본 연구결과에서는 대학과 지역사회를 연계한 ‘건강한 대학캠퍼스(Healthy Campus)’ 모형을 개발하기 위해 기초정보 수집 및 분석과정을 수행하여 건강증진사업을 위한 필요 정보를 추출하였고, 이를 기초로 하여 대학 건강증진의 추진 방향을 설정한 후, 건강증진대학을 추진하기 위한 체계적인 수행방안을 모색하였다. 구체적인 연구결과는 다음과 같다.

본 연구결과에서는 대학과 지역사회를 연계한 ‘건강한 대학캠퍼스(Healthy Campus)’ 모형을 개발하기 위해 기초정보 수집 및 분석과정을 수행하여 건강증진사업을 위한 필요 정보를 추출하였고, 이를 기초로 하여 대학 건강증진의 추진 방향을 설정한 후, 건강증진대학을 추진하기 위한 체계적인 수행방안을 모색하였다. 구체적인 연구결과는 다음과 같다.
1. 고등교육법, 학교보건법, 국민건강증진법, 학교건강검사규칙, 한국대학교육협의회 규정 등을 검토하여 교육기관에서 수행할 수 있는 관련법령 및 추진 방향, 대학생 및 교직원 건강관련 사항, 대학보건실의 기능 및 역할 등을 파악하였다. 또한 주요 대학의 건강증진사업 수행실태를 살펴보기 위해 국내 대학과 미국, 일본 대학의 수행사례를 분석하였다. 현재까지 대학 본부가 주도하는 대학 건강증진사업은 드물었고, 대학보건실이 중심이 되어 일차보건의료 서비스를 주된 업무로 제공하고 있었다. 반면 해외 사례에서는 제공되는 서비스의 내용은 유사하였으나, 이를 제공하기 위한 관련법령 및 조직, 네트워크, 모니터링 방식이 우리나라 대학보다 앞서 있음을 알 수 있었다.
2. 8개 권역(특별시, 광역시, 경기지역)에서 수행된 대학생 건강조사의 주요 결과는 다음과 같다. 최근 1년간 경험한 주요 건강 문제로는 스트레스가 51%로 가장 많았고, 최근 2년간의 건강검진 수검률은 21%이었고, 예방접종 경험률은 29.6%이었다. 자신의 건강수준을 ‘건강하다’고 응답한 학생이 57.1%이었고, 최근 1년간의 진료 및 입원경험, 정신문제 상담 경험률은 각각 33.6%, 4.7%, 2.6%로 나타났다. 또한 50%의 학생이 일상생활에서 스트레스를 받는 것으로 응답하였고, 2.9%의 학생이 자살시도를 했던 경험이 있었다. 83.7%의 대학생이 대학캠퍼스가 안전하다고 응답하였고, 4%의 학생이 최근 질환 및 사고, 중독을 경험하였고, 대학생 중 20.4%가 현재 흡연자인 것으로 나타났고, 이 중 남학생의 흡연율이 35.3%, 여학생의 흡연율이 6.2%이었다. 최근 1년간 술을 마신 경험이 있는 학생이 95.1%이었고, 고위험률이 16.6%이었으며, 문제음주율이 37.3%, 알코올남용률이 9.3%, 알코올의존률이 11.5%이었다. 대학생 중 28.1%가 아침식사를 먹지 않았고, 1일 평균 과일, 채소의 섭취 횟수는 1-2회가 55%로 가장 많았고, 43.1%가 기능성 건강식품의 복용하고 있었다. 식품구매 시 영양표시를 읽는 학생은 32.1%에 불과하였다. 대학생의 비만도를 살펴보면 저체중군이 20.7%로 매우 높았고, 과체중군은 7.4%이었다. 주 5일 이상 걷기운동을 30분 이상 실천한 비율은 25.7%이었고, 주 2일 이상 근력운동을 실천한 비율은 26.7%이었다. 30분이상의 중등도 신체활동을 주 5일 이상 실천한 비율은 5.3%이었고, 20분 이상의 격렬한 운동을 주 3일 이상 실천한 비율은 15.6%이었다. 29.3%의 학생이 대학보건실을 이용한 경험이 있었고, 제공받은 경험이 있는 건강증진 서비스는 예방접종이 12.4%로 가장 많았고, 다음이 정기건강진단, 응급처치, 운동 프로그램이었다. 또한 대학생들이 건강정보를 습득할 때, 가장 적극적으로 활용하는 정보출처는 인터넷(70%)이었고, 다음이 대중매체, 가족 • 친지 • 친구, 의료인과의 상담, 수업시간, 학보 • 서적 • 잡지 • 리플렛, 건강강좌, 학교 게시판 및 학교 홈페이지 순이었다.
3. 대학보건 전문가집단이 현재 수행 중인 대학 건강증진사업의 문제점으로 대학 당국의 관심 부족, 대학 건강증진 프로그램을 지원할 수 있는 정책 및 자원 부족, 대학 건강증진사업의 중요성에 관한 이해부족, 대학 건강증진사업에 관한 전략적 기획과정의 부재, 대학 건강증진 프로그램을 기획 • 관리 • 평가할 수 있는 전문 인력의 부족, 교육행정조직 공무원들의 대학 교육과 건강의 중요성에 관한 인식 부족 등을 심각한 문제로 지적하였다. 대학구성원의 참여방안으로 정책 및 정부지원, 학교본부의 추진력, 건강생활독려, 학업연계, 인센티브 제공, 취업연계, 자율적 참여 유도, 정보제공, 건강조사와 관련된 수행전략을 제안하였다.
4. 기초정보수집단계에서 분석내용을 기초로 하여 대학 건강증진의 추진체계 및 필수사업 요소를 추출하였다. 추진방향은 대학구성원의 건강증진 도모, 사전적 예방관리, 안전사고 예방이며, 추진전략으로는 대학구성원의 건강인식 제고 및 예방수칙 확산, 대학건강정책 수립 및 캠퍼스중심의 사업모형 개발, 건강증진 서비스 가이드라인 및 사업지침 활용, 지역사회 자원 및 관련조직과의 연계방안 구축을 설정하였다. ‘건강한 대학캠퍼스(Healthy Campus)’ 구축을 위한 기본영역으로는 예산 확보(대학의 지원금, 외부 기금 등), 정부 및 학내 대학건강정책의 수립 및 지원, 대학구성원(학생, 교직원 등)의 참여, 인력 및 자원(전문 인력, 시설, 장비 등), 건강증진 프로그램과 활동, 건강한 대학캠퍼스 운영체계(수행원칙, 가이드라인), 건강한 생활터(setting)로서의 접근성과 지속가능성, 건강증진환경 조성, 평가 및 피드백, 관련조직과의 협력 및 지역사회 자원과의 파트너십을 설정하였다.
5. 지역사회 자원과의 연계체계를 구축하기 위한 방안으로 건강과 관련된 지역사회 중장기계획과 대학의 발전방안 공유, 건강증진환경 조성을 위한 가용자원의 활용 및 새로운 자원 개발, 건강기술 제공을 위한 다양한 채널 및 수행방안 개발, 구성원의 참여 유도를 통한 지역사회 건강분위기 조성, 자원의 연계적 활용과 전문 인력의 자문, 체계적인 운영방안의 모색을 위한 협력, 예산 확보 및 기금 조성, 평가 지표의 개발 및 피드백 방안의 지속성 확보를 위한 노력, 건강결과의 편익 및 건강정보 공유, 대학과 지역사회가 함께 발전하는 win-win 방안 모색을 제안하였다.

This study reported the results of explored the essential element and effective strategies to develop the standardized program on healthy campus in university health. Major results were as follows.
1. In relevant regulatory, we reviewed the aim, the subjects, the administrative, the health service recommendation, and the official requirement of health care center in college or university. We reviewed the relevant academic and policy-related literature undertaken to clarify theory, scope practice and key contextual issues. Health care center was main agency to provide university members the primary health services on campus. Health promoting school model has adopted the framework and measurement index of WHO.

Individual in-depth interviews indicated that the large proportion of students had stress(51%), cold/flu/strep throat(49%), drinking (47.7%), allergic rhinitis(22.9%), sleeping disorder(20.6%), conflict with friend and family(19.6%) and anemia(17.1%). 21% of the students had received the health screening in the last 2 years, and 29.6% of them had completed vaccination in the last 1 year. More than three-quarters(83.7%) of the students reported a safety campus of their universities. A few students(4%) had experienced a accident or poisoning on their campus in the last 1 year. In the university students, the exercising to strengthen muscle(26.7%) was the highest, the next were the walking rate(25.7%), the vigorous exercise(15.6%), the moderate exercise(5.3%) in order that. 46.7% of the students had never performed any exercise. 20.4% of them was current smoker, and binge drinker was 16.6%. While Overweight(≧25) group was 7.4%, under weight(<18.5) group was 20.7%, 29.3% of the student had visited health care center on their campus. The large proportion of students had received health services from health care center on vaccination(12.4%), health screening, first aid, exercise program. Most students got the health information from internet(70%), and the next informants were mass media, friend/family/relatives, counseling with medical personnel, health lecture, newsletter/leaflet/magazine, bulletin broad and homepage of their university.

According to opinions of the advisory group, the obstacles to perform the university health were lacking of interest in university, no policies on university health, inadequate system, incomplete process of health planning, absent health-related personnel. Key challenges highlighted at the university health included establishment of healthy policy, progressive performance, encourage to improve life style, connection with health and academic achievement, providing incentives, supportive job placement, autonomous participation, informed choice and health survey.
4. Essential programs to build the healthy campus were secure a budget or rising fund, establishment of healthy policy, health promotion program, personnel and resources, participation of university members, systematic process, healthy environment, evaluation and feedback, collaboration with health-related sectors and community partnership. Although there were no strong views, effective strategies were providing health promotion services, environment improvement, planning of healthy campus, building of health program infra, enhancement of disease prevention, safety network of university and support to primary health care. The health service of university health center brings together medical care, disease prevention, health promotion, and occupational and public health services. Components are high quality services and healthy workplace for university members, healthy campus policy and health promotion services, deductible health plan, health assessment, and collaboration with community resources. To build a healthy university campus, we suggest six areas of health services (medical care, health education, counseling, immunization, heath promotion, and Employee Assistance Program). In addition, we suggest that universities develop effective strategies to improve health on campus, such as community partnerships.
5. The health service operates these in partnership with students, campus departments, and community health care organizations. University health services and programs enable students, faculty, and staff to make the most of their university experience by minimizing the impact of illness, emotional distress, and injury in university and workplace. The standardized guideline for healthy university included: first, the process of review and composition to prepare the implementation, second, establishment of essential components, such as healthy university policy, health organization, operating system, rising fund, main actions and community partnership, finally selection of measurement for structure, process, outcome evaluations.
