The purpose of this project was to identify the pathogenetic mechanisms underlying cardiovascular complications and to develop new methods in the treatment and prevention of them in dialysis patients. This project consists of cross-sectional and experimental clinical studies in dialysis patients with cardiovascular complication. We divided the main project into three small projects and achieved the following results. Project 1: Analysis of cardiovascular risk factors in Korean patients with end-stage renal disease 1 )Association of plasma fibrinogen concentration with vascular access failure in hemodialysis patients. High plasma fibrinogen level is an independent risk factor for vascular access failure in hemodialysis patients. [Published in Nephrology Dialysis Transplantation 14(1): 137-141, 1999] 2) Hyperfibrinogenemia is an independent risk factor for atherosclerotic renal artery stenosis By multivariate logistic regresslOn analysis, older age, the presence of hypertension and a higher fibrinogen level were significant risk factors of renal artery stenosis. These results suggest that hyperfibrinogenemia as well as old age and hypertension are independent risk factors for atherosclerotic renal artery stenosis. Koreans have mainly high-molecular weight apo(a) phenotypes and serum Lp(a) is elevated in CAPD patients compared to HD patients for common apo(a) phenotypes, which may contribute to the frequent cardiovascular mortality in CAPO patients. Lipid peroxidation was increased in CAPO patients and this was -not due to weakening of antioxidant defenses of the plasma. Increased T AC was mainly caused by higher level of uric acid. Reduced, rather than increased, MDA in hypoalbuminemic patients and lack of the effect of albumin infusion on the levels of MDA suggest that the frequent occurrence of cardiovascular disease in dialysis patients with hypoalbuminemia is not due to the decrease in antioxidant capacity resulting from low serum albumin.
