Ⅰ. 연구개발의 목적 및 필요성
이제까지 항암 면역치료에 대한 연구는 주로 암세포에 대한 면역관용이나 면역회피를 극복하기 위하여 면역반응을 효과적으로 유도하거나, 암세포를 공격하는 실행세포의 활성을 높이는 방법들에 대한 것이었다. 그러나 임상시험 결과는 실용적인 측면에서 성공적이라 할 수 없다. 이것은 생체내 면역반응이 실행 세포와 조절세포 간의 긴밀한 상호작용을 통해 면역 항상성을 추구하기 때문이다. 즉, 실행세포의 활성이 증가할수록 조절세포의 억제활성도 함께 증가하여 항상성을 유지하려는 되먹이기 조절기전이 있기 때문이다.
Ⅰ. 연구개발의 목적 및 필요성
이제까지 항암 면역치료에 대한 연구는 주로 암세포에 대한 면역관용이나 면역회피를 극복하기 위하여 면역반응을 효과적으로 유도하거나, 암세포를 공격하는 실행세포의 활성을 높이는 방법들에 대한 것이었다. 그러나 임상시험 결과는 실용적인 측면에서 성공적이라 할 수 없다. 이것은 생체내 면역반응이 실행 세포와 조절세포 간의 긴밀한 상호작용을 통해 면역 항상성을 추구하기 때문이다. 즉, 실행세포의 활성이 증가할수록 조절세포의 억제활성도 함께 증가하여 항상성을 유지하려는 되먹이기 조절기전이 있기 때문이다. 본 연구에서는 생체내 면역항상성 이론을 바탕으로 실행세포가 아닌 조절세포를 표적으로 하는 생체내 면역조작을 통한 항암 면역치료 전략에 대한 타당성을 검토하고자 하였다.
Ⅱ. 연구개발의 내용 및 범위
우선, 조절T림프구(regulatory T cells, Tregs)의 활성을 정량적으로 분석할 수 있는 시험법을 확립하였다. 조절이라는 개념 자체가 정량적 의미를 내포하고 있는 만큼, Treg 활성의 정량 분석은 매우 중요하다. 그러나 실험에 사용되는 CD4+CD25+ 세포 분획이 실제 Treg이라 할 수 있는 FoxP3+ 분획과 차이에 기인한 오류 때문에 Treg 활성 시험법의 신뢰성이 떨어져 있었다. 본 과제를 통해 오류의 원인을 분석하고 새로운 정량 분석법을 확립하였다. 이를 바탕으로 Treg 활성 변화를 표현할 수 있는 수학적 모델을 확립하였고, 자연면역 자극 물질 및 천연물 272종을 스크리닝하여 Treg의 활성을 억제하는 물질 8종을 선정하였다. 이중 6종이 생체내 실험을 통해 Treg의 활성을 억제하여 면역항진효과가 있음을 최종적으로 확인하였다. 아울러 흑색종 폐전이 모델 및 국소 유방암 모델에서 생체내 면역 항상성의 변화를 분석하였다.
Ⅲ. 연구개발결과
Treg은 자극 강도에 따라 억제 양상에 차이를 보인다. 즉, 약한 자극과 강한 자극에 대해 초기 분열증식 반응 및 후기 증폭반응을 대변할 수 있는 두 가지 정량변수, precursor frequency와 CD25MFI를 사용한 정량분석법을 확립하였다. 이 분석법에 의해 IL-6와 IL-21에 의한 contra-suppression 현상을 직접 확인하였으며, 그 기전에 관해 Teff에서의 IL-2 생산 및 IL-2Rα 발현 증가, 증폭된 Treg에서 FoxP3 발현 감소 등의 현상을 관찰하였다. 아울러 CTLA-4를 차단하면, 강한 자극의 경우에만 Treg의 활성이 억제되고 약한 자극의 경우에는 차이가 없었다. Treg 활성의 정량분석을 통해 스크리닝한 자연면역 자극 물질 중에서는, Pam3CSK4, LPS-EK, FSL1, ssRNA40, ODN1826 이 Treg 활성을 억제하는 효과가 있었다. 천연물 중에서는 괴하, 마발, 번맥수, 시호, 아출, 연자, 자화지정 및 지구자가 시험관내 시험에서 Treg 활성을 억제하였는데, 이중 마발, 번맥수, 시호, 아출, 연자, 자화지정이 생체내 실험에서 Treg 활성을 억제함으로써 면역항진효과가 있었다. 흑색종 세포주 B16을 꼬리 정맥에 주사하여 유발시킨 폐 전이 흑색종 모델에서 Teff와 APC의 활성이 저하된 반면 Treg의 활성을 항진되어 면역반응성이 저하되어 있었다. 반면에, 유방암 세포주 4T1을 피하 주사하여 유발시킨 국소 유방암 모델에서는 면역반응성이 항진되어 있었다.
이러한 차이는 암종, 암 진행 경과 및 전이 여부에 따라 생체내 면역반응성에 변화가 있음을 시사하였다.
Ⅳ. 연구개발결과의 활용계획
본 과제 연구 결과, 생체내 면역항상성 이론에 기반하여 Treg을 표적으로 하는 항암 면역치료 전략의 타당성을 확인하였다. 아울러 암종에 따른 차이, 암의 진행 및 전이 여부에 따른 면역반응의 차이를 확인하였다. 이러한 결과는 각각의 암종에 따라 구체적인 항암 면역치료법 개발에 관한 후속 연구의 필요성을 시사하는 것이다. 아울러, 본 연구를 통해 확립한 Treg 활성의 정량 분석법 및 스크리닝 결과를 후속 연구를 통해 신약 개발 연구에 활용할 계획이다.

In order to develop an effective anti-cancer immunotherapy, many investigators have studied on the methods that can enhance the effector function of immune responses, such as cytotoxic T cells, NK cells, dendritic cells, etc.
Many methods have been developed so far, and proved to be effective in vitro. However, almost all the methods turned out to be failure in the clinical studies, suggesting a different regulatory mechanism in vivo from that in vitro. Previously, we have proposed a new concept on the immune regulation in vivo, immune homeostasis maintained by regulatory T cells (Tregs). According to the feedback theory, we have to move the target of in vivo immunomodulation from effector cells to regulatory cells. In the present study, based on our hypothesis, "immune-homeostasis maintained by Tregs in vivo," we investigated on the anti-cancer immunotherapeutic strategy targeting Tregs.
First of all, we established a quantitative functional assay for Tregs. It is very important to quantitatively assess the suppressive function of Tregs, while 'regulation' itself implies a quantitative concept. Nonetheless, in vitro suppressive assay for Tregs were error-prone. The reasons are complicated, mainly due to the difference between the true FoxP3+ Tregs and CD4+CD25+ cells that are popularly used in the experiments as Tregs. Accordingly, the in vitro suppressive assay was regarded to be unreliable. In the present study, we dissected the reasons raising the errors in the suppressive assay and established an alternative error-free quantitative assay. The assay employs two quantitative parameters, precursor frequency and CD25 MFI, that reflect early and late proliferative responsiveness.
By using the quantitative assay, we developed a mathematical model for the suppressive function of Tregs, that can explain how immune balance is changing in vivo in an easy way. The quantitative assay directly shows the phenomenon, 'contra-suppression,' that is, the suppressvive activity of Tregs is suppressed by IL-6 and/or IL-21. IL-6 and/or IL-21 increased the production of IL-2 and expression of IL-2Rα in Teffs, and decreased the expression of FoxP3 in the expanded Tregs.
Blocking CTLA-4 abrogated the suppressive activity of Tre gs only in response to strong stimulation, suggesting that CTLA-4 is not essential in the suppressive action by Tregs at least in response to weak stimulation. We also screened TLR ligands, dactin, and natural products for the modulating effect on Tregs. Out of the 272 natural products, eight samples were selected for their suppressive effects on the function of Tregs. Finally, six samples, Lasiosphaera fenslii, Zea mays, Bupleurum chinense, Curcuma zedoaria, Nelumbo nucifera, and Viola yedoensis, were turned out to be effective in vivo in the immune enhancement by suppressing Treg function. In addition, mouse models for melanoma was established by injecting B16 melanoma cells into tail vein. After 2 weeks, metastatic lung cnacers were observed grossly as well as microscopically. Teff and APC functions were decreased while
Treg function was increased in the mice with metastatic lung cancers. By contrast, in the mice with local breast cancer developed by subcutaneous inoculation of 4T1 cells, Teff and APC function was increased while Treg function was decreased. The results suggest that immune balance is changing in vivo according to the type, progression and presence or absence of metastasis of cancers.
In conclusion, the results in the present study validated the anti-cancer immunotherapeutic strategy targeting Tregs based on the hypothesis, "immune-homeostasis maintained by Tregs in vivo." Meanwhile, we also found different immune balancing in mice with different cancers.
