(1) 한국형 어린이 노출계수 항목

Ⅳ. 연구 결과
(1) 한국형 어린이 노출계수 항목
국내 환경유해인자의 어린이 노출 및 위해성 평가를 위한 노출계수 항목은 국내·외 문헌고찰과 전문가 자문을 통하여 다음과 같이 조사되었다. 매체를 중심으로 실내환경의 시간활동양상(time indoors and activity factors), 호흡률 및 체중(inhalation rate and body weight), 주택특성(building characteristics), 소비자제품(consumer products) / 토양 및 먼지의 비식용 소화(non-dietary ingestion), 토양과 먼지의 소화(soil and dust ingestion), 체중 및 표면적(body weight and body surface area), 토양 부착물(soil adherence) / 목욕시간(time showering or bathing), 체중 및 표면적(body weight and body surface area), 호흡률(inhalation rate), 모유섭취(human milk intake), 음용수 섭취(ingestion of water and other select liquids) / 식품의 과일과 야채 섭취(intake of fruits and vegetables), 곡류 섭취(intake of grain products), 집에서 만든 음식물(intake of home produced foods), 모유 섭취(human milk intake), 고기류와 지방 섭취(intake of meats, diary products and fats), 생선과 조개류 섭취(intake of fish and shellfish), 총 음식물 섭취(total food intake) 이었다.
(2) 어린이 시간활동 양상
개발된 설문지와 시간활동 양상 기록지를 통해 전국적으로 표본 추출된 10세 이하 1,049명 (남자 어린이 541명, 여자 어린이 508명)의 시간활동 양상을 여름(519명)과 가을(530명)에 측정하였다. 평일과 주말을 종합할 때 주택실내에서 보낸 시간은 약 18.03±3.67 hr(75.12%) 이었으며, 학교 또는 유치원/어린이집 실내에서 보낸 시간은 약 2.56±3.31 hr(10.67%)로 하루 중약 92.20%를 실내환경에서 보내고 있었다. 반면 실외 및 이동에서 보낸 시간은 1일 중 각각 4.42%와 3.38%를 나타내었다. 평일에 참여 어린이들이 주택실내에서 보낸 시간은 16.78±3.22 hr, 학교 또는 어린이집 실내에서 보낸 시간은 4.97±3.06 hr, 기타실내 0.86±1.52 hr을 나타내었다. 1일 중 실내환경(주택실내, 학교 또는 어린이집, 기타실내)에서 보낸 시간은 대략 94.22% 이었다. 한편 주말에 참여자가 주택실내에서 보낸 시간은 19.28±3.68 hr, 학교 또는 어린이집 0.14±0.94 hr, 기타실내 2.22±2.99 hr을 나타내었다. 1일 중 실내환경(주택실내, 학교 또는 어린이집, 기타실내)에서 보낸 시간은 대략 90.18% 이었다. 평일은 주말에 비해 주택실내에서 보낸 시간은 평균 2.50 hr 낮았다.
(3) 시간활동 양상과 설문지 건강지표
설문지를 통한 질병력인 만성 호흡기 질환, 알레르기성 비염, 아토피성 피부 질환, 음식 알레르기가 있는 어린이가 주택실내에서 보내는 시간이 유의하게 적었으며, 그리고 초등학교 또는 어린이집(유치원)에서 생활하는 시간이 유의하게 많았다(p=0.000). 이것은 어린이의 시간활동 양상에서 초등학교 또는 어린이집(유치원)에 머무는 시간이 많을수록 관련 질환의 유병률이 높을 가능성을 나타낸다.
(4) 시간활동 양상을 이용한 어린이 주요 행위
참여 어린이 대상자의 주요 행위를 평일과 주말 모두 합하여 분석하였다. 10세 이하의 어린이의 가장 많은 행위는 수면(43.41%) 이었으며, 놀이, 수업, 가족과의 식사 순 이었다. 1순위에서 10순위(수면-학교 외 스스로의 학습)까지가 1일 중 87.87%를 나타내었다. 이것은 어린이의 주요 행위 중 1순위-10순위에서 유해인자에 노출될 수 있음을 나타낸다.
(5) 참여 어린이의 호흡률
어린이 참여자 총 68명에게 실측한 호흡률에서 만 5세-만 10세 남자 32명의 안정시 평균 호흡률은 0.48±0.10 m3/hr 이었으며, 여자는 0.41±0.06 m3/hr 이었다. 남자 어린이 호흡률에서 Step1 및 Step2의 호흡률은 각각 평균 0.80±0.14 m3/hr 및 0.93±0.17 m3/hr 이었다. 반면 여자 어린이들은 남자 어린이에 비해 유의하게 낮은 값을 나타내었다(p<0.001). 안정시, Step1, Step2를 평균한 값은 남자 어린이와 여자 어린이에서 각각 0.74±0.23 m3/hr와 0.65±0.22 m3/hr 이었으며, 유의한 차이를 나타내었다(p<0.05). 한편 Step1과 Step2 때는 안정시에 비해 남자 및 여자 어린이들 모두에서 호흡률이 유의하게 증가하였다. Arcus-Arth와 Blaisdell(2007)이 보고한 미국 결과와 비교하였을 때(호흡률 단위를 m3/day로 변환), 본 연구 참여자의 호흡률은 상대적으로 적었으며 미국 어린이들에 비해 대략 80% 정도를 나타내었다.
호흡률을 체중으로 나누어 표현(m3/hr-kg)할 때는 기존 보고된 결과와 같이 연령이 증가함에 따라 유의하게 감소하는 경향을 나타내었다. 그리고 안정시, Step1, Step2 모두 연령이 증가함에 따라 유의하게 감소하는 경향을 나타내었다. 안정시, Step1, Step2에서 연령이 증가함에 따라 체중으로 보정한 호흡률은 유의하게 감소하는 경향을 나타내었다. 참여자를 군집으로 구분하여 호흡률(m3/day-kg)을 분석하였다. 참여자 중 만 5세-만 6세(어린이집 군집)와 만 7세-만 10세(초등학생)으로 구분할 때, 종합적으로는 어린이집 군집보다 초등학생 군집의 호흡률이 높음을 나타내었다. 그리고 미국 EPA(2006)의 결과와 비교할 때 상대적으로 호흡률이 낮았다.
참여자(남녀 모두)의 체중을 고려할 경우, 심박동수와 호흡률의 관계에서는 유의한 양(+)의 상관관계를 나타내었다. 안정시와 Step1에서 심박동수와 호흡률(m3/hr-kg)의 회귀식은 각각 Y=0.002X-0.003(p=0.000)와 Y=0.002X+0.008(p=0.008) 이었으며, Step2에서는 Y=0.001X+0.021(p=0.073) 이었다. 이것은 연령 만 5세-만 10세에서는 심박동수에 따른 호흡률(체중 보정)의 증가를 나타낸 것이며, 그리고 심박동수와 체중을 이용하여 호흡률을 추정할 수 있음을 나타내었다.

Exposures among children may be expected to be different from exposures among adults because of physiological and behavioral differences. Children may be more exposed to some environmental pollutants because they consume more of certain foods and water per unit of body weight and have higher ratio of body surface area to volume than adults. And rapid changes in behavior and physiology may lead to differences in exposure as children grow up. In terms of risk, children may also differ from adults in their vulnerability to environmental pollutants because of toxicolodynamics differences.
Exposure factors for children have been recommended in the following areas; ingestion of water and other select liquids, non-dietary ingestion factors, ingestion of soil and dust, inhalation rate, dermal exposure factors, body weight, intake of fruits and vegetables, intake of fish and shellfish, intake of meat, dairy products and fats, intake of grain products, intake of home-produced foods, total food intake, human milk intake, activity factors, and consumer products.
This study of child-specific exposure factors has been prepared to focus on various factors used in assessing exposure specially for children ages 0 to 10 years old, and provides nonchemical-specific data on exposure factors for the inhalation rates and time-activity factors. The purpose of the child-specific exposure factors focusing on inhalation rate and time-activity pattern is to provide exposure factors for children. The major results of this study may be used by exposure and risk assessors and other interested parties as a source for data needed to estimate childhood exposure to toxic contaminants.
Activities or times spent in microenvironments may vary among children on the basis of culture, hobbies, location, gender, age, socioeconomic characteristics, and personal preferences. Meanwhile, limited information is available regarding time-activity pattern in children, Korea. Time-activity pattern data for 1,049 subjects in Korea were collected by 24 diaries with two-day weekday and weekend. The diary used a modular tool, which combined standardized questions and a time-activity diary. The questions included demographic, socioeconomic, and family information. The time-activity diaries were recorded by parents in place of children. Time-activity pattern information was collected in 30min intervals. The activities were organized according to the following classifications: main activity, transportation, indoors, and outdoors. Transportation included walking as well as riding in a vehicle in this study such as commuting and moving other places. Activities were later coded by a technician according to 9 major classifications, 50 intermediate classifications, and 144 detailed classifications. The diary classifications included residential indoor environment, outdoor, and transportation. The residential indoor time in weekend was greater than that in weekday, whereas transportation time in weekend was less than that in weekday. The mean times for staying at home for the weekday and weekend day were 16.78 and 19.28 h, respectively.
Ambient and indoor air are potential sources of children’s exposure to toxic substances. Children can be exposed to air pollutants during a variety of activities in different environments. Children may also inhale chemicals from indoor use of various consumer products and may be exposed due to sources that contribute pollution to ambient air. Due to their size, physiology, and activity level, the inhalation rates of children differ from those of adults. Infants and children have a higher resting metabolic rate and oxygen consumption rate per unit of body weight than adults, because of their rapid growth and relatively larger lung surface area per unit of body weight that required cooling.
A total of 68 children were recruited from 5 to 10 years, and inhalation rates were 3 kinds of activity levels; resting, light activity (walking), heavy activity (rapid walking. The hourly average inhalation rates for short-term exposures for male and female children were 0.48±0.10 m3/hr and 0.41±0.06 m3/hr in resting, respectively. Comparing to other reports, inhalation rates of this study were lower than those of US and European countries. Adjusting the body weight, the inhalation rate (m3/hr-kg) was significantly decreased as age increased.
Aspects of exposure factors variability have to be considered. This research attempts to characterize the variability of inhalation rate and time-activity pattern among child-specific exposure factors. Variability refers to true heterogeneity or diversity in children. Differences among children are referred to as inter-individual variability. This report was first offered to the public in 2014. Since this time, the National Institute of Environmental Research will continuously update the child-specific exposure factors. Therefore the further research will be needed.
