본 연구에서는 허혈성 혈류장애질환 치료에 사용되는 한약재에 대한 항허혈 기전, 약리활성, 면역계 등에 미치는 효과를 최신 연구기법을 동원하여 새로운 시각에서 효과를 검증하여 한방의 과학회를 이룩하는 계기를 만들고자 하였다. 본 연구과제에서는 많은 한약재 중 청피를 선정하여 이것이 실험동물의 적출 평활근(기관지, 자궁, 동맥혈관)의 수축력, 복강 대식세포의 식작용, 뇌허혈 실험동물의 뇌혈류역학(뇌연막동맥 직경과 국소뇌혈류) 변동 및 뇌경색 형성 등에 미치는 효과를 관찰하였다. 이를 위하여 적출 평활근의 수축력은 isometric transducer에 연결된 physiograph를 사용하였고, 대식세포의 식작용은 luminometer를 사용하여 chemiluminescence를 측정하였고, 뇌혈류역학 변동은 단선조 나일론 봉합사를 사용한 중대뇌동맥 폐쇄법으로 만든 일과서 뇌허혈 동물모델의 두개창을 통한 videomicroscopy 및 laser-Doppler flowmetry로 측정하였고, 뇌경색의 크기는 형태계측학적으로 측정하였으며, 뇌부종 생성 정도는 형태계측학 및 뇌내 수분 함량측정법으로 시행하였다. 그 결과는 다음과 같다. 1) 청피는 기관지, 자궁 및 동맥혈관의 수축력을 이완시켰다. 2) 평활근의 이완작용에 미치는 청피의 기전으로는 교감신경계 α1수용체와 관련이 있다. 3) 청피(H2O) 분획은 신동맥 평활근 이완작용과 관련이 있다. 4) 청피는 복강 macrophages외 phagocytic activity를 활성화시켰다. 5) 청피는 국소뇌혈류량을 증가시켰고, 뇌연막동맥의 직경을 확장시켰다. 6) 뇌혈류역학에 미치는 청피의 기전은 nitric oxide 합성효소의 활성과 관련이 있다. 7) 청피는 특히 청피(MC) 분획과 청피(hexane) 분획은 농도-의존적으로 뇌연막동맥을 확장시키고 국소뇌혈류를 증가시킴으로써 뇌허혈 동물모델의 뇌혈류역학을 개선시켰다. 8) 청피(MC) 분획과 청피(hexane) 분획의 뇌혈류역학 개선 효과는 NOS와 cyclooxygenase의 활성도 증가와 관련이 있었다. 9) 청피(MC) 분획과 청피(hexane) 분획은 뇌경색의 크기를 용량-의존적으로 감소시켰다. 10) 청피(MC) 분획과 청피(hexane) 분획은 뇌부종의 발생을 용량-의존적으로 억제시켰다. 이상의 결과를 종합해보면 청피는 주로 nitric oxide 합성요소의 활성을 통한 뇌연막동맥 확장 및 국소뇌혈류 증가에 의하여 허혈성 뇌혈유역학을 개선시킴으로써 항허혈 효과를 가지고 있는 것으로 사료되며, 향후 유효성분의 개발과 함께 새로운 뇌허혈 치료제로서의 사용이 가능할 것으로 기대된다.

This study was designed to investigate the pharmacological effects of herb drug (Citri Reticulatae Viride Pericarpium, CRVP) on the isolated smooth muscle, immune system, and cerebral ischemia using modem techniques, and further to provide the possibility of scientification of· oriental medicine. Isolated smooth muscle(trachea, uterus, and arteries) contractility was measured by physiography, phagocytic activ~ty of peritoneal macrophage by luminometry, cerebral hemodynamics(pial arterial diameter and regional cerebral blood flow) by videomicroscopy and laser-Doppler flowmetry, and the cerebral infarct size and brain edema by morphometry and brain water content determination, respectively. The results are as follows: 1) CRVP reduced the contractility of trachea, uterus, and arterial smooth muscle. 2) The smooth muscle relaxation was associated with adrenergic α1-receptor. 3) The water fraction of CRVP had an action of renal arterial relaxation. 4) CRVP increased a phagocytic activity of peritoneal macrophages. 5) CRVP increased regional cerebral blood flow (rCBF) and pial arterial diameter. 6) The effect of CRVP on the cerebral hemodynamics is associated with the activation of nitric oxide synthase (NOS). 7) Both the methylene chloride fraction and the hexane fraction of CRVP dose-dependently improved the altered cerebral hemodynamics of cerebral ischemic animal by increasing rCBF and pial arterial diameter. 8) Improvement of cerebral hemodynamics of cerebral ischemia was associated with activation of NOS and cyclooxygenase. 9) The cerebral infarct size was dose-dependently reduced by the methylene chloride fraction as well as the hexane fraction of CRVP. 10) Brain edema was decreased by the methylene chloride fraction as well as the hexane fraction of CRVP in a dose-dependent manner. Based on these results, it is suggested that CRVP has an antiischemic effect through the improvement of ischemic cerebral hemodynamics by increasing the NOS activity, and that in future further isolation and development of main effective constituent in CRVP can provide a novel therapeutic strategy for cerebral ischemia.
