세계보건기구(WHO), 국제원자력기구(IAEA) 등 6개 국제기구에서는 1996년에 공동으로 두부 촬영 등 엑스선검사에서 환자가 받는 참고선량(Guidance Level)을 발표한 바 있다. 또한 국제방사선방어위원회(ICRP)에서도 환자의 방사선방어를 최적화하고 환자가 받는 방사선량을 줄이기 위하여 2007년에 신 권고인 ICRP 103을 권고하면서 환자선량 권고량(Diagnostic Reference Level)을 자국에 적정하게 확립하도록 하고 있다. 그러나 소아의 경우 국내에서는 흉부촬영 부위에서만 환자선량 권고량이 마련되어
세계보건기구(WHO), 국제원자력기구(IAEA) 등 6개 국제기구에서는 1996년에 공동으로 두부 촬영 등 엑스선검사에서 환자가 받는 참고선량(Guidance Level)을 발표한 바 있다. 또한 국제방사선방어위원회(ICRP)에서도 환자의 방사선방어를 최적화하고 환자가 받는 방사선량을 줄이기 위하여 2007년에 신 권고인 ICRP 103을 권고하면서 환자선량 권고량(Diagnostic Reference Level)을 자국에 적정하게 확립하도록 하고 있다. 그러나 소아의 경우 국내에서는 흉부촬영 부위에서만 환자선량 권고량이 마련되어 있고, 두부 촬영에서의 환자선량 권고량이 마련되어 있지 않다. 따라서 본 연구에서는 전국 131개 의료기관을 대상으로 132대 장치에 대하여 소아 두부 촬영 시 진단영상정보를 분석하고, 유리선량계를 사용하여 의료기관의 환자선량을 측정·분석하여 우리나라 실정에 적정한 환자선량 권고량을 확립하였다.
전국 131개 의료기관에서의 소아 두부 촬영 시 진단영상정보를 조사한 결과 두부 전후면 촬영에서는 관전압이 52.0 ~ 85.0 kVp를 사용하였으며 평균적으로 68.7 kVp를 사용하였으며, 관전류-시간곱 (mAs)는 2.50 ~80.00 mAs를 사용하였고, 평균적으로는 15.43 mAs를 사용하였다. 또한 두부 측면 촬영에서는 관전압이 52.0 ~ 81.0 kVp를 사용하였으며 평균적으로 67.7 kVp를 사용하였으며, 관전류-시간곱(mAs)는 2.40 ~71.00 mAs를 사용하였고, 평균적으로는 13.78 mAs를 사용하였다. 소아 두부 촬영 시 환자가 받는 입사표면선량(Entrance Surface Dose)을 유리선량계로 측정하고 그 자료를 통계처리한 결과 두부 전후면 촬영에서의 환자가 받는 입사표면선량은 0.18 mGy ~ 3.52 mGy이었고, 평균값은 0.83 mGy, 제1사분위값은 0.51 mGy, 중앙값은 0.71 mGy, 제3사분위값은 1.00 mGy이었으며, 두부 측면 촬영에서의 환자가 받는 입사표면선량은 0.12 mGy ~ 3.19 mGy이었고, 평균값은 0.66 mGy, 제1사분위값은 0.40 mGy, 중앙값은 0.57 mGy, 제3사분위값은 0.80 mGy이었다.
연구결과에 따라 소아 두부 전후면 촬영에서의 환자선량 권고량은 1.0 mGy, 측면 촬영에서의 환자선량 권고량은 0.8 mGy로 확립하였다. 환자선량 권고량은 국내 의료기관에서 소아 두부 촬영 시 환자가 받는 방사선량을 줄이고 환자의 방사선방어를 최적화 하는데 사용하여 국민보건 향상에 기여할 것으로 사료된다.

In subsequence, 6 world organizations, including World Health Organization(WHO) and IAEA co-published in 1996, the Guidance Level for the amount of exposure during X-Ray examinations including skull radiography. In addition, in order to optimize the patient radiation protection and to reduce the level of radiation for the patients, International Commission on Radiological Protection(ICRP) gave its recommendation ICRP 103 in 2007 imposes on each of their countries to establish Diagnostic Reference Level to the appropriate level.
However in Korea, Diagnostic Reference Level for pediatric was established only chest X-Rays, and such references are not yet established for pediatric skull radiography. Therefore in this study, the direction of pediatric skull radiography and details on patient dose were investigated by survey, and the dose measurement is targeted in general hospitals that participated in the survey. The dose measurement was done with head phantom, and a radiophotoluminescent glass rod dosimeter was utilized in 131 medical institutions nationwide.
It was shown that the tube voltage of 52.0∼85.0 kVp was used for anterior-posterior pediatric skull radiography and an average 68.7 kVp was used. Also, as for tube current– time product(mAs), 2.50∼80.00 mAs was used and the average 15.43 mAs was used. In the case of Lateral pediatric skull radiography, the tube voltage of 52.0∼81.0 kVp was used and an average 67.7 kVp was used. Also, as for tube current– time product(mAs), 2.40∼71.00 mAs was used and average 13.78 mAs was used. The survey and dose measurements on the patient dose showed that the 75th percentiles in the distribution of pediatric skull radiography were 1.0 mGy (average: 0.83 mGy) and 0.8 mGy (average: 0.66 mGy) for the anterior-posterior direction and for the lateral pediatric direction, respectively. Based on these research data, the target patient doses that are believed to be achievable for pediatric skull radio-examination in Korean circumstances are about 0.18∼3.52 mGy on anterior-posterior and 0.12∼3.19 mGy on lateral
These values can be used by hospitals as a diagnostic reference level to optimize patient protection of the patients and to reduce the doses of skull X-ray that are imposed on the patients during the skull radiography in Korea.
