연구 목표
우리나라 일반국민을 대표할 수 있는 인구집단에서 중금속노출 수준 및 건강영향지표에 대한 기초조사가 되어 있는 코호트를 추적 조사하여 중금속 노출수준을 결정하는 요인과 중금속 노출에 따른 건강영향을 분석하는 것을 일차적인 목표로 하였다. 본 연구를 통해 유해중금속 노출에 기여하는 식이요인을 규명하고 저 농도 중금속 노출의 건강영향을 평가함으로써 중금속 노출 저감화 정책을 위한 기반자료를 제공하는 것을 최종 목표로 하였다.
연구 방법
1차 연도에는 중금속 등 유해물질 노출 정도가 이미 평가된 코호트 (성인 2,000명, 아동 및 청소년 1,000명, 영유아 1,000명, 영유아 부모 600명 대상)에서 추적조사의 신뢰성 확보를 충족시키는 표본수로 대상자 수 결정하고 대상자 섭외를 하였다. 또한 대상자 섭외를 하면서 중금속 노출정도가 이미 평가된 코호트 구성원을 대상으로 식이섭취 실태 간이 조사를 실시하였고 2차년도 추적조사를 위한 예비조사를 실시하였다.
2차 연도에는 중금속 노출 수준과 중금속의 건강영향 생체지표의 변화 수준에 대한 추적조사를 실시하였고 추적조사를 통해 확보된 시료의 중금속 농도를 분석하였다.
3차 연도에는 1,2차년도 추적조사를 통해 고노출군으로 분류된 대상에서 반복 추적조사 및 중재연구를 실시하였으며 중금속 노출에 대한 주요 식이 기여요인 분석과 코호트 분석을 통해 저농도 중금속 노출이 건강에 미치는 영향을 분석하였다.
연구결과
1차년도 대상자 섭외 후 2차년도 추적조사에서 총 870명을 조사하였다 (성인 515명, 어린이 161명, 어린이 부모 194명).
중금속 노출 수준은 혈중 납의 경우, 기반조사 대비 추적조사 때 체내 농도가 감소한 연구참여자가 약 2배 정도 많았다 (감소한 군: 430명 (65.2%), 증가한 군: 230명 (34.8%)). 혈중 카드뮴의 경우, 기반조사 대비 추적조사 때의 중금속 체내 농도가 증가한 군과 감소한 군이 거의 비슷하였다 (감소한 군: 330명, 증가한 군: 326명). 하지만 요중 카드뮴의 경우, 추적조사 때 농도가 감소한 연구 참여자가 더 많은 것을 확인하였다 (감소한 군: 463명 (72.0%), 증가한 군: 183명(28.0%)). 혈중 수은의 경우, 기반조사 대비 추적조사 때 체내 농도가 감소한 연구 참여자가 더 많았다 (감소한 군: 473명 (71.5%), 증가한 군 189명(28.5%)).
동일인의 중금속 농도를 반복측정해서 생기는 상관성과 연령 및 조사기간의 효과를 같이 고려하기 위해, 개인의 랜덤절편(random intercept) 효과와 연령 및 조사기간을 설명변수로 하는 선형혼합모형 (Linear Mixed Model)을 이용하여 분석하였다.
선형혼합모형 분석결과 연령이 증가할수록 체내 농도는 증가하였으며 조사기간은 최근으로 올수록 감소하는 추세였다. (혈중 납은 연령이 1세 증가할 때 1.1% 증가하고, 조사기간이 1년 지날 때 4.0% 감소하였다. 혈중 수은은 연령이 1세 증가할 때 혈중 수은은 1.0% 증가하며, 조사기간이 1년 지날 때 혈중 수은은 7.1% 감소하였다. 혈중 카드뮴은 연령이 1세 증가할 때 1.6% 증가하며, 조사기간이 1년 지날 때 2.0% 감소하였다. 요중 카드뮴은 연령이 1세 증가할 때 2.3% 증가하고, 조사기간이 1년 지날 때 8.2% 감소하였다.) 이러한 결과는 연령이 증가할 수폭 중금속의 체내에 축적되기는 하나, 연령대별로 체내 축적율이 다른 것은 연령에 따른 체내 대사과정(흡수, 분포, 축적, 분해, 배출)이 중금속 축적에 중요한 영향을 미치고 있음을 함의한다. 그럼에도 불구하고, 기간에 따른 중금속 농도의 감소는 환경의 변화에 따라 감소된 것으로 해석할 수 있다.
본 연구의 경우 24시간 회상법을 통해 산출된 성인의 단위체중 당 식품을 통한 중금속 노출량은 납 0.14 μg/kg bw/day, 카드뮴 0.201 μg/kg bw/day, 수은 0.071 μg/kg bw/day로 2010년-2012년에 실시한 기반조사와 유사한 수준이었으며 중국과는 비슷하거나 약간 낮았고, 미국과 유럽 국가와도 유사한 수준으로 조사되었다. 식이를 통한 납, 카드뮴 및 수은의 노출수준은 안전한 것으로 판단되었다. 이번 연구에서 식품군별 카드뮴, 수은의 기여율은 어린이와 성인 모두 곡류와 어패류가 가장 높았고, 납의 경우 어린이는 과실류, 성인은 어패류가 가장 높았다. 이러한 결과는 기반조사를 통해서도 확인할 수 있었다. 기여식품의 경우 카드뮴의 백미를 제외하고는 어린이와 성인에서 차이를 보였는데, 어린이의 경우 납은 수박, 과일음료, 아이스크림 등을 통해 성인의 경우는 홍합, 고구마 등이 기여율이 높았다. 수은의 경우 어린이는 백미, 우유 순이었지만, 성인의 경우는 흰살 생선, 고등어, 명태, 백미, 갈치 순으로 어패류가 높은 순위를 차지하였다.
혈중 납 농도와 고혈압 유병 관련성은 통계적으로 유의한 관련성은 관찰되지 않았으나, 혈중카드뮴 농도가 높은 군에서 간기능 지표(혈청지오티(AST), 혈청지피티(ALT), 감마지피티(GGT))가 높았다. 또한, 혈중 카드뮴 수준이 높은 군에서 일부 골 손상 또는 신장 손상지표가 높았다. 혈중 수은 농도 수준이 높은 군에서 다수의 대사증후군 관련 지표 값들이 높았으며 혈중 수은이 높은 군에서 간 기능 지표 및 일부 신장 손상지표가 높았다. 이러한 지표들의 변화는 향후 중금속 노출에 대한 건강영향과의 관련성 규명을 통해 건강영향 지표로 활용될 수 있을 것이다.
중금속 저감을 위한 식생활 교육 중재연구 대상자는 HBM 등 국제 참고치를 기준으로 저농도 중금속 노출자 그룹 중 카드뮴의 농도가 상대적으로 높은 사람 15명과 수은 농도가 상대적으로 높은 108명 총 118명 (중복 5명)을 선정하였다. 대상자의 지역, 나이 분포를 파악한 결과 경상도 지역이 총 60명으로 전체 118명의 50%를 차지하였고 나이는 56.6세 (중위수)이다.
중재는 저농도 중금속 노출자 그룹 중 상대적으로 중금속 농도가 높은 그룹 대상의 중금속 노출 수준 저감을 위한 개인 식이섭취 및 생활 점검과 노출을 줄이기 위한 교육으로 진행하였고 시범 연구 참여 전후의 혈중 수은 농도 분포를 비교해 보았다. 중재 전에 비하여 중재 후의 혈중 수은 농도는 감소함을 관찰하였다 (중재 전: 기하평균 7.79 ㎍/L, 중위수 6.90 ㎍/L;중재 후: 기하평균 6.65 ㎍/L, 중위수 6.44 ㎍/L, p-value <0.001).
본 연구를 통해 우리나라 국민들의 중금속 노출 수준의 변화 추이를 파악할 수 있었으며 식이섭취 변화가 중금속 농도에 미치는 영향을 평가함으로서 미래 위해관리 상황에 능동적으로 대처할 수 있는 기반을 조성하였다. 또한 저농도 중금속 노출의 건강영향 평가 결과를 제시함으로서 과학적인 중금속 노출 저감화 정책을 마련할 수 있는 기반자료를 제공하였다.

Domestic studies also estimate that more than 90% of exposure to the hazardous heavy metals such as lead, cadmium and mercury resulted from intake (Ministry of Food and Drug Safety, 2012).
Control of chronic low level exposure of heavy metals have been increasingly strengthened.
In 2010, the JECFA withdrew the previous PTWI standard for lead of 25 ㎍/kg bw/week and strengthened the PTWI standard for cadmium from 7.0 ㎍/kg bw/day to PTMI 25 ㎍ /kg bw/month. For mercury, the JECFA withdrew the PTWI standard of 5.0 ㎍/kg bw/week and strengthened the standards for methylmercury and inorganic mercury to 1.6 ㎍/kg bw/week and 4.0 ㎍/kg bw/week respectively. In Korea, studies on the health impact by low-level exposure of heavy metals have been conducted in a limited way through the cross sectional research, resulting in lack of basis to establish the policies to control heavy metal exposure control, which is on a par with criteria of other advanced nations. Therefore, it is necessary to carry out the long-term follow up survey to monitor the heavy metal exposure status through intake of food, evaluate its health impact and provide the basis to develop the policies to lower heavy metal exposure.
Objective of research
This study aimed to analyze contributory factors to determine the level of heavy metal exposure and its health impact through a follow up survey on the cohort, which was selected from the population group that can represent the general public in Korea.
The baseline study on the level of heavy metal exposure and the health impact indicator had been carried out on this cohort. The object of this study is to provide fundamental data to establish policies to lower the exposure level of heavy metals by investigating dietary factors that contribute to exposure of hazardous heavy metals and evaluating the health impact of low level exposure of heavy metals.
Research methodologies
In the first year, the research team set the number of subjects so that the number of samples was sufficient to ensure the credibility of follow-up survey from the cohort (2,000 adults) of which the exposure level to hazardous chemicals including heavy metals had been evaluated. While contacting the subjects, the research team conducted a brief survey on the dietary intake of the cohort members, whose heavy metal exposure levels had been evaluated, and carried out the preliminary study for the follow-up survey in the second year.
In the second year, the team conducted the follow-up survey on heavy metal exposure levels and changes in indicators that indicate the heavy metals’ health impact. The research team also analyzed heavy metal concentration levels from the samples obtained by the follow-up survey.
In the third year, the team carried out the repetitive follow-up survey and intervention study for the subjects who have been categorized as the high-exposure group in the first and the second years. Through analysis on the dietary factors that contribute to heavy metal exposure levels and analysis on the cohort, the team analyzed the health impact caused by low-level exposure to heavy metals.
Research result
After selecting and contacting the subjects for the first year, a total of 870 persons were surveyed in the follow-up study for the second year (515 adults, 161 children and 194 parents of the children).
In case of lead in blood, the number of subjects whose heavy metal concentration decreased between the baseline survey and the follow-up survey was twice as high as others (Decreased group: 430 persons (65.2 %), increased group: 230 persons (34.8 %)).
In case of cadmium in blood, the numbers were similar between the increased group (326 persons) and the decreased group (330 persons)). However, in case of cadmium in urine, the researchers confirmed that the number of subjects whose concentration decreased in the follow-up survey was higher than the other (Decreased group: 463 (72.0 %), increased group: 183 (28.0 %)). In case of mercury in blood, more subjects showed decreased concentration levels in body between the baseline survey and the follow-up survey (Decreased group: 473 (71.5 %), increased group: 189 (28.5 %)) To take into consideration the co-relationship from repetitive measurement of heavy metal concentration on the same person and the effect of subjects’ age and survey period, analysis was performed by using the linear mixed model which had the random intercept effect of individuals, the age and the survey period as explanatory variables.
Lead in blood increased by 1.1 % when the subjects grew a year older and decreased by 4.0 % when 1 year of the survey period passed. Mercury in blood increased by 1.0 % when the age increased by 1 year and decreased by 7.1 % when the survey period increased by 1 year. Cadmium in blood increased by 1.6 % when the subjects grew a year older and decreased by 2.0 % for an increment of 1 year in the survey period.
Cadmium in urine increased by 2.3 % with an increment of the age by 1 year and decreased by 8.2 % when the survey period increased by 1 year. Using the 24-hour recall method, the researchers also calculated heavy metal exposure levels through food intake per unit weight of an adult, which were 0.14 μg/kg bw/day for lead, 0.201 μg/kg bw/day for cadmium, and 0.071 μg/kg bw/day for mercury. They were similar to the baseline survey results found between 2010 and 2012. They were equivalent or lower to those of China and on a par with levels in the U.S and European nations.
Based on the food group, contribution level of cadmium and mercury exposure was highest in grain and seafood for children and adults. In case of lead, fruits and seafood showed the highest contribution level for children and adults respectively.
This result was also confirmed through the baseline survey. The list of contributory food for heavy metal exposure showed a big difference between children and adults, excluding white rice for cadmium. The list of contributory food for lead exposure in children included watermelon, fruit juice and ice cream in the order of contribution. For adults, the top contributory food were mussel and sweet potato. In case of mercury, the top contributors for children were white rice and milk while those for adults were white fish, mackerel, Pollack, white rice and cutlass fish, showing seafood ranked at the top of the list.
Correlation between lead concentration in blood and hypertension prevalence showed a significant increasing trend in both baseline survey and follow-up survey where the hypertension prevalence increased according to the increase of lead concentration in blood among the tertiles. However, the age and gender-controlled model didn’t showed the statistically significant correlation. When subjects had high cadmium concentration in blood, liver function indicators were high accordingly. During the follow-up survey period, some liver function indicators showed an increasing trend but there was no obvious increasing trend in liver function damage. With higher cadmium concentration in blood, the subjects showed increasing trends in damage indicators of some bones or kidney.
However, there was no report on increase in bone or kidney damages during the follow-up survey period. For the subjects who showed high mercury concentration in blood, they showed an increasing trend in the metabolic syndrome related indicators. Some metabolic syndrome indicators went up during the survey period but there was no special increasing trend in the number of patients with metabolic syndrome. Those with high mercury concentration in blood showed high level of liver function indicators and some kidney damage indicators. However, there was no clear sign of impact to liver or kidney damages caused by mercury during the follow-up period.
Based on the reference level, such as HBM and other international standards, the research team selected 15 subjects who had been highly exposed to cadmium and 118 subjects highly exposed to mercury (5 subjects were overlapped). By categorizing subjects per region and age, the study showed that 60 subjects were from Gyeongsang Provinces, which accounted for 50% out of the total number of 118. The median age was 56.6. The researchers also compared the mercury concentration in blood before and after subjects’participation in the intervention study. Compared to the results before the intervention, mercury concentration in blood dropped significantly (Before intervention: geometric average 7.79 ㎍/L, median 6.90 ㎍/L; After intervention: geometric average 6.65 ㎍/L, median 6.44 ㎍/L, p-value <0.001). When checking up the distribution of subjects in recognition on mercury, awareness on the risk of mercury and opinions over mercury, recognition on mercury and awareness of the risk of mercury were in high levels even before the intervention study. Still, the median values of recognition and awareness on the risk increased from 8 to 9, and from 5 to 7 respectively after the intervention study.
Achievement
Through this study, the research team was able to analyze the trends in exposure to heavy metal among the public in Korea. By evaluating the impact of dietary intake to the heavy metal concentration, this study provided the foundation to take proactive actions for hazardous material control in the future. In addition, evaluation results on health impact by low level heavy metal exposure is expected to serve as the fundamental data to establish data-based policies to lower heavy metal exposure.
