우리나라에서는 높은 음주율(OECD2위)과 사회경제적 부담에도 불구하고 음주의 건강영향을 연구한 개별 문헌들을 통합하여 체계적으로 고찰한 국내 연구가 없었다. 또한 근거기반의 지식과 정보에 기반을 둔 절주(금주) 관련 예방서비스 권고의 제정 및 보급·확산이 시급하다. 만성질환 전체를 대상으로 같은 분류기준을 적용하여 알코올 섭취 용량별 효과크기를 분석하여 질환 간 일관성 있는 결과 해석 및 권고 도출을 위한 명확한 근거의 확보도 필요하다.

본 체계적 문헌고찰의 목적은 첫째, 지역사회 인구집단을 대상으로 비음주군과 비교하여 성별로 음주 수준별 현재 음주군의 만성질환 발생 혹은 사망 위험을 분석하고, 둘째, 알코올 섭취량과 만성질환 발생 혹은 사망과의 관계(dose-response relationship) 유형을 파악하고, 셋째, 음주 수준별로 만성질환 발생 혹은 사망 위험이 국가 간, 연령층 간 차이가 있는지를 분석하는 데에 있다.
연구대상 만성질환은 14개 질환 이었다(2형 당뇨병, 고혈압, 심혈관질환, 관상동맥질환, 심근경색, 허혈성심장질환, 협심증, 심부전, 심방세동, 뇌졸중, 허혈뇌졸중, 출혈뇌졸중, 간경화, 지방간). 연구의 전문성을 위하여 3개 근거검토 및 SR 위원회(2형 당뇨병, 간질환 및 심혈관질환)를 다학제로 구성하여 운영하였다. 메타분석을 포함하는 체계적 문헌고찰은 Cochrane SR 방법론을 준수하여 수행하였다. Cochrane SR 방법론의 표준화를 위해 개발된 3 종류의 소프트웨어(1차 및 2차 문헌 스크리닝을 위한 COVIDENCE, 근거의 합성방법인 메타분석을 위한 RevMan 5.3, 최종 근거표(Evidence table) 생성을 위한 GRADEpro)를 모든 연구 참여 위원들이 학습하여 사용함으로써 연구과정의 투명성과 결과물의 신뢰도를 높였다.
3개 근거검토 및 SR 위원회에서 도출한 PICOH를 적용하여 검색전략을 개발하였고, 4개 국내 검색엔진(KoreaMed, Kmbase, RISS, NDSL)과 5개 국외 검색엔진(Cochrane, PubMed, Embase, PsycINFO, Wprim)을 대상으로 체계적 검색을 수행하였으며, Hand-searching도 수행하였다(중복제거 후 문헌검색 총수 23,737건, 2형 당뇨병 4,176건, 심혈관질환 10,907건, 간질환 8,654건). 포함 및 배제기준을 적용하여 1차 및 2차 스크리닝을 수행하여 총 184개 문헌이 본 연구에 포함되었다(2형 당뇨병 38개, 심혈관질환 121개, 간질환 25개). RCT 문헌이 없는 지역사회 인구집단 기반의 연구 영역이어서 관찰연구(코호트 및 환자-대조군 연구)가 포함되었다. Cochrane SR Handbook에서 Non-RCT 문헌의 질 평가를 위해 권고하고 있는 도구 중 New-Castle Ottawa scale을 사용하여 184개 문헌의 질을 평가하였고, 그래프로 표현하였다. 근거 문헌들의 효과 크기를 합성하기 위해서 전문자의 자문을 받아 Inverse Variance 방법, Random-model을 적용하는 메타분석 방법을 선택하였고, 총 1,288 세트의 메타분석을 수행하였다(1,074 세트의 random-effects meta-analysis, 214 세트의 sensitivity analysis). 이질성을 평가하여 I2가 60% 이상인 경우에는 민감도 분석을 수행하였다. 국가(혹은 대륙)별, 연령층별, 지역사회 혹은 직장별, Follow-up 기간별, 혼란변수 통제 개수별, 문헌의 질 수준별(NOS score)로 하위분석을 수행하여 하위그룹 간 차이를 분석하였다(2형 당뇨병은 BMI 수준, 가족력 유무 포함).
알코올 섭취량은 다음과 같이 6개 군으로 분류하여 사용하였다; Low(0.01-10.0 g/day), Light to Moderate(10.1-20.0 g/day), Moderate(20.1-40.0 g/day), Moderate to High(40.1-60.0g/.day), High(60.1-120.0 g/day), Very High(120.1+ g/day).
지역사회 인구집단을 대상으로 음주 수준별 현재 음주군의 만성질환 발생 혹은 사망위험을 요약하면 다음과 같다. 비음주군과 비교 했을 때 남성의 경우 하루 10.1-20.0 g 음주군 부터 고혈압 발생 위험이 증가하였으며(RR 1.21, 95% CI 1.08-1.36), 음주량이 증가함에 따라 고혈압 발생 역시 증가하였다(하루 120.1 g 이상 음주군의 경우 RR 2.28, 95% CI 1.99-2.61) (linear relationship). 비음주군과 비교 했을 때 하루 120.1 g 이상 음주군 에서 2형 당뇨병 발생 위험이 증가하였으며(RR 1.92, 95% CI 1.12-3.29), 중등도 알코올 섭취(20.1-40.0 g/day)군에서는 2형 당뇨병 발생의 위험이 감소하였다(남자 RR 0.75, 95% CI 0.68-0.84, 여자 RR 0.72, 95% CI 0.60-0.86) (U-shaped relationship). 비음주군과 비교 했을 때 하루 10.1-20.0 g 이상 음주군 부터 간경변 발생 위험이 증가하였으며, 하루 40.1-60.0 g 알코올 섭취군의 경우 간경변 발생 위험이 남자는 10.34배, 여자는 16.66배 높았다. 알코올 섭취량과 간경변의 발생 혹은 사망 위험과는 유의한 관련성이 있었다(linear relationship). 비음주군과 비교 했을 때 하루 0.01-10.0 g 과 10.1-20.0 g 음주군의 경우 허혈뇌졸중 발생이 감소하였으며(RR 0.60, 95% CI 0.42-0.88; RR 0.61, 95% CI 0.40-0.93, respectively), 하루 120.1 g 이상 음주군의 경우 허혈뇌졸중 발생이 증가하였다(RR 5.05, 95% CI 1.53-16.67) (J-shaped relationship).
2형 당뇨병, 지방간 및 심혈관질환의 발생 혹은 사망은 보통 정도 용량의 알코올을 섭취한 경우까지 보호효과가 확인되기는 하였으나, 코호트 연구 혹은 환자-대조군 연구와 같은 관찰연구로부터 도출된 결과로 메타 분석에 사용된 문헌들의 Certainty가 높지 않고 (낮음 혹은 매우 낮음), 이로 인한 비뚤림을 배제할 수 없으므로 보호효과가 있다고 명확히 결론을 내리기는 어렵다. 따라서 2형 당뇨병, 지방간 및 심혈관질환의 경우에는 소량 혹은 보통 정도의 음주를 통한 이득 보다는 높은 용량의 알코올 섭취로 인한 사망 혹은 발생의 위험으로 인한 위해가 크기 때문에 금주로 얻을 수 있는 이득이 더 크고 확실하여, 만성질환의 발생 및 사망을 예방하기 위해서는 가능하면 가벼운 음주군 부터 발생이 증가하는 고혈압, 간경변 등과 같은 만성질환 간의 밸런스를 감안하여 금주 권고를 고려할 필요가 있다.

Despite the high drinking rate (OECD 2nd place) and the socio-economic burden in Korea, there were no domestic studies that systematically reviewed individual literatures that studied effects of alcohol on human health. In addition, the establishment and dissemination of recommendations for abstinence (or stop drinking) utilizing evidence based knowledge and information are urgent. It is necessary to analyze scale of effect by level of alcohol consumption using the same classification criteria for the entire chronic disease and to provide a clear basis for interpreting the consistent results between diseases and for deriving recommendations.
The purposes of this systematic review are as follows; first, to analyze the risk of chronic disease incidence and/or mortality of current drinking group compared to non-drinkers according to the level of alcohol consumption by gender in the community, second, to explore the type of dose-response relationship between alcohol consumption and chronic disease incidence and/or mortality, third, to analyze whether there is a difference of incidence and/or mortality of chronic disease among countries and age groups according to the level of alcohol consumption.
14 chronic diseases (type 2 diabetes, hypertension, cardiovascular disease, coronary heart disease, myocardial infarction, ischemic heart disease, angina pectoris, heart failure, atrial fibrillation, stroke, ischemic stroke, hemorrhagic stroke, cirrhosis, and fat liver) were identified. For expertise of research, we have established three evidence reviews and SR committees (type 2 diabetes, liver disease, and cardiovascular disease) in a multidisciplinary manner. A systematic review including meta-analysis was conducted in accordance with the Cochrane SR methodology. Three types of software developed for the standardization of the Cochrane SR methodology were used (COVIDENCE for primary and secondary screening, RevMan 5.3 for meta-analysis, which is a synthesis method of evidence, GRADEpro for creating Evidence table). All researchers learned and utilized three Cochrane’s softwares mentioned above to enhance the transparency of the research process and reliability of the results.
We have developed a search strategy using PICOH derived from the committee. We have developed a search strategy for four domestic search engines (KoreaMed, Kmbase, RISS, NDSL) and five overseas search engines (Cochrane, PubMed, Embase, PsycINF) (23,737 cases of document retrieval after de duplication; 4,176 cases of type 2 diabetes, 10,907 cases of cardiovascular disease, and 8,654 cases of liver disease). A total of 184 articles were included in this study (38 type 2 diabetes, 121 cardiovascular disease, and 25 liver disease) by performing primary and secondary screening using the inclusion and exclusion criteria. Observational studies (cohort and patient-control studies) were included due to a population-based research area without randomized controlled trials. The quality of 184 studies was evaluated and represented in graph using the New-Castle Ottawa scale which is recommended by Cochrane for the quality evaluation of non-RCT.
A total of 1,288 sets of meta-analyzes were performed using Inverse Variance method and random model (1,074 sets of random-effects meta-analysis, 214 sets of sensitivity analysis). Sensitivity analysis was performed when I2 was 60% or more to resolve heterogeneity. Sub-group analysis was performed by country (or continent), age group, community and/or workplace, follow-up period, number of adjusted confounders, quality of studies (NOS score) (include the presence or absence of a family history and BMI for Type 2 diabetes).
Alcohol intake was classified into six groups as follows: Low (0.01-10.0 g / day), Light to Moderate (10.1-20.0 g / day), Moderate (20.1-40.0 g / day), Moderate to High (40.1-60.0 g / day), High (60.1-120.0 g / day), and Very High (120.1+ g / day).
Compared to non-drinkers in the community population, the risk of chronic disease of current drinkers by level of alcohol consumption is as follows. The risk of hypertension (RR 1.21, 95% CI 1.08-1.36) increased from 10.1-20.0 g/day in men compared to non-drinkers, and the incidence of hypertension also increased as the drinking level increased (120.1+ g/day; RR 2.28, 95% CI 1.99-2.61) (linear relationship). The risk of type 2 diabetes was increased in the very high alcohol intake group (120.1+ g/day) compared to non-drinkers (RR 1.92, 95% CI 1.12-3.29), and the incidence of type 2 diabetes was reduced in the moderate alcohol intake group (20.1-40.0 g/day) (male RR 0.75, 95% CI 0.68-0.84, female RR 0.72, 95% CI 0.60-0.86) (U-shaped relationship). The risk of liver cirrhosis increased from 10.1-20.0 g/day, and the risk of cirrhosis was 10.34 times higher in men and 16.66 times higher in women compared to non-drinkers in the moderate to high alcohol intake group (40.1-60.0 g/day). There was a significant linear relationship between alcohol intake and the risk of cirrhosis incidence and/or mortality. Compared to the non-drinkers, the incidence of ischemic stroke was decreased in the low (0.1-10.0 g/day) and light to moderate alcohol intake group (10.1-20.0 g/day) (RR 0.60, 95% CI 0.42-0.88, RR 0.61, 95% CI 0.40-0.93, respectively). Incidence of ischemic stroke was increased in very high alcohol intake group (120.1 g/day) (RR 5.05, 95% CI 1.53-16.67) (J-shaped relationship).
The incidence and/or mortality of type 2 diabetes, fat liver, and cardiovascular disease has been shown to be protective at the moderate alcohol intake (20.1-40.0 g/day), but it is difficult to conclude definitively that there is a protective effect. The certainty of studies included in meta-analysis was not high (low or very low) and it could not rule out the risk of bias for observational studies such as cohort or case-control studies.
Therefore, the harm due to incidence and/or mortality of high-dose alcohol intake is greater than the benefit of small or moderate drinking for type 2 diabetes, fatty liver and cardiovascular disease. To prevent incidence and/or mortality of chronic diseases, it is necessary to recommend stop drinking by considering the balance between chronic diseases such as hypertension and cirrhosis which are more likely to occur from a light drinking group.
