본 과제는 슈퍼박테리아 감염증 치료를 위한 신개념 항생제 CG400549의 개발에 관한 과제임 (전임상에서 임상 1상까지), 항생제의 빈번한 사용으로 인한 내성균반현으로 기존의 치료제로 치료가 어려운 박테리아가 많이 발현됨(MRSA, VRSA, VISA, MSSA 등등), CG400549는 이러한 내성 박테리아를 박멸하고자 기존의 항생제와 작용기전이 전혀 다르고 박테리아생존에 필수적인 지방산(fatty acid)합성에 관여하는 효소인 Fabl(enoyl-[acyl-carrier-protein]reductase)억제제임. 본 과제는 슈퍼박테리아 감염증 치료를 위한 신개념 항생제 CG400549의 개발에 관한 과제임 (전임상에서 임상 1상까지), 항생제의 빈번한 사용으로 인한 내성균반현으로 기존의 치료제로 치료가 어려운 박테리아가 많이 발현됨(MRSA, VRSA, VISA, MSSA 등등), CG400549는 이러한 내성 박테리아를 박멸하고자 기존의 항생제와 작용기전이 전혀 다르고 박테리아생존에 필수적인 지방산(fatty acid)합성에 관여하는 효소인 Fabl(enoyl-[acyl-carrier-protein]reductase)억제제임. CG400549는 전임상(GLP Tox.)을 유럽(스위스)에서 성공리 마치고, 유럽(네덜랜드)에서 임상 1상(SAD, single ascending dose)를 성공적으로 완료하였고, 임상1상 후기단계인 MAD(multiple ascending dose)를 유럽(네덜랜드)에서 추진중이며, 올해5월 28일 사람에게 투약될 예정임.
전임상연구중에 최대 28일 반복독성을 설치류(랫드)와 비설치유(개)에서 실시하여 NOEL(개)은 400mg/kg 와 NOAES(랫드)는 100mg/kg을 얻음. 임상 1상(SAD)은 80mg/tkfka - 1920 mg/사람까지 투약을 하여 사람에서 약동학 parameter를 얻었고 부작용을 관찰함.

CG400549 is an antibacterial drug candidate that inhibits Fabl (enoyl-[acyl-carrier-protein(ACP) reductase), an essential enzyme in the fatty acid synthesis. Since fatty acids are an ingredient of bacterial cell wall, its synthesis is critical for the survial of bacteria, A Fabl inhibitor could possess antibacterial activity against those pathogens in which Fabl is the sole enoyl-ACP reductase, e.g. Staphylococcus aureus, Haem ophilus influenza, Moraxella catarrhalls, and Escherichia coli, The potent activity of CG400549 inhibiting Fabl purified from S. aureus suggests a potential antibacterial activity against Staphylococcus spp. Both in vitro and in vivo studies have shown that CG400549 has antimicrobial activity against a variety of clinical isolates of staphylococci. CG400549 has selectivity for S. aureus including vancomycin-resistant S. aureus (VRSA), vancomycin-intermediate S. aureus (VISA), methicillin-resistant S. aureus(MRSA), and methicillin-susceptible S. aureus (MSSA).
The safety studies were designed to comply with global requirements for phase I human safety studies and therefore included 28-day repeat dose safety in rats and beagle dogs. Repeat oral dose studies were performed in rats and dogs to support investigational new drug (IND) submissions for Phase I trials in human. Based on the results of these studles, the NOEL(no-observed-effect-level) was set at 400 mg/kg in dogs and the NOAEL(no-observed-adverse-effect-level) was set at 100 mg/kg in rats. Moreover, CG400549 can be administered orally, and showed good safety profile in the (Phase IIND-enabling toxicity studies including genotoxicity, safety pharmacology, and single dose and repeat dose toxicity studies. CG400549 was subiected to the a first in man study in Netherlands and got the approval from Independent Ethics Committee in Netherlands. The clinical study was a first-in-man study with CG400549(oral doses: 80, 160, 320, 640, 1280, 1980 mg/person) and studied safety, tolerability and PK of single ascending doses of CG400549 in healthy male subjects. In addition, the effect of food on the PK of CG400549 following a singel oral administration(640mg/person)of CG400549 in healthy male subjects was evaluated. Treatment with single oral doses of CG40059 at the dose range studied was found to be safe and well tolerated by a groud of healthy male subiects. Only a limited number of TEAEs were reported but none of these TEAEs were considered by the Medical Investigator to be related to the study medication, In addition, there were no findings of clinical relevance with respect to clinical laboratory, vital signs, ECG, or physical examination, In the SAD part of this study, exposure to CG400549(Cmax, AUC)Increased with dose in a less than dose-proportional manner. The drugcandidate CG400549 is planed to MAD(multiple ascending dose) and a clinical phase II study as well soon.
