□ 연구의 목적 및 내용
염증성 장질환은 위장관을 침범하는 원인 불명의 만성 난치성 염증성 질환임. 염증성장질 환 환자의 대부분이 사회 생활과 경제활동의 주역인 20-40대에 발생 일평생 악화와 호전을 반복하는데, 국내 설문 조사에 따르면 전체 환자의 64%는 조퇴/결근/휴직에 따른 스 트레스 및 압박감을 받고 있으며, 47%는 업무성과/성적이 떨어진다고 답했으며, 37%는 자살 충동을 느낀다고 답하였음. 따라서, 염증성 장질환은 개인적 문제를 넘어 국가, 사회 적인 문제로 다뤄져야 함. 하지만 염증성장질환은 아직 병태 생리가
□ 연구의 목적 및 내용

연구자들은 본 과제 연구를 통하여 (1) 염증성장질환 환자에서 가장 흔히 발견되는 돌연 변이인 NOD2가 발현하는 Paneth cell에서 함께 발현되는 CD1d의 역할을 평가함; (2) CD1d와 NOD2의 상호작용을 분석함 (두 molecule이 모두 장내 미생물유래 antigen을 염 증세포에 전달하는 역할을 하기 때문에 장내 미생물 변화를 평가함); (3) Paneth cell의 최근의 역할은 장줄기세포를 지지하여 장줄기세포의 분화를 지지하는 역할을 주로 하기 때문에 장crypt를 isolation하여 organoid를 만들어 염증성장질환 마우스 모델에서의 특징 을 살펴 보고 Paneth 기능을 연구함; (4) 미국의 유수 연구 기관을 파견을 통해서 선진 연구 기법을 확보하고 공동 연구를 진행함; (5) 본 연구의 타켓 질환인 염증성 장질환과 소장 질환에 대한 임상 자료를 수집하여 분석하고 연구함.
□ 연구결과
1. CD1d-mediated NKT cell의 immune regulation 기능에 대한 연구는 잘 알려져 있지 않아 많은 연구가 필요한 분야임. 본 과제연구에서 7DW8-5의 항염증 효과를 확인하였으며 향후 새로운 강력한 glycolipid adjuvant 연구 파급에 도움이 될 것으로 사료됨.
2. 염증성장질환은 genetic susceptibility가 존재하는 환자에서 환경적 요인이 trigger하여 발생하는 것으로 생각됨. 하지만, 실제 염증성장질환 발생과 가장 밀접하다고 알려진 NOD2조차도 아직 어떻게 유전자의 돌연변이가 장관 염증 발생을 유도하는지 명확하지 않은 상태임. 본 연구는 NOD2 기능이 저하된 상태에서 CD1d 기능이 장관 염증을 조절하는 능력이 있음을 확인하였음. NOD2와 CD1d는 모두 Paneth cell에서 발현되는 면역 연관 물질로 본 과제에서 확인한 NOD2와 CD1d의 cross-talk외에도 Paneth cell에서 발현하는 다른 pattern recognition receptor와의 상호 작용 혹은 다른 Paneth cell에서 특이적으로 발현하는 물질과의 연관성에 대한 연구로 파급 될 것으로 생각됨
3. Samp1/YitFC 마우스에서 isolation된 crypt의 regeneration 능력이 감소되는 것을 확인하였으며 이는 염증성장질환의 병인이 stem cell dysfunction으로 인한 장점막 regeneration 장애로 인한 가능성을 규명하기 연구로 파급될 것으로 생각됨.
4. 미국 UCLA에 파견을 통해서 최근 장점막연구의 트랜드인 장줄기세포와 3D organoid 모델의 최신 지식을 습득하고 Intestinal subepithelial myofibroblasts-derived factors for intestinal regeneration after radiation과 Biomechanical cues mediate gut adaptation by modulating Wnt/βß-catenin and Hippo signaling in murine, porcine and human models과 같은 공동 연구를 수행함
5. 본 연구의 타겟 질환인 염증성 장질환과 소장 질환에 대한 임상 자료를 수집하여분석하고 연구함. 염증성장질환 환자에서 발생하는 결핵 감염, 소장질환으로 소장에 발생하는 메켈 게실과 소장 종양의 진단법 연구, PPI와 같은 약제 사용시 장내미생물총 변화에 대한 연구를 진행하고 논문을 발표함.
□ 연구결과의 활용계획
1. CD1d-mediated NKT cell의 활성을 유도하는 glycolipid로 많이 사용되는 α-GalCer보다 NKT cell 활성시키는 능력이 강력한 7DW8-5의 항염증 효과를 확인함. 현재 α-GalCer의 실제 임상 적용이 시도 되고 있는데 7DW8-5는 더 효과적인 약제로 사용될 수 있을 것임.
2. 염증성장질환은 사회, 경제적을 중요한 질환인데 완치가 어려운 이유는 아직 그 발병 기전을 완전히 이해하지 못하기 때문임. NOD2는 염증성 장질환에서 가장 중요한 돌연 변이로 아직 어떻게 NOD2의 기능 이상이 장관 염증 발생을 유도하는지 명확하지 않은 상태임. 본 연구는 NOD2 기능 이사에 CD1d가 기여함을 확인 하였으며, 이 유전자 발현은 장점막세포 중 Paneth cell에서 주로 발현 하는 것을 확인하였고 장내 미생물 변화와 연관됨을 확인하였음. 향후 NOD2는 돌연 변이를 동반한 염증성 장질환 환자에서 CD1d 활성 조절을 통한 치료법 개발에 사용될 수 있을 것으로 생각됨

Inflammatory bowel disease (IBD) is a chronic refractory inflammatory disease of unknown etiology involving the gastrointestinal tract. The majority of patients with IBD developed in their 20s to 40s who are the mainstay of social life and economic activities. The surveys for IBD patients show that 64% of all patients experience stress and pressure due to absence of workplace or school/ 47% said poor work performance / and 37% said they felt suicidal attempt. Therefore, IBD should be managed as a national and social issue beyond individual problems. However, IBD are not yet well understood, and therefore, there is no treatment method for cure.
This research project aimed to (1) evaluate the role of CD1d, which is expressed inpaneth cells expressing NOD2, the most common mutation in IBD patients; (2) analyze the interaction between CD1d and NOD2 (assessing intestinal microbial changes because both molecules are responsible for transferring antigens of intestinal microbiota to inflammatory cells). (3) isolated intestinal crypts to make 3D organoids (recently, the role of Paneth cell is considered to support the renewal and differentiation of intestinal stem cells (ISC)). (4) obtain advanced research techniques and conducting joint research through dispatching leading research institutes in the United States. (5) perform the clinical study for IBD and small bowel disease, the target diseases of this project.
□ Result
1. The immune regulation function of CD1d-mediated NKT cells are not well known and need to evaluated in detail. We confirmed the anti-inflammatory effect of 7DW8-5, which is expected to contribute to the development of new potential glycolipid adjuvant studies in the future. 2. IBD is thought to be triggered by environmental factors in patients with genetic susceptibility. However, even NOD2, which is known to be most closely related to the IBD, is still unclear as to how the mutation of the gene induces the development of intestinal inflammation. This study confirmed the ability of CD1d to regulate intestinal inflammation in the condition of decreased NOD2 function and the cross-talk of NOD2 and CD1d was identified. Because both NOD2 and CD1d are immune-related substances expressed in Paneth cells, our results can induce the interaction with other pattern recognition receptors expressed in paneth cells or substances specifically expressed in Paneth cells. 3. We revealed the decreased regeneration ability of crypt isolated from Samp1/YitFC mice. This result suggest IBD can be caused by stem cell dysfunction and results mucosal damage. 4. The latest knowledge of stem cell and 3D organoid models from UCLA was obtained. In addition, we performed the research with the UCLA, including “Intestinal subepithelial myofibroblast-derived factors for intestinal regeneration after radiation” and “biomechanical cues mediate gut adaptation by modulating Wnt / β ß-catenin And Hippo signaling in murine, porcine and human models.” 5. Clinical studies for IBD and small bowel diseases, the target disease of this project, showed that the increased risk of tuberculosis in patients with IBD, optimal diagnostic modality of Meckel 's diverticulum and small bowel tumor, and the change of intestinal microbiota caused by proton pump inhibitors.
□ Expected Contribution
1. We confirmed the anti-inflammatory effect of 7DW8-5, which has the ability to activate NKT cells rather than α-GalCer, which is often used as a glycolipid to induce the activity of CD1d-mediated NKT cells. Currently, practical clinical application of α-GalCer is being tried, and 7DW8-5 could be used as a more effective agent. 2. IBD is non-curable disease because of unclear pathogenesis. NOD2 is the most important mutation in IBD, yet it is unclear how NOD2 dysfunction induces intestinal inflammation. We confirmed that CD1d contributes to the NOD2 function, which was mainly expressed in Paneth cell. In the future, glycolipid adjuvant to augment CD1d function may be used as a treatment modalities in IBD patients with NOD2 mutation. 3. The regeneration ability of isolated crypt in Samp1 / YitFC mouse is decreased, and it is thought that intestinal stem cell transplantation can be used as a theoretical basis for treatment of IBD. 4. We will conduct research on intestinal stem cells and organoids using advanced research techniques obtained from UCLA. In addition, joint research should be continued. 5. Clinical studies on IBD and small bowel diseases, which are the target diseases of this study, suggested that recognition of increased risk of tuberculosis in IBD patients treated with biologics, optimal diagnosis method of small bowel diseases. We believed that this result will improve the quality of patient care.
