방사성 활막절제술은 유럽등지에서 50여년간 시행되어 왔으며 외과적 수술의 단점을 보완하여 수술이 어려운 부분까지 완전히 치유가 가능하며 혈전 및 감염의 위험이 없고 국소마취로 시술이 가능하며 회복시간이 짧고 경제적인 점등 많은 장점이 있다. 효과적인 방사성 활막절제술의 시술을 위하여는 적절한 β- energy를 갖고 활막세포에 고르게 분포하지만 관절외로 누출이 되지않는 물성이 유지되어야 하며 이러한 조건을 갖는 방사성의약품의 개발이 관건이다. 지금까지 여러 가지 핵종을 대상으로 다양한 형태의 방사성 의약품이 개발되고 연구되었으나 아직 완전한 조건을 갖춘 방사성의약품이 개발되지 못한 실정이다. 166Ho과 Chitosan의 착화합물로 만들어진 DW-166HC(밀리칸주)는 동물실험등을 통하여 활막내의 고른 분포와 체류성이 확인되었으며 166Ho의 방사화학적인 특성이 방사성활막절제술에 적합하므로 이 약물의 임상적 유용성을 확인하기 위하여 류마티스 관절염환자를 대상으로 임상시험을 진행하여 새로운 관절염치료용 방사성의약품으로 개발코자 연구를 진행하였다. 연대 세브란스병원에서 1상 및 초기 2상 임상연구를 수행하였는데 관절강내에 10mCi, 15mCi 및 20mCi의 3단계 투여용량으로 각 용량별 5명의 환자를 대상으로 관절강내에 1회 주사하여 12주간 (필요시 14주) follow - up을 통해 각 환자의 유효성과 부작용유무를 평가하였다. 평가내용으로는 WHO의 이상반응 평가기준을 이용하여 안전성을 평가하였고 시간대별 혈액채취 및 혈액중 방사성농도 측정을 통하여 체내 분포여부를 확인하였고 치료반응도에 대한 평가(global assessment)를 1차 유효성 변수, 통증(visual analogue scale), 운동범위(range of motion) 및 종창(joint swelling)을 2차 유효성 변수로 하여 유효성을 평가하였다. 그 결과 유효성 평가 가능한 피험자 15명중 12명(80%)의 피험자가 good 또는 fair로 판정되어 높은 유효율을 나타내었으며, 단계별로 보면 1단계가 60%, 2단계가 100%, 3단계 80%로 2단계에서 유효율이 가장 높았다. 투여 12주후의 통증에 대한 통증(visual analogue scale) 값을 볼 때, 2명을 제외한 13명의 피험자에서 투여전에 비하여 감소되었는데 그중 6명은 50%이상 현저하게 감소하였다. 운동범위(range of motion)의 경우는 투여전에 비하여 크게 변화하지 않았고 무릎 둘레는 투여전에 비하여 12주째의 측정치가 11명의 피험자에서 감소하여 종창에 대한 효과가 있음이 예측되었다. 안전성의 경우 본 임상시험중 이상반응으로 인하여 시험을 중단한 경우는 없었으며 중증의 부작용 역시 한건도 보고되지 않았다. 보고된 부작용은 발열, 국소동통등 이었으나 대부분 경미한 일시적인 부작용으로 별다른 처치없이 곧 회복되었다. 각 용량단계에서 3명에 대하여 혈액과 뇨중의 방사능을 측정하였는데 혈중 농도는 시험약 투여전의 baseline과 비교하여 거의 변화가 없었으며 뇨중 배설되는 방사능의 누적량도 1단계가 0.031%, 2단계가 0.009%, 3단계가 0.004%로 매우 낮았다. gamma-scan결과 모든 피험자에서 무릎으로 잘 집적되었고, 다른 기관으로의 이행은 거의 나타나지 않았다. (기관당 3% 미만) 이러한 결과로부터 DW- 166HC의 류마티스관절염 치료제로의 개발 가능성을 확인할 수 있었고 후기 2상 임상시험에서는 다수의 환자를 대상으로 15mCi의 투여용량으로 안전성과 유효성의 평가를 하고자 한다. 이 과제의 초기 목표는 3차년도까지 후기 2상 임상시험을 완료하고 3상 임상시험에 진입하는 것이었으나 후기 2상임상시험의 임상연구계획서를 수립하는 과정에서 본 연구팀은 이 약물이 방사성의약품이고 기존의 대조가 될 만한 약물이 없기 때문에 후기 2상 임상시험도 다기관에서 single, open test로 계획하여 임상연구자들과 협의를 거쳐 연구계획서를 작성하고 1상 전기2상 임상시험보고서와 같이 식약청에 제출하여 협의하였으나 식약청에서는 약물의 특성은 이해하나 후기 2상 임상시험에서는 double blind test를 수행하여야 한다고 보완을 요구하여 후기 2상의 임상계획서를 완전히 다시 작성하게 되어 현재 임상계획서를 승인받아 연세대 신촌세브란스병원과 삼성서울병원에서 후기 2상 임상시험을 착수하였으나 초기 목표에 비해 진행이 많이 지연되었다.

Rheumatoid arthritis is a chronic inflammatory disorder of joint s that is ch ar ect erized by the inflammation and proliferation of synovial tissues and often leads to the destruction and deformity of the involved joints. Radiation synovectomy is the ablation of inflamed synovium by intra-articular injection of a beta-emitting radionuclide in colloidal or particulate form. Since the first trial of radiation synovectomy in 1952, this procedure has been used for almost 50 years and is currently employed to varying degrees in Europe, Australia, Canada and other areas except the United States. An ideal radiopharmaceutical for radiation synovectomy should have the following characteristics ; (1) beta-emission of the radionuclide should have an appropriate energy level for the effective removal of inflamed synovium, (2) the radionuclide should be spread evenly through the inflamed synovium but not cause any significant extra-articular leakage, (3) the physical half-life of the radionuclide should be short enough to reduce the risk of prolon ged radiation in vivo, but long enough to endure adequate manufacturing and transport to hospital distant from the manufacturing site, (4) the radiopharmaceutical should be biocompatible and biodegr adable. Holmium-Chitosan complex (DW-166HC) is a good radiopharmaceutical for radiation synovectomy based on the above characteristics. Previous studies have demon strated that DW-166HC possesses certain physical and biological properties suitable for use in radiation synovectomy, such as an appropriate half-life and minimal extra-articular leakage. So, we conducted clinical studies for developing DW-166HC as a tharapeutic agent for rheumatoid arthritis. First of all, clinical phase I/IIa study was performed in Yonsei University College of Medicine to investigate the safety and the ther apeutic effect of DW-166HC for the treatment of rheumatoid arthritis. It was designed in three steps according to the administration dose, and a total of 16 patients [1 man, 15 women ; median age 49(range 36-65) years] who had RA knee synovitis refractory to disease-modifying anti-rheumatic drug treatments of > 3 months' duration were randomly as signed to three treatment groups : 10mCi(n =6), 15mCi(n =5), 20mCi(n =5). In each treatment group, blood and urine radioactivity were analysed by beta-counter and biodistribution of the inject ed DW-166HC was evaluated using a gamma scan camera. Clinical assessment was done according to three variables (evaluation method ) : knee joint pain (visual analogue scale), range of motion (goniometry) and joint swelling (circumference of knee joint). The duration of follow-up observation was 3 months. According to the clinical assessment criteria used, at 12 weeks after the intra-articular injection of DW-166HC, 3 patients (60%) in first step (10 mCi), 5 patients (100%) in second step (15 mCi), 4 patients (80%) in third step were determined to have a good response. Following the intra-articular injection of DW-166HC, the blood radioactivity was little changed from the baseline measurement and the accumulated radioactivity excreted in urine was minimal. Gamma scan study indicated that most of the injected radiochemical was localized within the injected joint cavity, and the extra-articular leakage was negligible at 24h after the injection : brain 0.3%, lung 0.6%, abdomen 0.7% and pelvis 0.8%. Major adverse events were transient post-injection knee joint pain and swelling. From these clinical phase I/IIa study, we can suggest that DW-166HC might be a safe and effective agent for radiation synovectomy. And now we finished the protocol design of clinical phase IIb study and this study started in Yonsei University, college of Medicinal and Samsung Medical Center.
