연구의 목적 및 내용
본 연구는 만성통증에서 insular cortex (IC)에서 나타나는 신경가소성 변화와 이에 의한 통증조절 관련성을 알아보고 통증조절의 가능성을 체계적으로 분석하고자 하였다. 이에 본 연구에서는 long-term potentiation(LTP)을 유도하는 protein kinase M zeta (PKMζ), 세포의 증식을 조절하는데 관여하는 mammalian target of rapamycin (mTOR), 그리고 통증의 정보전달에 주요한 역할을 수행하는 fatty acid amide hydrolase
연구의 목적 및 내용
본 연구는 만성통증에서 insular cortex (IC)에서 나타나는 신경가소성 변화와 이에 의한 통증조절 관련성을 알아보고 통증조절의 가능성을 체계적으로 분석하고자 하였다. 이에 본 연구에서는 long-term potentiation(LTP)을 유도하는 protein kinase M zeta (PKMζ), 세포의 증식을 조절하는데 관여하는 mammalian target of rapamycin (mTOR), 그리고 통증의 정보전달에 주요한 역할을 수행하는 fatty acid amide hydrolase (FAAH)가 IC의 신경가소성 변화와 연관되어 있는지 알아보고, 통증조절과 어떠한 관련성이 있는지 행동학, 조직학, 분자생물학, 뇌영상 등 다차원적 융합 연구를 통해 규명하였다.
연구결과
본 연구는 총 3년의 연구계획을 수립하여 연차별 연구계획을 세우고 이를 달성하고자 하였다.
1차년도에는 통증관련 대뇌영역에서 신경손상에 따른 대뇌 활성도를 측정하고, 만성통증상태의 IC에서 발생되는 PKMζ 관련 신경가소성과 통증조절작용 기전을 밝히는데 주력하였으며, 통증에 의한 대뇌 pain matrix의 활성화 변화와 통증관련 표지인자 발현의 변화, PKMζ 관련 인자들을 추적하여 그 기전을 찾고자 하였다. 이를 토대로 만성통증이 발생한 이후 IC에서의 LTP가 강화됨을 보여주었으며, LTP소거시에 통증의 조절이 가능함을 증명하였다.
2차년도에는 통증관련 대뇌영역인 IC에서 일어나는 만성통증 관련 신경가소성과 mTOR signaling pathway의 연관성 규명하는데 주력하였으며, mTOR 신호전달을 저해하는 rapamycin을 처리한 뒤 그 하위인자들의 변화를 관찰함으로써 이로 유도되는 신경가소성 변화와 long-term potentiation(LTP)의 억제가 가능한지를 검증하였다. 또한 PKMζ와의 연관성을 분석한 결과 mTOR 감소시에 PKMζ의 감소를 확인하여 둘간의 상호작용이 LTP형성에 중요한 역할을 수행함을 증명하였다.
3차년도에는 통증관련 대뇌영역인 IC에서 일어나는 신경가소성과 FAAH의 관련성 및 기전을 이해하고 통증 조절과의 관련성을 규명하고자 했다. FAAH 조절에 의한 통증감소 효과를 검증한 결과 FAAH가 효과적으로 억제될 때 만성통증의 감소를 행동학적, 분자생물학적인 방법을 통하여 관찰하였으며, 또한 PKMζ와의 연관성을 분석한 결과 mTOR와는 달리 독립적으로 통증의 조절에 관여하는 것을 증명하였다.
본 연구를 통하여 통증의 유발과 지속적인 만성통증으로의 발달에 IC에서의 LTP가 연관됨을 밝혀내었으며, 신경세포의 구조적 변화유도가 만성통증의 발달과 밀접한 연관성이 있음을 보여주었다. 또한 만성통증으로의 발달과정의 기전을 연구함으로써 이를 저해할 수 있는 therapeutic target의 가능성을 제시하였고, 실제적인 통증의 경감효과를 증명하였다.
본 연구는 6편의 SCI급 국제학술지 발표를 통해 학술적 가치를 인정받았으며, 국내 및 국제학술대회에서의 다수 수상하였다. 또한 준비 중인 2편의 논문과 후속연구를 통해 보다 명확한 통증 정보 처리의 기전과 역할에 대해 탐구할 수 있으리라 생각되며, 이러한 연구는 만성통증과 조절에 관여하는 기초연구는 물론 새로운 통증치료 약물을 선도적으로 개발하는데에 훌륭한 초석이 될 수 있다.
연구결과의 활용계획
1. 본 연구의 결과는 신경손상에 의한 뇌 가소성과 만성통증의 상관관계를 밝혀내어 만성통증 기전을 설명할 수 있는 새로운 모델로 사용될 수 있다.
2. 신경가소성과 관련하여 PKMζ와 mTOR, FAAH의 새로운 signalling pathway를 밝혀내는데 기여할 수 있다.
3. 신경손상에 따라 만성통증이 유발되는 기전을 규명함으로써 새로운 therapeutic target을 밝혀낼 수 있는 선도적인 연구가 될 수 있다.
4. 본 연구결과 중 특히 mTOR의 signalling pathway의 조절은 세포의 증식을 조절하는 과정에 관여하는데, rapamycin과 같은 mTOR 억제제는 기존의 암 연구에서 사용되어온 안정화된 약물로 통증의 경감에도 사용될 수 있는 새로운 가능성을 보여준 것으로 그 활용가치가 매우 크다.
5. 이에 현재는 mTOR의 통증관련 세포증식 및 시냅스 변화와 관련한 연구를 추진 중이며, 이를 통하여 신경가소성과 관련된 만성통증의 조절작용을 규명하고 만성통증환자의 치료에 크게 기여할 수 있다.

The purpose of this study was to investigate the change of neural plasticity in the insular cortex (IC) in chronic pain and to investigate the possibility of pain modulation. In this study, we investigated the effects of long-term potentiation (LTP) induced protein kinase M zeta (PKMζ), mammalian target of rapamycin (mTOR), which regulates cell proliferation, and fatty Acid amide hydrolase (FAAH) associated with changes in neuronal plasticity of the IC, and its relationship to pain modulation is examined through multidimensional fusion studies such as behavioral, histology, molecular biology, and brain imaging research.
Result
This study was conducted as a three year research plan and an annual research plan was established.
In the first year, we measured brain activity according to nerve injury in the pain related cerebral regions, and focused on PKMζ-related neural plasticity and pain modulation mechanism in the IC regarding chronic pain. Activation of the pain matrix by chronic pain and PKMζ-related factors, and its mechanism were investigated. In the present results, we showed that LTP was strengthened in the IC after chronic pain, and proved that the pain can be controlled by LTP elimination.
In the second year, We focused on identifying the relationship between chronic pain-related neural plasticity and the mTOR signaling pathway in the IC. We also investigated the effects of rapamycin, which inhibits mTOR signaling, and the inhibition of LTP was verified. In addition, we analyzed that the decrease of PKMζ were associated with reduced mTOR activity, and demonstrated that the interaction between the two plays a major role in LTP formation.
In the third year, FAAH inhibitor effects were investigated in the area of pain related cerebrum to understand the mechanism of FAAH with neural plasticity in the IC. The results of pain reduction using FAAH inhibitor showed that when FAAH was effectively inhibited, the proof of reduced chronic pain were observed through behavioral and molecular methods. However, FAAH related pain modulation was not correlated with PKMζ and mTOR.
In this study, we found that LTP is involved in the induction of pain and development of chronic pain in the IC, and showed that initiation of structural changes are closely related to the development of chronic pain. In addition, we could suggest the possibility of a therapeutic target to inhibit the developmental process of chronic pain and the effect of pain relief by LTP modulation was demonstrated.
This study has been acknowledged in the academic value through the publication of six SCI - level international journals and had many opportunities for achieving outstanding research awards at domestic and international conferences. In addition, two papers (in preparation) and follow-up studies will allow to explore the mechanism and role of pain information processing. These studies will lead to the development of basic research related to control chronic pain and new pain medication.
Expected Contribution
1. The results of this study can be used as a new model to explain the correlation between brain plasticity and chronic pain caused by nerve injury.
2. It contributes to clarify the new signaling pathway of PKMζ, mTOR, and FAAH in relation to neural plasticity.
3. It can be a leading study to identify a new therapeutic target by identifying the mechanism of nerve injury induced chronic pain.
4. The mTOR inhibitor, such as rapamycin, is a stabilized drug that has been used in previous cancer studies. In the present study, we found that it can be used to relieve pain. It shows a new possibility and the utilization value is very huge.
5. Currently, we are conducting the research of the pain-related cell proliferation and synaptic changes via mTOR. We could identify the control of chronic pain related to neural plasticity and contribute to the treatment of patients with chronic pain.
