본 연구의 최종 목표는 알츠하이머 치매(AD) 조기진단을 위한 뇌기능 자기공영영상(fMRI) 기술을 개발하고, 이 기술을 정상노인(CN), 경도인지기능장애(MCI) 환자 및 알츠하이머 치매(AD) 환자에,치매의 조기진단 및 치매 치료효과 평가를 위하여 임상적용을 하며, 마지막으로 Beta Amyloid 단백질 독성으로 인한 활성산소발생 억제제 개발 및 알츠하이머 치매(AD) 동물모델을 이용한 자기공영영상 기술 효과 확인을 위한 기초연구를 진행하는 것이었음.

본 연구의 최종 목표는 알츠하이머 치매(AD) 조기진단을 위한 뇌기능 자기공영영상(fMRI) 기술을 개발하고, 이 기술을 정상노인(CN), 경도인지기능장애(MCI) 환자 및 알츠하이머 치매(AD) 환자에,치매의 조기진단 및 치매 치료효과 평가를 위하여 임상적용을 하며, 마지막으로 Beta Amyloid 단백질 독성으로 인한 활성산소발생 억제제 개발 및 알츠하이머 치매(AD) 동물모델을 이용한 자기공영영상 기술 효과 확인을 위한 기초연구를 진행하는 것이었음.
기술개발 연구로는 뇌세포에서 나오는 신호를 얻을 수 있는 BOLD 뇌 기능 영상외에 뇌 관류를 이용한 뇌기능영상 및 양성자 농도 변화를 추적하여 얻는 뇌기능영상 및 뇌 대사물질 변화를 관찰하는 뇌기능 대사물질 영상을 개발하였음. 이들 개발된 기술을 CN, MCI, AD 환자에 적용하여 3년간 추적하는 뇌기능 영상을 획득하였으며, 각 군에서의 뇌기능의 년차별 변화와, 각 년차별 피험자 군 간의 뇌 영상 변화 비교 및 CN에서 MCI로 변화는 경우와 MCI에서 AD로 변화하는 것을 평가하였고, AD 환자 군에서 약물반응이 있는 경우와 없는 경우를 나누어 평가하였음. 새로운 fMRI 기술을 피험자 모집한 피험자 군에게 적용하여 군간 차이를 발견하였고 또한 유전자 검사와 혈액검사를 통하여 군간 특정 단백질에서 차이를 확인하였음. 피험자군은 세군(CN,MCI,AD) 3군에 대하여 모집하였고, 정상군 30명, 경도인지장애군 17명, AD 12명에 대하여 1,2,3차 MRI 영상을 촬영 및 혈액/유전자 검사를 실시하였음. 정상노인에서 MCI 환자로, MCI 환자에서 AD 환자로 전환되는 것을 예측하는 영상법으로 사용 가능으로 보임. 또한 알츠하이머의 병인인 Beta Amyloid 단백질 독성이 활성산소라는 것을 세포 수준에서 증명하였으며, Beta Amyloid 단백질 독성이 세포 신호전달에 주는 영향 분석 확인하였음.
새로운 fMRI 기술들을 임상 적용 결과 AD 환자를 조기에 발견 할 수 있는 imaging biomarker로 이용 가능할 것으로 보임.현재 개개의 피험자에서의 모든 자료는 분석완료 하였고 각 군 간에 비교하는 것도 완료되었음. 이들 결과를 바탕으로 하는 연구논문 작성이 연구종료 이후에도 계속될 것임.

The objectives of this research were: 1) to develop functional MR techniques (fMRI) for early detection of Alzheimer's disease (AD), 2) to apply the fMRI techniques for subjects with AD and mild cognitive impairment (MCI), and cognitive normal(CN), and 3) to develop an inhibitor of free oxygen radicals caused by beta amyloid proteins.
Ⅱ. The Contents and the Range of the Study
1. Development of new fMRI technology to detect AD in its early stage: We will develop the following fMRI techniques to evaluate brain function in AD, MCI, and CN subjects. BOLD and CBF-based, metabolite-based, proton-density-based, and diffusion-based fMRI techniques will be developed to image cellular-level brain functions. Finally, a blood brain barrier (BBB) mapping MR technique will be developed to apply it to the patients.
2. Clinical application of the developed fMRI technique to AD and MCI patients: The new fMRI technologies will be applied into CN, MCI, and AD subjects to evaluate the treatment effect of the AD patients.
3. Developing an inhibitor of free oxygen radicals produced by poisonous beta amyloid proteins:
a) Performed in vivo, in vitro study of free oxygen radical productions by beta amyloid proteins
b) We traced the cyclophilin mechanisms and developed a restrainer of the AD progression.
Ⅲ. Results
1. BOLD and CBF-based fMRI, proton-density, diffusion, metabolite-based fMR techniques were developed to investigate cellular-level function in the brain. Furthermore, a dynamic ASL method was developed to obtain CBF and permeability in the brain.
2. Subjects was recruited to scan subjects longitudinally. All data were analysed as the individual and group comparisons to evaluate the treatment effect and converts.
3. We proved that the poisonous beta amyloid proteins produce free oxygen radicals.
4. We published 19 full papers and applied for 3 patents from our results.
Ⅳ. Future applications
1. Suggestions to the developed fMRI techniques: The new developed fMRI techniques can be useful to apply to patients with various diseases and the early detection of AD.
2. The future usage of our clinical outcomes: We may apply our techniques to MCI patients progressing to AD.Furthermore, we can utilize the chemical reactivity of AD patients to cholinesterase inhibitors.
3. The future usage of the free oxygen radical inhibitor development: We can further evaluate the influence of beta amyloid to the cell signalling process. Finally, our technique can be applied to other diseases caused by protein folding.
