IV. 연구개발결과의 활용계획 인간 골격근육병증 환자에게서 발견되는 정토필린2 돌연변이들인 S165F와 Y141H 에 의한 근육 세포 비대 현상은, 돌연변이 정토필린이 PKC에 의한 무인산화, TRPC3 칼슘 채널과의 결합 이상, Orai1 칼슘 채널의 발현 증가 등으로 인해 최종적으로 칼슘 신호 이상을 유발하여 발생하는 것으로 판단할 수 있다. Y141H가 과발현된 근육 세포의 비대 현상은 S165F의 과발현과는 다른 기작에 의해 일어남을 확인하였고, 이는 인간 비대심장근육병증의 분자적, 세포적 발병 기작을 이해하는 초석이 될 것으로 예상한다.

Junctophilins (JPs) contribute to the formation of junctional membrane complexes in muscle cells by physically linking the transverse (t)-tubule and sarcoplasmic reticulum (SR) membranes. In humans with hypertrophic cardiomyopathy (HCM), mutations in JP2 are linked to altered Ca2+ signaling in cardiomyocytes, however, the effects of these mutations on skeletal muscle function have not been examined. Herein, we tried to identify the dominant negative role of a JP2 mutation (S165F which is associated with human HCM) in skeletal muscle. Consistent with the hypertrophy observed in human cardiac muscle, over-expression of S165F mutant in primary mouse skeletal myotubesled to a significant increase in myotube diameter and resting cytosolic Ca2+ concentration. Single myotube Ca2+ imaging experiments showed reductions in both the gain of excitation-contraction coupling and ryanodine receptor1-mediated Ca2+ release from the SR. Immunoprecipitation assays revealed defects in phosphorylation of S165F mutant at S165 by protein kinase C and in binding of S165F to TRPC3 (canonical-type transient receptor potential cation channel 3 on t-tubule membrane). Therefore, both the hypertrophy and altered intracellular Ca2+ signaling in the S165F-expressing skeletal myotubes maybe linked to altered phosphorylation of JP2 and/or altered cross-talk among Ca2+ channels on t-tubule and SR membranes. Consistent with S165F-expressing skeletal myotubes, over-expression of Y141H led to a significant increase in myotube diameter and resting cytosolic Ca2+ concentration. Unlike S165F, single myotube Ca2+ imaging experiments with Y141H-expressing myotubes showed a reduction in the gain of excitation-contraction coupling without an alteration in both ryanodine receptor1-mediated Ca2+ release from the SR and the amount of SR Ca2+. Immunoblot assays suggested that the increased resting cytosolic Ca2+ concentration is possibly due to the increased expression level of an extracellular Ca2+-entry channel, Orai1. Indeed, store-operated Ca2+-entry (SOCE) and punta formation by Orai1 and STIM1 were significantly increased in Y141H-expressing myotubes. Therefore, the hypertrophy in the Y141H-expressing skeletal myotubes would be differently progressed from that in the S165F-expressing skeletal myotubes, and abnormal SOCE could be the main cause of the hypertrophy in the Y141H-expressing skeletal myotubes.
