아토피피부염은 유소아에서 발생하여 흔히 성인까지 지속되는 심한 소앙증과 특징적인 피부 소견을 보이는 만성 염증성 질환임. 최근 사회의 공업화, 산업화에 따른 환경 오염, 생활 습관의 변화, 유전적 영향 등에 의해 아토피피부염의 발병률이 점차 증가하는 추세를 보이고 있음. 아토피피부염의 병인으로는 유전적 배경과 면역학적 기전, 환경적 요인 등의 다양한 인자가 복합되어 있는 것으로 알려져 있지만 주된 발병 기전이 무엇인지는 아직 명확히 밝혀져 있지 않음.
땀분비가 아토피피부염 환자를 악화시키는 것으로 알려져 있으나 그 기전에 대해
아토피피부염은 유소아에서 발생하여 흔히 성인까지 지속되는 심한 소앙증과 특징적인 피부 소견을 보이는 만성 염증성 질환임. 최근 사회의 공업화, 산업화에 따른 환경 오염, 생활 습관의 변화, 유전적 영향 등에 의해 아토피피부염의 발병률이 점차 증가하는 추세를 보이고 있음. 아토피피부염의 병인으로는 유전적 배경과 면역학적 기전, 환경적 요인 등의 다양한 인자가 복합되어 있는 것으로 알려져 있지만 주된 발병 기전이 무엇인지는 아직 명확히 밝혀져 있지 않음.
땀분비가 아토피피부염 환자를 악화시키는 것으로 알려져 있으나 그 기전에 대해서는 아직 확실치 않음. 아토피피부염 환자의 경우 약 80%의 환자가 7월, 8월 같은 여름에 악화된다고 보고하였고 그 다음으로 11월, 12월, 1월 같은 겨울에 악화됨을 보고함. 여름에 악화되는 원인으로는 땀이 가장 큰 원인이었으며, 땀에 의해 악화될 경우 대부분 땀을 흘리고 난 1시간 내에 악화되었으며 주로 2-3일 이내에 호전되는 경향을 보임. 땀에 의해 악화되는 경우 가려움증이 가장 많았으며 이차적으로 긁은 병변도 많이 발생하였으며 약 10%의 환자에서 는 두르러기 양상의 병변을 호소함. 여름이나 스트레스 또는 운동에 의해 땀분비가 발생하는 경우 대다수 악화되었으나 목욕이나 사우나의 경우 약 40%의 아토피피부염 환자에서 악화를 호소함. 아토피피부염 환자의 경우 목, 머리, 등, 팔 오금, 겨드량이, 다리 오금 순으로 땀분비가 많이 되었으며 땀분비에 의해 악화되는 경우 목, 팔 오금, 다리 오금순으로 나타남. 프로테오믹스 기법을 이용하여 아토피피부염 치료 후 호전된 환자에서 lipid와 연관된 Apolipoprotein A-IV 단백의 증가를 확인할 수 있었으나 아토피피부염 환자에서 HDL, LDL, Total cholesterol의 경우 중증도와 연관이 없었으며 비만도 또한 아토피피부염 중증도와는 연관이 없었음.
결론적으로 아토피피부염에서 땀분비에 의한 땀의 악화는 목이나 팔 오금, 다리 오금 같은 굴측부의 전형적인 병변의 악화와 연관이 있으며 프로테오믹스 기법을 통한 표적 단백 발굴은 땀에 의한 악화 기전 및 나아가 치료제 발굴에 많은 도움을 줄 수 있을 것으로 사료됨.

Atopic dermatitis (AD) is a highly pruritic, chronic relapsing inflammatory skin disease. Sweating is known to aggravate AD and autologous sweat is reported to cause allergic reactions in patients with AD. However, the exact role and the mechanism of aggravation by sweating are unclear. In this study, we investigated the role of sweating in the aggravation and pathogenesis of AD.
We enrolled patients with AD from the Severance hospital. Patients were diagnosed according to the criteria of Hanifin and Rajka. Clinical history and features were evaluated and the severity of AD was scored by eczema area and severity score (EASI). Patients were requested to complete the questionn aire which asked them the following aspects; whether sweating aggravates eczema, when, how it does and several other questions on sweating and their symptoms.
Among AD patients, eczema was aggravated by sweating in more than 80 percentages of AD patients usaually during summer season such as July and August. Most patients showed pruritus, and then secondary skin change due to pruritus. Approximately 10% of AD patients showed urticaria-like lesion after sweating. The time of aggravation after sweating was usually within one hour but aggravation persists more than one week in some cases. Most patients showed aggravation by sweating during summer season, exercise, or stress, while only 40% of AD patients experienced aggravation by sweating during bathing or taking a sauna. The most commonly aggravated sites were flexural areas, such as neck, antecubital fossa and popliteal fossa.
Through proteomic approaching from AD patients, we found that apolipoprotein A-IV (ApoA-IV) spot was up-regulated after HDM immunotherapy. Since ApoA-IV is a glycoprotein that circulates in association with high density lipoprotein (HDL), ApoA-IV may affect the lipid profile in AD patients. Among lipid profiles, total cholesterol and LDL were lower in AD patients than in urticaria patients. In addition, we analyzed relationships between clinical parameters, including EASI score, pruritus, sleep disturbance, and lipid profiles, but showed no correlation between lipid profiles and clinical parameters.
This study suggests that sweating can be one of aggravating factor of AD and the causative sweat antigen can become one of important targets in the treatment and management of AD.
