본 연구과제는 인간줄기세포를 이용하여 각종 특정세포로의 분화유도와 질병모델 동물로의 이식을 통하여 난치병 치료를 위한 세포대체요법을 개발함과 아울러 조직공학적 기법을 점목하여 줄기세포를 이용한 인공장기의 생산가능성을 타진하기 위해 실시하였다. 지난 4년간의 연구성과를 각 세부과제별로 요약하면 다음과 같다.
제1세부과제는 지난 4년 동안 본 과제에서는 연구자 소속기관인 포천중문의대 차병원 여성의학연구소에 내원하여 시험관아기 시술을 시행한 불임환자로부터 채취된 냉동 잔여배아를 기증받아 총 3종의 인간 배아줄기세포주의 확립에 성공하
본 연구과제는 인간줄기세포를 이용하여 각종 특정세포로의 분화유도와 질병모델 동물로의 이식을 통하여 난치병 치료를 위한 세포대체요법을 개발함과 아울러 조직공학적 기법을 점목하여 줄기세포를 이용한 인공장기의 생산가능성을 타진하기 위해 실시하였다. 지난 4년간의 연구성과를 각 세부과제별로 요약하면 다음과 같다.
제1세부과제는 지난 4년 동안 본 과제에서는 연구자 소속기관인 포천중문의대 차병원 여성의학연구소에 내원하여 시험관아기 시술을 시행한 불임환자로부터 채취된 냉동 잔여배아를 기증받아 총 3종의 인간 배아줄기세포주의 확립에 성공하였고 이를 세부과제 연구자들에게 분양하여 연구가 진행되도록 하였다. 또한 인간배아줄기세포의 안전한 동결보존을 위한 다양한 항동해물질의 안정성 및 동결효율을 극대화 할 수 있는 세포주 보관장치를 포함하는 유리화동결보존술을 개발하는데 성공하였으며 세포주의 대량증식을 유도할 수 있는 배양체계도 개발하는데 성공하였다. 인간배아줄기세포를 이용한 연골세포로의 분화유도를 위한 다양한 분화유도물질의 효능을 검토하였고 분화유도된 연골세포의 특성을 확인하기 위해 RT-PCR, immunocytochemistry 및 western bloting을 실시함으로 성공적인 연골세포로의 분화유도 가능함을 확인하였다. 동시에 인간배아생식세포 확립과정에서 채취가능하였던 인간태아간세포로부터 전분화 능을 갖는 새로운 줄기세포의 발굴에 성공하였고 이 세포주를 이용하여 성공적으로 연골세포로의 분화유도가 가능함을 확인할 수 있었다. 보다 효율적인 세포분화유도 기술을 개발하기 위해 연골세포 분화의 신호전달 기전의 최상위 유전자로 알려진 human Sox9 유전자를 clonig 하였고 이를 각종 유전자 도입방법을 사용하여 유전자 변형 줄기세포주 확립에 성공하였다. 이들 유전자 변형 줄기세포주를 활용하여 보다 수율이 높은 연골세포 분화유도 기술개발이 가능함을 확인하였으며 현재 퇴행성관절염 모델동물에 대한 전임상 연구를 진행중에 있다. 제2세부과제에서는 배아줄기세포 (ES cell)를 이용한 효율적인 세포치료법개발을 위해 C57BL/6 inbred mouse로부터 새로운 ES cell line을 확립하여 특성을 규명했으며, spontaneous differentiation 및 guided differentiation 방법을 통해 각각 심근세포 및 신경세포로의 분화조건을 확립했음. 또한 파킨슨병의 세포치료에 필요한 도파민성 신경세포를 ES cell로부터 효율적으로 분화시키는 방법을 개발했음. 세포이식의 최적조건의 결정을 위해 주입한 세포의 개수에 따른 종양형성빈도를 조사했으며, 아울러 종양형성과 관련된 문제점을 극복하는 방안을 제시했음. 또한, 안전적인 파킨슨병 동물모델을 제작한 후 이를 대상으로 세포이식을 실시한 결과 기능이 회복됨을 조직학 및 행동학적 분석을 통해 확인했음. 제 3세부과제에선는 뇌졸중, 헌팅톤병동물모델을 개발하여 배아간세포이식를 투여, 배아간세포의 분화 및 기능회복과 관련된 효율성을 검증하고 임상환자 적용에 필효한 기초적인 정보를 제공함.
뇌신경질환 (뇌졸중, 헌팅톤병)과 관련된 동물모델개발로 Stroke model은 bilateral carotid arterial clamping method를 이용한 global ischemia model이나 modified Longa's method를 이용한 focal ischemia model. Huntington's disease model은 Quinolinic acid의 unilateral striatal injection을 이용한 방법을 활용함. 세포이식의 투여경로 및 시기적 효율성 검증을 위해 injection site를 intraventricular (동측 및 편측), intralesional, intravenous로 분석하였고 intravenous방법이 가장 less invasive한 적절한 방법으로 분석됨. Disease model후 immediate acute stage, subacute, chronic stage에 stem cell 주입시 효율성 검증한 결과 acute stage에 주입함이 효과가 있음. Differentiated cell line주입시의 효과규명은 별차이가 없었음. 분화양상 분석으로 Neural marker (Neurofilament, MAP-2, NeuN등) 또는 각종 glial marker (GFAP, CNPase, etc)로 분화시기 및 이에 따른 정량적인 분석을 시행하였고, 질환모델에 있어서 기능회복 분석을 위해 Global ischemia: memory task (Water maze, figure 8, passive avoidance test등) Focal ischemia: animal neurologic scale, rotating behavior, HD model: rotating behavior, rota-rod test 및 기타 grasping behavior 측정하였으며, rota rod test 및 apomorphine induced rotation test가 가장 reliable한 결과를 보임. 이러한 자료는 향우 임상환자 적용에 있어서 기초적인 전임상 자료로 활용 될것으로 사료됨.
제4세부과제에서는 Adult 조혈모세포와 ES 세포로부터 분화 유도된 조혈모세포의 특징과 기능상의 차이점을 비교, 분석하여 성공적인 골수이식을 위해 상호보완적으로 사용할 수 있는 방법을 확립하기위하여 adult 조혈모세포를 분리하고 장기생착 가능한 조혈모세포를 분리함. OP9 supporting cell system을 이용하여 효과적으로 J1 ES cell line으로부터 hematopoietic cell로 분화시킴. ES에서 조혈모세포로 분화시키는 과정은 성공적으로 이루어져 조혈모세포 marker로 확인하였으나, ES cell에서 분화 유도시키는 조혈모세포의 functional limitation으로 인하여 (lymphoid potential의 기능부족) in vivo transplantation을 실시하여 adult 조혈모세포와 비교하지 못함. 그 대신 조혈모세포의 장기생착 및 골수귀소 관련 연구로 연구방향을 전환함. 그에 대한 연구성과로서는 조혈모세포의 장기생착에 결정적인 CXCR4의 internalization을 확인함. 조혈모세포의 장기생착과 모든 myeloid와 lymphoid 세포로 분화하는데 중요한 역할을 하는 factor를 stem cell aging model에서 발견함. adult 조혈모세포의 transdifferentiation potential을 장기손상모델에서 실험함 (lung 손상모델이용) 또한 심근경색모델을 이용하여 계속 연구가 진행 중에 있음. 제5세부과제에서는 치아 손실을 대체하기 위해 다양한 치료방법이 행해지고 있으나, 기존의 치료법은 치아의 손실을 원상태로 그대로 복원시키지 못할 뿐만 아니라, 감염에 이환되기 쉽고, 치아의 불필요한 삭제를 동반하고 있음. 요즘 많이 시행되고 있는 임플란트와 같은 치료법도 감염의 위험성과 고비용이어서 많은 사람이 혜택을 받을 수 없음. 원래의 치아 형태 및 치아 기능을 복원시킬 수 있는 치료방법으로서 줄기세포를 이용한 인공치아의 제작이 제안되었으나 전세계적으로 그 연구가 현재 걸음마 단계 수준에 있음. 줄기세포를 이용한 인공치아의 배양에 있어 중요한 요소들은 줄기세포를 이용하여 원하는 크기, 형태, 구조, 기능, 생체적합성을 모두 만족시킬 수 있는 인공치아를 빠른 배양시간 안에 대량으로 생산할 수 있는 기술을 개발하는 것임. 우리의 연구 전략은 치아의 고유한 형태가 이루어지는 과정, 즉 pattern formation과 morphogenesis를 세포 및 분자생물학적 수준에서 규명하고, 줄기세포를 이용한 인공치배의 제작 가능성과 형태 및 크기를 신호전달물질로써 조절하는 것임. 줄기세포를 이용한 인공치아제작을 위하여 먼저 여러 가지 실험 시스템을 개발 및 평가하였음. reaggregation법, 재조합법(recombination), 교차재조합법(cross-recombination), 절단배양법(slice culture), 신장에 치배를 이식하는 방법 등등이 인공치아 제작에 이용될 수 있는 방법으로 개발되었음. 줄기세포는 성체줄기세포와 배아줄기세포를 얻어 재응집시킨 다음 배아의 치아상피와 결합시키는 방법을 진행하였음. 현재까지 성공을 하지는 못했지만 조금씩 조건을 바꾸어가며 계속 진행 중에 있음. 또하나 인공치아에서 중요한 치아의 형태와 크기를 조절하기 위해 여러 가지 방법을 시도하였음. 유전자를 이용하여 치아의 형태와 크기를 조절하는 특이 유전자와 방법을 알아냈으나 더 정밀한 조절을 위해서 다양한 형태조절 유전자의 발굴이 필요함. 이상의 연구 결과를 토대로 줄기세포를 이용한 인공치아의 제작 가능성을 한층 높였다고 할 수 있겠음.
제6세부과제에서는 줄기세포와 조직공학을 이용하여 바이오인공연골과 심장근육을 개발하는 연구를 시행하였음. 효율적 인공장기 생성을 위한 최적의 생분해성 매트릭스를 생분해성 합성고분자와 천연고 분자를 재료로 개발함. 주사요법이 가능한 매트릭스는 덜 가해적인 방법으로 세포이식을 가능하게 함. 개발된 삼차원적 매트릭스의 생체적합성을 측정함. 줄기세포로부터 분화된 연골세포, 심장근육세포의 삼차원적 매트릭스에 부착시키고 배양하는 방법을 최적화함. 먼저, 면역결핍 쥐의 피하조직에 줄기세포와 매트릭스를 이식하여 연골조직재생을 확인함. 토끼무릎연골 결손모델에서 줄기세포/매트릭스 복합체를 이식하여 연골을 재생시킨 후, 재생된 연골조직의 생성과 기능을 조직학적, 생화학적, 기계물성학적으로 조사함. 허혈성 심장 쥐 모델에서 배아줄기세포로부터 분화된 심근세포/매트릭스 복합체를 심장에 이식하여 성공적으로 심근이 재생되었고, 심장의 기능도 개선됨. 심근재생용 세포 이식시 G-CSF 처리는 심근의 혈관재생과 심근기능 회복을 더욱 향상시킴.

These project were forcus on the development of cell replacement therapy and bioartificial oragn through human stem cell differentiation. For achieving the goals of this project, six sub-project were divided for the effective research. The results obtaind from each sub-project were as follows; In the 1th sub-project, we have performed a study on establishment and characterization of human embryonic stem cell lines and its differentiation into chondrocyte for the development of cell therapy. First, we are establish and characterize the 3 lines of human embryonic stem cell (hES) and develop the large scale culture system by using specific culture methods. Futhermore we develop the new type of human stem cell from human fetal liver tissues ranging 8-10 weeks which have pluripotent related maker expression and differnetiated into the various type of cells in vivo and in vitro. Until now, we are eatablish the effective cryopreservation method for hES cells and hFLDSC. For the induction of chondrocyte differentiation from established hES cell and hFLDSC, we are developed the effective chondrocyte differentiate by using treated with various combinations of cytokine and chemicals. In addition we also developed the chondrocyte differnetiaion method by using hSox9, master gene for chondrocyte development, overexpressed hES cells. For the development of more efficient differentiation method for chondrocyte, we are apply the injection of hES cell or hFLDSC with differentiation inducible factor combined hydrogel. From our results suggest that hES cell and hFLDSC were good candidate celll for degenerative arthritis cell therapy.Sub-Project 2: In order to develop efficient cell therapy strategies using embryonic stem (ES) cells, we have carried out the following studies: 1) isolation and characterization of a new ES cell lines from C57BL/6 mice 2) differentiation into cardiomyocytes and neural tissues by spontaneous or guided differentiation methods, respectively 3) efficient generation of dopaminergic neurons from ES cells 4) determination of optimal transplantation conditions 5) understaing and prevention of tumor formation following ES cell transplantation 6) stable generation of a rodent model of Parkinson's disease 7) functional recovery following transplantation, judged by histological and behavioral analyses. In the 3rd sub-project, purpose of research was to evelop efficient cell transplantation strategies in stroke and Huntington's disease. Experimental animal models was established using Quinolinic acid in Huntington's disease, and forebrain ischemia model for stroke. For the development of efficient conditions for neuronal differentiation, differentiation of neural precursors, neurons and GABAergic neurons from mouse and human ES cells, different injection routes, cell numbers, time window and assessment of functional recovery using systematic behavioral criteria were performed. Intravenous injection was less invasive but efficient method and application of stem cell in acute stage is more appopriate. The behavioral test shows that rotatod and apomorphin-induced rotation tests were most reliable. Accumulation of pre-clinical data through the analysis on the safety and efficacy of the stem cell transplantation using small animals (e.g., rodents) would provide the strategic development of clinical trials using end-stage human patients. In the 4th subproject, we investigated the characteristics and functions of hematopoietic cell that differentiated from adult hematopoietic stem cells and ES cells. To establish a useful method for a successful bone marrow transplantation and long-term marrow engrafting adult hematopoietic stem cells from mouse bone marrow, human bone marrow and human umbilical cord blood were purified.
J1 ES cells successfully differentiated into hematopoietic cells in OP9 supporting cell system. Hematopoietic differentiation from J1 Es cells was confirmed by flow cytometry using markers for hematopoietic stem/progenitor cell and mature hematopoietic cell. However, ES cell has some functional limitations for lymphoid potential. Therefore we couldn't compare in vivo transplantation capacity of ES cell with the transplantation capacity of adult hematopoietic stem cells without any modification of in vitro hematopoietic differentiated ES cells to overcome this defects. Instead of that, we investigated organ-specific engraftment and bone marrow homing of hematopoietic stem cell. As a result of it, we confirmed the internalization of chemokine receptor, CXCR4 that play an important role in organ-specific engraftment of hematopoietic stem cell. We found a important factor for organ-specific engraftment and lymphoid / myeloid switching mechanism of hematopoietic stem cell in stem cell aging model. We are continuing investigation of a potential for transdifferentiation of adult hematopoietic stem cell using damaged-organ model. In the 5th sub-project, there are many kinds of treatment to replace the space, in which teeth were lost. Most of treatments have many disadvantages such as infection-susceptibility and unnecessary removal of tooth structures, and tooth structures could not regenerate completely after these treatments. Nowadays, dental implant is very popular treatment with minimum removal of tooth structure, but it can not regenerate tooth structures. To regenerate not only the tooth structure but also the shape and function of tooth, new strategy called as artificial tooth using stem cells has been suggested. Important factors for the artificial tooth using stem cells are as follows. Right size, right shape and right function, biocompatibility of artificial tooth have to be fulfilled in an appropriately short time. Our strategies are composed of two components. The one is the identification of the mechanism in patterning and morphogenesis during normal tooth development, and the other is the regulation of the size and shape of artificial tooth with this mechanism. At first, various culture systems for artificial tooth germ and stem cell reaggregation method were invented and evaluated. To identify the signalling molecules in controlling the size and shape of tooth, recombination and cross-recombination methods were applied beween tooth germs of various species. Stem cells have been reaggregated and recombined with embryonic dental epithelium under the various conditions. Some signaling molecules were found to be related with the size and shape of tooth, but more signaling molecules have to be identified for the delicate controls of size and shape. Our results show the increasing possibility in the formation artificial tooth.In the 6th sub-project, we have performed a study on regeneration of cartilage and myocardium using stem cells and tissue engineering. Cell transplantation matrices were fabricated from biodegradable synthetic polymers and natural polymers. Injectable scaffolds enable us to transplant stem cells with less invasive method. Biocompatibility of the developed matrices were evaluated. We optimized the methods of stem cell adhesion and culture on the matrices. Tissues were regenerated by implanting bone marrow stem cells and matrices into subcutaneous space of athymic mice. In knee defects of rabbits, cartilage were regenerated by implanting bone marrow stem cells and matrices, and the regenerated tissues were characterized with histological, biochemical, and mechanical assays. In infarctd myocardium of rats, myocardium was successfully regenerated by implanting embryonic stem cells and matrices, and cardiac functions were improved. G-SCF treatment potentiated the cell therapy in terms of microvessel regeneration in the infarcted myocardium and cardiac function recovery.
