목적 및 방법: 노인당뇨병의 대사이상과 미토콘드리아 기능이상의 원인과 역할을 규명하여, 노인당뇨병 표적인자를 발굴하고자, 노인코호트와 골관절수술 시행 노인당뇨병군, 노화동물모델을 이용하여 대사지표과 미토콘드리아 기능분석을 시행하고, microarray 분석으로 표적유전자를 발굴, 일부 표적인자의 기능을 과발현실험을 통해 확인함. 결과: ① 노인근육이 양·질적으로 감소하고, 근감소증이 대사지표 악화와 연관됨. ② 노인당뇨군에서 체지방량 증가, 인슐린저항성 악화 관찰 보였음. 노인당뇨군에서 근육량은 차이 없었으나, 혈당조절 불량시에는

• As the aging progresses, muscle mass decreases in their quantity and quality and the body fat mass decreases. If one has sarcopenia, especially sarcopenic obesity, the metabolic parameters worsen.
• Diabetes group compared to control group in elderly showed increase in body fat, increase in insulin resistance index, and no difference in muscle mass and muscle strength. However, decrease in muscle strength and decrease in muscle quality was observed in diabetes patients with poor glycemic control. Cytokines like IL-6, TNF-a etc. increased in elderly diabetes group.
2. Change in skeletal muscle and mitochondrial function in elderly diabetes group
• Decrease in quantity and quality of mitochondrial function was observed in patients with diabetes. This change was more significant in elderly patients; additionally their mitochondria were morphologically destroyed.
• Due to aging and diabetes, the change in muscle quality was observed. Type 2 fiber was decreased and especially in diabetes, the size in one fiber was decreased
3. Animal models for aging
• Mouse model for aging with long term high fat diet showed increase in body fat mass, change in adipocytokine as a result. In skeletal muscle, decrease in mitochondrial function and increase in mitochondrial quantity were observed.
• Increased cytokine expression, decreased Catalase/GPx expression and increased ROS production as a result found in aged SD rat’s muscle, improved by long term exercise.
4. Target gene findings in various age groups and elderly groups with diabetes through gene analysis.
• By analyzing the gene expression, using affymatrixchip, in the skeletal muscle of human and animal models, candidate genes were found as follows.
Elderly group vs. young group (Human- 473 count increase, 180 count decrease in elderly gene expression; SD rat- 2224 count increase, 2565 count decrease in elderly gene expression) Elderly control group vs. diabetes group (Diabetes group- 2336 count increase, 2600 count decrease)
• By analyzing each gene expression, researchers were able to find many target genes, their pathways, and are continuing the analysis. The candidate gene related to the muscle mass or mitochondria has shown an increase in clusterin, PLA2G7, MYH3, MYH10, and GPx3 while showing a decrease in myostatin, IRS, UCP3, ARRDc2 and no significant change in Sirts in the elderly group.
5. Functional study of candidate genes
• Clusterin: Experiments on Clusterin overexpression were performed. It is believed that the increase of Clusterin in the muscle will perform compensatory effect to the glucose metabolism.
• Glutathione peroxidase 3: In the case of GPx3 overexpression, extracellular oxidative stress was reduced. It is believed that the reason the GPx3 increases in the muscles of elderly people is to compensate and overcome oxidative stress and insulin resistance.
• Other studies of genes are in progress such as UCP3, IRS, Nampt(nicotinamide phosphoribosyltransferase).
