3. 여성결혼이민자들의 거주지별, 출신국에 따라 다음과 같이 서로 다른양상의 주요 신체적, 정신적 건강문제가 도출되었다. 농촌에 비해 도시 여성결혼이민자들이 끼니를 거르는 경우(49.8%)가 더 많았으며, 영양위험군 비율(35.2%)도 더 높은 것으로 나타났다. 반면, 농촌 여성결혼이민자들은 당뇨, 빈혈 유병율이 더 높았으며, B형간염, C형간염, 풍진 등에 더 많이 이환되어 있는 등 도시 여성결혼이민자들에 비해 질병 위험에 더 많이 노출되어 있었다. 또한 여성결혼이민자의 출신국에 따라 다른 양상의 건강행위와 건강문제가 나타났다. 베트남 여성은 다른 출신국 여성에 비해 수축기와 이완기 평균 혈압(105.56/63.80mmHg)이 가장 낮은 반면, 영양위험군 비율(37.3%)이 가장 높았으며, 저체중 비율(18.4%)도 가장 높았다. 또한 암검진율, 풍진항체 양성율이 낮은 것으로 나타나, 건강수준은 물론 향후 임신과 출산에서 자녀 건강이 악화될 가능성이 높은 집단이라고 볼 수 있다. 그러나 이들의 주관적 건강에 대해서는 ‘매우 건강하다’, ‘건강하다’라고 다른 출신국 여성에 비해 긍정적으로 응답한 비율(61.2%)이 높았고, 주관적 행복감 점수도 가장 높았으며, 우울증상 비율은 가장 낮았다. 이들은 신체적 건강 수준은 좋지 않은 상태이지만, 정신적 건강상태는 양호한 편이었다. 필리핀 여성은 한국에 와서 체중이 증가하였다고 응답한 비율(66.7%)이 가장 많았다. 이들은 다른 출신국 여성에 비해 비만으로 분류된 비율 (22.2%)이 가장 높았고, 수축기와 이완기 평균 혈압(111.79/69.52mmHg)도 가장 높았다. 비만은 당뇨, 고혈압 등 만성질환의 주요 위험요인이므로 필리핀 여성에게는 만성질환 예방을 위한 관리가 제공되어야 할 것이다. 중국 여성은 주관적 행복감 점수(4.75점)가 가장 낮았으며, 우울 증상을 보이는 경우가 58.6%로 다른 출신국 여성에 비해 정신적 건강상태가 가장 좋지 않은 것으로 나타났다.
4. 여성결혼이민자의 의료보장율은 89.5%로 나타났다. 이들은 아플 때 주로 병의원(78.4%)을 가장 많이 이용하며, 도시지역에서는 외국인 노동자 무료진료소, 한의원, 약국을 월등하게 많이 이용하며, 농촌지역에서는 보건(지)소와 보건진료소를 더 많이 이용하는 것으로 나타났다. 한국 체류기간이 1년 미만인 경우에 의료기관 이용율이 저조하였으며, 체류기간이 길어질수록 병의원, 보건(지)소와 보건진료소 등의 이용율이 뚜렷하게 증가하였다.
지난 1년간 아파서 병원에 가고 싶어도 가지 못한 병원 미치료 경험율이 29.7%로 나타났으며, 여성결혼이민자들이 병의원을 이용하지 못하는 이유로 ‘말이 통하지 않아서’ (27.4%), ‘병원비가 비싸서’ (27.4%), ‘혼자 가기 힘들어서’ (26.6%) 라고 가장 많이 응답하였다. 보건소는 도시(44.7%) 보다는 농촌(61.6%)의 여성결혼이민자들이 더 많이 이용하는 것으로 나타났으며, 보건소를 이용하게 된 계기는 가족이 알려줘서가 57.3%로 가장 많았으며, 다음이 결혼이민여성지원센터 17.9%,보건소 직원의 가정방문 11.0%의 순으로 나타났다. 보건소를 이용하지 않는 이유로 ‘보건소가 있는지 몰라서’라고 도시와 농촌 모두 가장 많이 응답하였다. 보건소에서 제공하고 있는 보건의료서비스에 대해 알고 있는지 인지도를 조사한 결과, 임부와 영유아 건강관리와 관련된 보건의료서비스에 대한 인지도가 높았으며, 제공되고 있는 보건소의 각종 보건의료서비스를 알고 있는 여성 중 해당 보건의료서비스를 이용한 경험이 있는지를 보면, 인지도가 높았던 보건의료서비스 순으로 많이 이용하였다.

The incidence of transnational marriage in Korea has been increasing since the middle of the 1990s. There were 144,385 marital immigrant women in Korea in May, 2008. Most of the research on marital immigrant women has explored their life and living conditions such as the demographic characteristics, marriage process, language problems, acculturation, marriage conflict, marriage adaptation, support from spouse, and child care from a sociologicalor social welfare point of view. However, few studies have focused on the health and health problems of marital immigrant women. The purpose of this research was to investigate marital immigrant women's health concept, their subjective and objective health status, access to health care system, and positive and negative factors affecting their utilization of the health care system. A total of 533 participants(224 in urban areas and 309 in rural areas) are recruited by convenience sampling to compare the characteristics of marital immigrant women according to where they lived. A structured questionnaire and medical examination were used. Focus group interviews with 6 Vietnamese women and 7 Chinese women, and a face-to-face private interview with 5 Filipino women were conducted to examine their health concepts. The criteria for sample selection were marital immigrant women from China, Japan, the Philippines, Vietnam, and other Asian countries who had been in Korea for less than 10 years after marrying Korean men. The results showed that marital immigrant women had a health concept in which health is the status of physical and mental well-being without disease. From the sample, 25.6% and 40.5% showed symptoms of anemia and depression, respectively. There were different physical and mental health problems according to where they lived and where they were from. Marital immigrant women who lived in urban areas were more likely to skip meals (49.8%) and be in the nutritional risk group (35.2%) than those who lived in rural areas, whereas the latter showed higher prevalence rates of diabetes and anemia and greater morbidity of hepatitis B, hepatitis C and rubella than the former. The health behaviors and health problems varied by nationality. Vietnamese women showed the lowest mean systolic and diastolic blood pressure but the highest rate of nutritional risk group and lowest body weight. They also showed the lowest rate of cancer examination and positive antibody for rubella. However, the Vietnamese women showed more positive cognition about their subjective health status, the highest score of subjective happiness, and the lowest rate of depression symptoms. Filipino women showed the highest rate of obesity, and mean systolic and diastolic blood pressure among marital immigrant women. Chinese women showed the lowest score of subjective happiness and more depression symptoms. Therefore, different health management and health education is required according to the nationality of the marital immigrant women. The rate of medical service coverage was 89.5%, and the most frequently used institutions were clinics or hospitals (78.4%). In the urban areas, free clinics for foreign worker, oriental medical clinics or pharmacies were more commonly used, while in the rural areas, public health centers or primary health care posts were more frequently used. As for the barriers in accessing the health care system, communication problems (27.4%), cost (27.4%), and difficulty to go to the clinic alone (26.6%) were pointed out. As for the awareness of the availability of health care service, programs for pregnant women, infants, and children were highly recognized among health care services provided by public health centers. This research can be applied to the design of health policy, development of health programs for marital immigrant women, and raising of social support and concern for the health issues of marital immigrant women.
