to metabolic syndrome and cardiovascular disease(CVD) and listed up the major SNPs. Also, we identified the interrelationship between genetic characteristics and environmental factors on metabolic syndrome through additional biochemical/environmental analyses. In the second year, we applied lifestyle modifications to each subjectsgroup( fasting glucose impairment/diabetes, obesity and hypercholesterolemia) and observed the effect according to genetic characteristics (PPARr, APM1, PLIN, APOE). In the third year, we provided the medical examination to publics and suggested each of them with the personalized genome-based lifestyle modification for preventing metabolic syndrome and related diseases. We also provided the valuable information to academic and industrial fields by publishing the above results to the world famous scientific journals and conferences:
1) C allele at promoter1131 of APOA5 gene is closely related to hyperpostprandial lipemia, particularly in high fat diet (Am J Clin Nutr 2005, J Am Coll Nutr 2006)
2) G allele at SNP276 at AMP1 can increase the CVD risk, independently of environmental factors (Am J Clin Nutr 2005, Metabolism 2006, Clin Chim Act 2006)
3) genetic variants at PLIN locus is associated with changes in serum free fatty acids and abdominal fat following mild weight reduction (Int J Obes 2006).
4) subjects with metabolically abnormal obesity showed higher body fat accumulation and impaired metabolic parameters compared to those with metabolically healthy obesity, but they had the big improvement in metabolic risk after 12 weeks-caloric restriction (-300kcal/d) (Int J Obes 2006).
5) Lp-PLA2 can be used as a major risk marker of CVD and F allele at Lp-PLA2V279F plays a role of antiatherogenic index (J Clin Endocr metab 2006).
6) S alleles at RAGE G82S can increase the CVD risk for example, by lowering sRAGE and increasing inflammatory markers, oxidative stress and insulin resistance (Metabolism, accepted).
In addition, we presented our results at American Heart Association in 2004-2006 and the Korea-Japan joint international symposium.
We provided the academic and industrial fields with the scientific information ofinterrelationship between genetic and environmental factors on metabolic
syndrome and CVD by publishing our study results in the world famous scientific journals and conferences. Also, we suggested them how to prescreen high
risk-groups and how to propose personalized lifestyle modifications.
