연구목적
방광내압 측정술은 요실금과 뇌, 척수 손상 환자 등의 방광기능 평가를 위한 중요한 방법이다. 하지만, 현재의 검사 방법은 비생리적으로 식염수를 방광내에 주입하면서 검사를 하게 되어 부정확하며, 환자에게 수치심을 초래하는 검사시간이 길다는 단점이 있다. 본 개발에서는 빈번한 추적검사가 필요한 환자를 대상으로 정확하면서도 비용이 적게 들고, 침상이나 가정에서도 간단히 시행할 수 있는 디지털 방식의 휴대용 방광기능 측정 시스템(방광압, 배뇨압, 배뇨상태 평가)을 개발하고, 임상평가를 수행하였다.

연구방법
연구내용은 방광내압 및 배뇨압력을 휴대용으로 연속 모니터링하고, 데이터의 저장과 분석이 가능한 장치 연구와, 신경인성 방광 환자 데이터 수집과 휴대용 모니터링을 위한 DB 구성, 그리고, 최종적으로, 고안된 장치를 기존에 병원 검사실에 설치된 장비들과 비교 평가하여 임상적 유용성을 평가하는 것이다.

Objectives
This study is to design and calibrate the ambulatory digital urodynamic study system that can discard the rectal pressure catheter, which can make patients more comfortable and doctors can get more physiologic data. We evaluated a method of estimating bladder pressure, bladder neck pressure, and detrusor pressure in patients who perform clean intermittent catheterization to empty the bladder.
Also, We evaluated the change of intra-abdominal pressure related with intravesical and intrarectal pressure in patients with spinal cord injury according to daily activities and postural changes which make the abdominal pressure elevated.
Materials and Methods:The developed device was tested in a bladder model consisting of a fluid-filled balloon as well as in a healthy subject to test the functionality and safety of the device. Subsequently it was clinically tested in a 30-year-old SCI male with an incomplete lesion at the C5 level. The subject sustained the spinal cord injury three months prior to the experiments and had not received any pharmacologic treatment. Patient symptoms included NDO and DSD, but bladder capacity had not yet been established in a conventional urodynamic study. The subject had no bladder sensation and suffered from frequent urinary incontinence episodes. On two separate days the system was tested with and without applied stimulation.
The tests without stimulation provided control data and were used to get an idea of the time-pressure curve of an untreated unstable contraction in order to be able to find a detection threshold. All experiments started with emptying of the urinary bladder. Abdominal and intravesical pressures were sampled at 20 Hz using a fluid-filled CH 2 catheter.
Ten subjects with lower thoracic and lumbar spinal cord injury were enrolled. Their mean age was 38 years. The urodynamic study was performed and the intravesical and the intrarectal pressures were measured with a double lumen catheter and a intrarectal probe. The intravesical and the intrarectal pressures were obtained during patient's speech, sneeze, cough, valsalva and various postural changes with supine to decubitus, decubitus to supine, supine to sit, and sit to supine according to empty and full bladders, respectively.
Conclusion:
A portable device was developed to sense intravesical,abdominal, and urethral pressures and automatically activate electrical stimulation on the dorsal penile/clitoral nerve in order to inhibit anongoing detrusor contraction. A block diagram of the device, together with a photograph of the electronics is shown in Fig. 1. A description of the various parts of the device follows below.
The system has three pressure inputs for recording pressure within the bladder, urethra, and abdomen. Amplification and filtering of the input signals is performed with an instrumentation. amplifier, a first-order passive-input low-pass filter and a second-order active-output low-pass filter.The cut-off frequency of both filters was initially set at 10 Hz, but was changed to 2.5 Hz in the intravesical and abdominal channels in order to attenuate undesired frequency components unrelated to detrusor activity. The preset gain of each amplifier allows a pressure range of 0-210 cm H2O and DC-offsets can be adjusted on all three channels individually.
