□ 연구의 필요성
우리나라의 의료 환경에서 의약품과 한약은 동시에 질병치료와 건강증진에 활용되고 있는데 의약품은 부작용보고와 약물감시시스템이 잘 갖춰져 있으나, 한약은 부작용보고가 잘 이루어지고 있지 않다. 한약은 의약품과 달리 ATC(Anatomical Therapeutic Chemical)코드와 같은 분류체계가 갖춰져 있지 않아 체계적인 부작용의 관리, 분석, 평가에 어려움이 있다. 따라서 한약의 부작용보고 관리와 국제공유를 위해 한약의 분류체계 마련이 필요하다.

□ 연구목표
이 연구는 한약의 부작용보고를 위한 분류코드 체계 마련 연구로 한약부작용보고를 위한 체계구축 및 기반조성을 목적으로 한다. 세부적으로 한약 부작용보고를 위한 코드 및 분류체계를 구성하고 코드 데이터베이스를 마련하고자 한다.
□ 연구내용 및 방법
세계보건기구(WHO) 및 국내외의 한약분류와 코드체계를 조사하였다. WHO의 ATC코드와 herbal ATC (HATC)코드의 분류와 색인을 분석하였으며, 일본의 의약데이터파일(IDF)코드와 ATC를 매핑한 CRT-J (cross reference tool-Japan)를 확보하여 분석하였고, 중국의 중약코드체계를 세계표준기구작업초안 18668-4 (ISO/WD 18668-4)를 확보하여 비교 분석하였다. 국내 한약분류 현황과 관련 코드를 검토하고, KMATC(Korean Medicine ATC)라 명명한 한약분류 코드체계를 제시하였다.
한약분류 코드체계에 맞추어 약재, 제제, 처방의 KMATC코드를 부여하였으며, 구성한 DB의 분류표와 색인을 작성하였다. 한약과 ATC를 매핑하는 CRT-KM을 개발하였으며, 한약분류체계의 국제조화를 위한 활동을 하였다.
새로운 한약분류체계가 한약 부작용보고에 잘 적용되는지 현장적용연구(Field Trial)를 실시하였으며,아울러 포커스그룹 인터뷰(Focus Group Interview)를 통해 KMATC코드의 평가를 병행하였다. 한약부작용보고를 위한 기반을 마련하기 위해 지역한약부작용보고센터의 설치와 국내 의약품이상사례보고시스템 개선을 제안하였다.
□ 연구결과 및 고찰
ATC코드는 WHO에서 개발한 국제적인 의약품 분류코드이며, 의약품을 보다 효율적이고 체계적으로 분류하기 위한 치료제군 별 의약품 분류코드로 5단계 7자리의 영문 및 숫자로 구성된다. Herbal ATC는 herbal medicine에 부여된 코드로 ATC시스템과 호환되며, 5단계 9자리의 영문 및 숫자로 구성된다. 4단계까지 ATC와 완전하게 동일하지는 않으나 유사한 수준을 유지하며, 5단계는 개별 한약 분류이다.
일본은 자국내 한약제제와 ATC를 매핑하는 Cross reference tool을 보유하고 있으며, 중국은 ISO TC249에 중약분류코드체계의 표준안을 제안하였는데, 한의학적 효능분류에 따라 20개의 대분류와 60개의 중분류로 구성하였다.
본 연구진 역시 한의학적 효능분류에 따라 한약분류코드체계를 제안하였으며, 관련 전문가와 학회 등의 자문을 거쳐 최종 22개의 대분류와 69개의 중분류로 구성하였다.
KMATC코드가 부여된 한약은 약재에 해당하는 주원료 776개, 제제에 해당하는 품목 4,405개, 그리고 10종 한약서에 수재된 처방 18,634개이며, KMATC코드는 Herbal ATC와 유사하게 5단계 9자리의 영문 및 숫자로 구성되었다. 1,2단계는 ATC에서 herbal medicine을 나타내는 V90이 부여되며, 3단계는 한약분류상 대분류, 4단계는 중분류, 5단계는 개별 한약 분류이다. 개별 한약 분류를 기준으로 KMATC의 코드수는 모두 10,498개이다. CRT-Japan과 유사하게 한약과 ATC의 연계테이블이 작성되었으며, 한약분류체계의 국제조화를 위해 국제약물감시학회(ISoP), 웁살라모니터링센터(UMC),ISO TC249의 전문가들과 네트워크를 구성하고 교류하였다.
한약 부작용보고시 KMATC의 적용가능성을 현장적용시험을 통해 시행한 결과, 모든 한약에 대해 KMATC코딩이 가능하였으며, 높은 코딩정확성과 일관성을 보였다. 또한 Focus Group Interview를 통해 KMATC의 상세성, 난이도, 상호배타성, 정의명료성을 5점척도로 평가하였으며, 각각 3.12, 2.62, 3.73, 3.92점으로 측정되었다.
KMATC코드체계의 마련과 함께 한약 부작용보고의 기반을 마련하기 위해서는 지역한약부작용보고센터의 설치와 의약품이상사례보고시스템의 개선이 필요하다고 제안하였으며, 지역한약부작용보고센터는 대학병원의 지역센터와 대한한의사협회의 중앙센터를 모형으로 제시하였다. 의약품이상사례보고시스템은 KMATC DB의 구축, 관리 및 유지, KMATC의 검색과 입력이 가능하도록 유저 인터페이스(U.I)의 개선, KMATC를 이용한 한약 안전성정보의 제공확대를 개선사항으로 제시하였다.
다만, WHO PV시스템에서 사용하는 ATC분류와 코드체계는 시판되는 제품에 대한 것이며, 시판되는 제제가 아닌 한약(한약재 혹은 한약처방)까지 포괄하여 부작용보고 체계를 구축하기 위해서는 이 연구에서 포함되지 않은 한약처방의 추가 등록과 전반적인 안전관리체계와 함께 고려되어야 한다.
한약 ATC코드가 국제적으로 활용되기 위해서는 WHO UMC의 승인이 필요하며, WHO PV의 ATC Code를 생성하는 WHO UMC와 NORWAY WHO CC 등과 좀 더 긴밀한 협조가 필요하다.

In the public health system of Korea, herbal medicines are concurrently used with medicines for treating diseases and promoting health. Adverse event (AE) reporting and pharmacovigilance system are established well in medicinal products, but AEs have not been reported in herbal medicines. Unlike medicinal products, herbal medicine has no classification system such as ATC classification, which makes it difficult to manage, analyze and evaluate AEs systematically. Thus, it is necessary to establish a classification system of herbal medicine for AE management.
□ Objectives
This is a study for establishing a classification code system for herbal medicines and the purpose of this study is to develop a basis for AE reporting. Specifically, researchers planned to organize herbal medicine classification system and code database.
□ Methods
Herbal medicine classification and code system of WHO, Korea, Japan and China were investigated. ATC and herbal ATC (HATC) were analyzed and cross reference tool-Japan(CRT-J),result in mapping Iyakuhinmei data file (IDF) and ATC code, was analyzed as well. The Traditional Chinese medicine (TCM) classification was reviewed and we focused on the International Standards Organization Working Draft 18668-4 (ISO / WD 18668-4). We proposed the herbal medicine classification code system named KMATC (Korean Medicine ATC).
The KMATC codes of herbal medicines (ingredients, products and prescriptions) were assigned according to the herbal medicine classification code system, and the index and the classification table of DB were prepared. We have developed CRT-KM that maps herbal medicines and ATC.
Field trials were conducted to confirm if the KMATC was appropriately applied to AE reporting of herbal medicine. In addition, an evaluation of KMATC was conducted through Focus Group Interview. In order to establish the basis for AE reporting of herbal medicine, the establishment of the regional pharmacovigilance center for herbal medicine and the improvement of AE reporting system in Korea were proposed.
□ Results and Conclusions
The ATC code is an international pharmaceutical classification code developed by WHO. It is composed of 7 digits in 5 levels for classifying medicines more efficiently and systematically.
Herbal ATC is a code assigned to herbal medicine that is compatible with the ATC system and consists of 5 levels and 9 digits. Up to the 4th step, Herbal ATC is not completely identical with the ATC, but similar, and the 5th step is the individual herbal classification.
Japan has a cross reference tool that can map IDF of herbal medicine and ATC. China proposed a standard of TCM classification system based on ISO/TC 249. It consists of 20 major categories and 60 minor categories. We also proposed the herbal medicine classification system. After consulting related specialists and academic societies, we made up the final 22 major categories and 69 minor categories.
We assigned KMATC to each herbal medicine; 776 herbal ingredients, 4,405 products, 18,634 prescriptions in 10 old pharmaceutical bible. KMATC are similar to Herbal ATC, and consist of 6 levels and 9 digits. In level 1 and level 2, V90, which represents herbal medicine in ATC, is assigned. In level 3 is the major category as per herbal medicine classification, the level 4 is the minor category, the 5th is the identifier of individual herbal medicine. Based on the individual herbal medicine classification, the number of codes of KMATC was 10,498. As with CRT-Japan, a bridge file between herbal medicines and ATC has been created. In order to harmonize the herbal medicine classification system, a network has been established and exchanged with the experts of the International Pharmacovigilance Surveillance Society (ISoP), Uppsala Monitoring Center (UMC) and ISO TC249.
As a result of Field Trial to confirm that KMATC can actually be applied to AE reporting of herbal medicine, all herbal medicines can be given a KMATC code and shows high coding accuracy and consistency. The Focus Group Interview was used to evaluate the KMATC's details,difficulty, mutual exclusiveness, and definition clarity on the 5-point scale, and the results was 3.12, 2.62, 3.73, and 3.92, respectively.
In order to establish the pharmacovigilance system of herbal medicine, it is suggested that the establishment of regional pharmacovigilance center for herbal medicine and the adverse event reporting system should be improved.
The WHO pharmacovigilance system is for licensed pharmaceutical products.
Therefore, it is necessary to obtain WHO UMC approval for using ATC code in order to report AEs including herbal medicine (ingredients or prescription) which is not a approved herbal medicine.
More cooperation with WHO UMC and WHO CC is needed.
