본 연구는 충북지역 3대 호발암에 대한 분자역학 및 실험적 연구로서, 위암과 폐암의 발생 및 예후에 영향을 미치는 유전적, 환경적 요인 및 이들의 상호작용을 규명하고, 위암 및 폐암 발생에 대한 예측 모델을 개발하며, 전이성 대장암 동물 모델에서 epithelial-mesenchymal transition (EMT)를 억제하는 기법을 적용하여 전이를 예방하고 예후를 향상시키는 새로운 치료법을 확립하고자 수행하였다.

본 연구는 충북지역 3대 호발암에 대한 분자역학 및 실험적 연구로서, 위암과 폐암의 발생 및 예후에 영향을 미치는 유전적, 환경적 요인 및 이들의 상호작용을 규명하고, 위암 및 폐암 발생에 대한 예측 모델을 개발하며, 전이성 대장암 동물 모델에서 epithelial-mesenchymal transition (EMT)를 억제하는 기법을 적용하여 전이를 예방하고 예후를 향상시키는 새로운 치료법을 확립하고자 수행하였다.
위암과 폐암에 대한 분자역학연구는 환자-대조군 연구 디자인으로 시행하였으며, 위암은 1400쌍, 폐암 966쌍의 환자-대조군 연령과 성별에 대해 짝지어 모집하였으며, 위암과 폐암의 발생 및 예후에 영향을 미치는 환경적 요인(식이, 생활습관, PAH 노출, H.pylori 감염, 아플라톡신 섭취)과 유전적 요인(SNPs) 및 외유전적 요인을 평가하였다. 폐암의 경우 지역 코호트를 이용하여 코호트내 환자-대조군연구를 시행하였다. 최종적으로 본 연구에서 규명된 위험 및 보호요인들을 이용하여 발생 및 예후를 예측하는 통계적 모델을 개발하였다. 대장암에 대한 실험적 연구는 대장암으로 외과적 수술을 시행한 환자들의 대장암과 간전이 조직에서 대장암의 전이 및 예후에 영향을 미치는 EMT와 CAFs 관련 인자 발굴하였으며, 대장암 세포에서 EMT를 조절하는 CAFs의 역할 규명 및 제어 기법 개발과 전이성 대장암 동물 모델에서 EMT, CAFs 조절을 통한 암 치료방법 개발하였다.
위암 및 폐암의 분자역학연구 결과를 토대로 최종 도출된 위암 및 폐암 발생 예측 모델은 각각 73%, 76%의 예측력을 가지는 것으로 확인되었으며, 위암 예측 모델에는 나이, 식이섭취(두류, 감자류, 양파류, 견과류, 붉은 고기류, 난류, 생선류, 김치류), 음주, 흡연, H.pylori infection, 유전자 다형성(ITGA1 rs2447867, ITGA/PELO rs1862610, PRKAA1 rs1454268, PSCA rs2294008)을 포함되었고, 폐암 예측 모델에는 나이, 누적흡연량, 음주, 광산직업력, 과거력(폐렴, 결핵), 유전자 다형성(ATR rs2229032, ERCC rs3810366)이 최종 포함되었다.
사람의 대장암 조직에서 EMT와 Cancer Associated Fibroblasts (CAFs)에 관한 단백을 면역화학염색 정도를 양적으로 평가하고 임상요소와 연관성을 분석하여 EMT와 CAFs는 불량한 예후와 연관성을 보임을 확인하였으며, 대장암 세포주와 Mesenchymal stem cell (MSC) co-culture 혹은 MSC 유래 conditioned media를 처리하면 EMT 연관 단백 및 mRNA 발현의 증가(western blotting, quantitative real time RT-PCR, confocal immuno histochemical staining), invasion 및 migration의 증가한 결과를 보여 미세환경에 의해 대장암의 EMT가 조절됨을 확인하였다. 대장암 세포주와 MSC (Mesenchymal stem cell) co-culture 혹은 MSC 유래 conditioned media와 동시에 PDGFR 억제제인 imatinib을 처리하면 EMT가 억제되었다. 전이성 대장암 모델을 확립하고 대장암세포주와 MSC를 혼합하여 주입하면 종양의 악성도와 전이가 촉진되었고 조직 분석 결과 EMT가 촉진되었음. PDGF 억제제인 imatinib을 처리하면 종양의 크기와 전이가 감소하였고 조직 분석 결과 EMT가 억제되었다.
본 연구의 결과는 위암과 폐암의 발생 기전 이해, 고위험군 선별, 예방 및 치료 지침 마련에 중요한 기초자료로 활용 될 것으로 기대되며, 전이성 대장암에 대한 표적 치료제 개발에 이용되어 치료 및 환자의 생존률을 높일 수 있을 것으로 판단된다.

The purposes of this study were to elucidate the genetic and environmental factors, and gene-environmental interactions influencing the incidence and prognosis of gastric cancer and lung cancer, to develop a predictive model of gastric and lung cancer risk, and to develop the methods to control epithelial-mesenchymal transition(EMT) and applied that strategy at metastatic colon cancer mouse model to reduce metastasis and to improve prognosis.
Molecular epidemiology study for gastric and lung cancer was carried out in the case-control study design. After an age- and sex matching, 1400 cases-controls pair for gastric cancer, 966 pairs for lung cancer were recruited. Environmental factors (food intake, lifestyle factors, PAH exposure, H.pylori infection, aflatoxin ingestion, etc.), genetic factors (SNPs), and epigenetic factor were evaluated for whether they associate with gastric cancer and lung cancer incidence and prognosis. For the lung cancer, using a regional cohort, nested case-control study were conducted. Finally, a statistical prediction models were developed to predict the incidence and prognosis of gastric cancer or lung cancer, using the risk and protective factors that have been determined in this study. Based on the results of this molecular epidemiologic study for gastric cancer and lung cancer, ultimately derived prediction model for gastric cancer and lung cancer (ROCAUC: 73% for gastric cancer, 76% for lung cancer). Age, dietary intake (beans, potatoes, alliums, nuts, red meat, egg, fish, kimchi), drinking, smoking, H.pylori infection, and genetic polymorphism (ITGA1 rs2447867, ITGA / PELO rs1862610 , PRKAA1 rs1454268, PSCA rs2294008) were included in gastric cancer prediction model. Age, the cumulative smoking amount, alcohol drinking, occupational history (mine), past diseases history (pneumonia and tuberculosis), and genetic polymorphisms (ATR rs2229032 and ERCC rs3810366) were final included lung cancer prediction model. We proved that EMT and Cancer Associated Fibroblasts (CAFs) were related poor prognosis in colon cancers by analysis of EMR or CAFs related protein expression and clinical parameters in human colorectal cancer surgical specimens. Colon cancer cells revealed EMT like features with EMT-related protein / mRNA expression (western blotting, quantitative real time RT-PCR, confocal immuno histochemical staining), increased invasion and migration by co-culture of Mesenchymal stem cell (MSC) or MSC derived conditioned media. These data demonstrated that EMT of colon cancers is controlled with tumor microenvironment including CAFs. Treatment with imatinib, tyrosine kinase inhibitor, inhibited EMT of colon cancer cells induced with co-culture of MSC (Mesenchymal stem cell) or MSC derived conditioned media. We demonstrated that implantation both colon cancer cells and MSC reveled metastasis and tumor volume with EMT enhancement in metastatic colin cancer model. Treatment of imatinib let reduce metastasis and tumor size via EMT inhibition.
The results of this study to be utilized as a valuable basic data for understanding the pathogenesis of stomach, lung, and colon cancer, screening the high-risk group, and for the development of targeted therapeutics against metastatic colon cancer.
