연구의 목적 및 내용
최근 재활의학과, 신경정신과 등에서 뇌졸중, 우울증, 간질, 통증 질환 등의 치료를 위해 급속도로 보급되고 있는 경두개직류자극기(tDCS)의 문제점인 넓은 자극 부위, 해부학적 정보 미활용으로 인한 자극 효율 저하 등을 해결하여 환자 개개인의 해부학적인 영상 정보를 바탕으로 향상된 공간 해상도로 자극을 가능하게 하는 신개념의 뇌신경 전기자극 기술 및 시스템 개발을 목표로 함

연구의 목적 및 내용
최근 재활의학과, 신경정신과 등에서 뇌졸중, 우울증, 간질, 통증 질환 등의 치료를 위해 급속도로 보급되고 있는 경두개직류자극기(tDCS)의 문제점인 넓은 자극 부위, 해부학적 정보 미활용으로 인한 자극 효율 저하 등을 해결하여 환자 개개인의 해부학적인 영상 정보를 바탕으로 향상된 공간 해상도로 자극을 가능하게 하는 신개념의 뇌신경 전기자극 기술 및 시스템 개발을 목표로 함
연구결과
- 본 연구 수행을 통해 연구계획서 작성 당시 계획한 세계 최초의 Array 타입의 영상유도 tDCS 시스템의 설계 및 제작을 완료하였으며, 두개골 팬텀 실험, 소동물 대상 실험, 인체 대상 테스트 등을 통해 제작 시스템의 성능을 검증하였음
- 이와 함께, 계획한 바와 같이 tDCS 전류의 정량적 분석 기술을 개발하고 이를 이용하여 extracephalic reference 전극의 효과 및 안전성 검증, craniectomy 환자에의 tDCS 적용 가능성 연구 등을 성공적으로 수행하였을 뿐 아니라 일반 연구자들이 tDCS 전류의 정량적 분석이 가능하도록 Matlab 툴박스를 개발하여 전세계에 배포하였음
- 기타 연구 성과로서 기능적 근적외선분광기를 이용한 소동물 실험을 통해 최초로 tDCS 자극 동안의 뇌혈류량 변화를 모니터링하였고, tDCS가 작업기억능력에 미치는 영향도(효과)의 개인차가 개인의 해부학적 차이에 기인할 것이라는 가설을 뒷받침하는 연구 결과물을 도출하였음
연구결과의 활용계획
- 본 연구에서 개발한 영상유도 tDCS 시스템은 기존 시스템이 지니는 넓은 자극 부위, 해부학적 정보 미활용으로 인한 비효율성 문제를 해결하면서도 tDCS 시스템이 가지는 장점인 높은 이동성, 상대적 가격 메리트, 피험자의 낮은 거부감, 임플란트 환자에게의 적용가능성 등을 유지할 수 있으므로 다양한 임상 분야에 적용이 가능함
- 개발한 시스템을 2곳 이상의 협력 연구 그룹에 설치하여 공동으로 실험을 수행하고 있으며 개발한 Matlab toolbox(Comets)의 전문가 버전을 상용화하기 위한 개발을 지속하고 있음
- 본 과제와 관련한 원천 기술에 대한 특허를 출원하고 현재 등록을 기다리고 있는 상황으로 향후 관련 기업에 기술이전이 가능할 것으로 예상됨
- 연구 종료 이후에도 임상/기초 연구진과의 공동 연구 형태로 지속적으로 연구를 수행할 계획이며 관련 분야에서 국내 연구자들의 연구에 도움이 될 뿐만 아니라 동 분야에서의 위상을 제고할 수 있을 것으로 예상됨
- 연구 과제 종료 이후에도 자체적인 연구 수행을 통해 연구 성과물들을 지속적으로 도출할 수 있을 것으로 예상됨

This study aims to develop an image-guided transcranial direct current stimulation (tDCS) system, which can focally stimulate specific brain areas based on the individual anatomical images, thereby making a great contribution to the popularization of a tDCS system.

This study aims to develop an image-guided transcranial direct current stimulation (tDCS) system, which can focally stimulate specific brain areas based on the individual anatomical images, thereby making a great contribution to the popularization of a tDCS system.
Result
- We accomplished all the goals that we had planned in the research proposal submitted in 2010. We implemented a world-first image-guided (array-type) tDCS system, and tested its performance through a series of experiments such as skull phantom experiments, animal experiments, and human test experiments.
- Moreover, as planned in the research proposal, we developed a technology to analyze tDCS stimulating currents quantitatively, which was then applied to various research topics including investigation of safety issue in the use of extracephalic reference electrode and a study on the applicability of tDCS to patients who underwent craniectomy surgery. Recently, we released a Matlab toolbox named Comets, which can be used for the realistic simulation of local electric field generated due to tDCS.
- We monitored changes in cerebral blood flow of a rat brain during tDCS, for the first time, and investigated the correlation between the tDCS outcome (enhancement of working memory performance) and individual anatomical characteristics.
Expected Contribution
- The proposed 'image-guided' tDCS technology can solve the problems of the conventional tDCS system, such as wide stimulation area, inefficiency due to lack of anatomical information, and so on.
- Currently, we are continuing developing advanced version of Comets (Matlab toolbox), which will be commercialized soon.
- We are still making lots of new results using the developed tDCS system, which will be soon published in world-renowned international journals.
- The present study will activate multidisciplinary research among brain science, biomedical engineering, and rehabilitation engineering.
