1. 사회경제적 지위에 따른 간접흡연 실태 및 간접흡연 관련 요인
- 대다수의 국가단위 보건자료나 선행연구에서는 간접흡연을 주류 담배연기와 부류 담배연기로 나누어 정의하고 있음.

- 간접흡연은 사회경제적 지위뿐만 아니라 인구사회학적 특성, 가정내 특성, 학교 및 친구 특성, 국가 금연 정책과 중요한 연관성이 있는 것으로 나타남.
2. 간접흡연 관련 국내․외 정책 및 실무지침
- 세계보건기구,미국, 호주, 캐나다, 스코틀랜드에서는 청소년기 간접흡연 노출 감소를 위하여 가정과 차 안에서의 흡연 감소 정책, 공공주택의 흡연 규제 정책, 학교에서의 금연교육 및 상담프로그램 운영, 캠페인 등을 시행하고 있음.
- 우리나라에서는 법률 및 정책의 제정, 금연구역 지정, 학교와 가정에서의 예방프로그램, 대국민 인식개선 캠페인 등이 시행되고 있으나, 우리나라의 정책 및 실무지침은 간접흡연 노출 관리보다는 금연에 초점을 맞추고 있음. 또한, 사회경제적 지위에 따른 청소년기 간접흡연 건강형평성 제고를 위한 체계적인 정책 및 실무지침의 개발이 필요함.
- 대다수의 간접흡연 노출 감소를 위한 중재효과를 살펴본 연구의 대상은 청소년보다는 아동임. 국외에서 청소년 또는 아동 대상 간접흡연 노출 감소를 위한 주된 중재방법은 상담과 교육이었음. 8건의 국외 연구 중 4건에서 중재효과가 유의한 것으로 나타났음. 국내에서는 3건의 간접흡연 중재 연구가 시행되었으며, 청소년을 대상으로한 간접흡연 노출 감소를 위한 중재는 없었음. 모든 국내 연구는 유아를 대상으로 간접흡연 예방프로그램의 효과를 검증하였으며, 중재효과가 있는 것으로 보고됨.
3. 우리나라 청소년 표본을 이용한 사회경제적 지위와 간접흡연 노출과의 연관성 분석 결과
- 2012년 조사된 제8차 청소년건강행태온라인조사 자료를 2차 분석함. 전체 표본, 흡연경험 청소년, 흡연을 경험하지 않은 청소년의 3개집단 각각을 대상으로 영향요인인 성별, 학교급, 부모와 동거여부, 도시규모, 흡연행태, 주관적 건강수준, 스트레스의 효과를 통제한 후 사회경제적 지위와 간접흡연 노출 위험성과의 연관성 분석을 시행함. 그 결과, 3개 모두의 표본에서 부모의 학력과 주관적으로 인식한 가구 경제수준이 낮을수록 대상자의 간접흡연 노출 위험성이 높아지는 것으로 나타남.
4. 사회경제적 지위에 따른 청소년기 간접흡연 건강형평성 제고를 위한 개선방안
- 간접흡연에 관한 정보수집 대상을 청소년뿐만 아니라 영유아와 학령기 아동까지 확대
- 금연 필요성을 인식을 개선을 통하여 가정과 차 안에서의 금연 유도
- 청소년기 간접흡연 노출 실태에 대한 이해 증진을 위한 체계적인 감시체계 구축
- 청소년기 간접흡연 노출 수준의 심각성, 간접흡연 노출이 건강에 미치는 해로움, 사회경제적 수준이 낮은 인구집단에서 간접흡연 노출위험성이 높음에 대한 국민 인식개선을 위한 적극적 노력
- 학교에서 청소년 간접흡연 노출 감소 및 예방을 목적으로 하는 교육프로그램의 체계적 운영

1. The current situation surrounding the impact of socioeconomic status on exposure to secondhand smoke and causes of secondhand smoke exposure.
- The majority of preceding studies and national health data differentiate secondhand smoke into two categories: mainstream smoke and sidestream smoke.
- The measure of exposure to secondhand smoke is largely divided into self-report measures, biological markers, and exposure assessment of airborne substances. Survey questionnaires are used for self-report measures. Biological markers include analysis of urine and hair for cotinine, analysis of NNK in urine, and inspection of DNA adducts. Exposure assessment of airborne substances involve testing for carbon monoxide, respirable particulates including PM2.5, and nicotine.
- Adolescents' and children's level of exposure to secondhand smoke in the United States, Canada and England is quite alarming. According to data released by the World Health Organization and the United States Centers for Disease Control and Prevention, exposure to secondhand smoke produces physical and mental health problems. Homes and cars are the main locations where adolescents and children are exposed to secondhand smoke. In comparison to foreign countries, ongoing systematic collection of information about exposure to secondhand smoke among Korean adolescents is not being implemented.

Studies from other countries report that there is a significant relationship between an increase in the risk of being exposed to secondhand smoke and the following: having parents with a low educational level, having low-income parents, having parents who are manual laborers, living in a small house, living in a high density residential area, not owning a home, having a small amount of furniture, living in a country with low economic development, living in a poor neighborhood, attending schools with low funding, etc.
- There is a significant relationship not only between socioeconomic status and exposure to secondhand smoke but also between secondhand smoke exposure and the following: demographic factor, school, friend, and smokefree policy.
2. Policies and guidelines regarding secondhand smoke exposure in the Republic of Korea and other countries.
- The WHO, the U.S., Canada, and Scotland are campaigning for and implementing policies aimed at reducing smoking at home and in the car, policies that regulate smoking in public housing, and programs and counseling for smoking cessation in schools.
- In the Republic of Korea, there are enforcement of laws and policies, designation of non-smoking areas, prevention programs in schools and at home, campaigns for raising the awareness of thegeneral public, etc. However, the Republic of Korea's policies and guidelines are focused more on smoking cessation than limiting exposure to secondhand smoke. Furthermore, there is a need to develop policies and guidelines for a systematic improvement of adolescent health equity with regards to socioeconomic effects on exposure to secondhand smoke.
- Most research looking into the effects of interventions aimed at reducing exposure to secondhand smoke targeted children rather than adolescents. In other countries, the main method of intervention for reducing exposure to secondhand smoke among children and adolescents was counseling and education. Four out of eight studies conducted abroad showed that the effects of intervention were noteworthy. In the Republic of Korea, three intervention studies were conducted but there were no interventions aimed at reducing adolescents' exposure to secondhand smoke. All research conducted in the Republic of Korea assessed the effects of prevention programs concerning children's exposure to secondhand smoke, and reports demonstrate the effectiveness of interventions.
3. Analysis of the relationship between socioeconomic status and the level of exposure to secondhand smoke through secondary analysis of a representative of a sample taken from the Korean adolescent population.
- A secondary analysis was conducted on the data gathered from a 2012 survey, the 8th Korea Youth Risk Behavior Web-based Survey. After controlling for factors such as the effects of sex, school levels, living with parents, the size of a city, smoking behaviors, subjective reports of health status, and stress, analysis of the relationship between socioeconomic status and the risk of exposure to secondhand smoking was conducted on three distinct groups: the complete sample, adolescent smokers, and adolescent non-smokers. Findings from all three groups showed that adolescents with parents that have a lower educational level and a lower subjective report of household income status were at a higher risk of exposure to secondhand smoke.

- Expansion of data collection on secondhand smoke exposure that not only targets adolescents but also encompasses infants and school-age children.

- Active engagement in improving national recognition of the increased risk of exposure to secondhand smoke among populations from lower socioeconomic status, the negative health effects of exposure to secondhand smoke, and the serious levels of adolescents' exposure to secondhand smoke.
- Systematic implementation of education programs in schools that aim to prevent and reduce adolescents' exposure to secondhand smoke.
- Through strengthening tobacco cessation policies, encourage the reduction of smoking rates and establish and implement specialized tobacco cessation policies specifically targeted towards populations from lower socioeconomic status.
