< 연구 개발 내용 >
본 과제를 통해 실리콘 보형물의 섬유화를 억제할 수 있는 약물전달, 표면 마이크로 형상제어방법론 및 생체적응소재등의 다양한 방법들을 개발하여 기능성 보형물을 개발하고
이를 전임상 소동물과, 대동물 모델에서 섬유화 평가 모델을 개발 및 평가하여 기존 일반 보형물에 비해 우수한 섬유화 억제 정도를 확인하였다.
이를 최적화하기 위해 1,2,3세부의 긴밀한 산학연병 협력으로 개발된 기능성 보형물의 실리콘 표면의 물리적/화학적 특성의 지속적 평가 및 최적화를 진행하였고.
다수의 Lot에서도 생상 및 재현성 확인평가를 수행하며 실제 양상공정을 지속적으로 개선하였다.
개발된 기능성 보형물은 공인인증평가를 통해 의약품 시험항목 공인인증 시험성적서를 확보하였으며 생물학적 안정성 및 다수 시험 보고서를 확보하였고,
이를 토대로 분당서울대병원 IRB 및 식약처 IND승인을 득하였다.
< 연구 개발 성과 >
위 실리콘 섬유화 및 표면개질 연구의 약물전달과 생체적합성 소재 개발, 표면연구등의 창의적 결과를 토대로 SCI급 국외논문 26개, 국내논문 9편 및 특허 등록 4건, 특허 출원(국내외) 8건, 시제품 개발 1건, 비임상시험 실시 10건, IND승인 1건을 달성 하였으며, 또한 코로나19 사태 이전 국내외 저명 학회 및 학술대회에 해당 연구결과를 발표하였음.
< 활용 계획 및 기대 효과 >
인공유방 보형물은 최근 3년 동안 부작용 보고 건수가 1389건으로 많은 부작용을 보고하고 있다. 이러한 국내외 가장큰 문제로 여겨지는 BIA-ALCL과 섬유화의 억제와 또한 다른 이식형기기의 부작용등을 해결 할 수 있는 활용가능성이 예상됨. 본 과제의 연구결과를 바탕으로 다양한 이식형 의료기기로의 기술 적용 및 이의 평가 방법의 제시하였으며 또한 다양한 약물 전달, 생체적합성 생체 소재 기반 코팅 방법을 개발함에 있어 원천기술을 제공하였다. 이를 다양한 의료기기에 적용함으로써, 이식형 보형물에 연관된 다수의 부작용 억제 기능을 부여, 이를 효과적으로 억제 가능한 기능성 의료기기 개발의 발판을 마련하였음.

According to a recent report, patients who underwent breast augmentation have suffered a lot of damage, and reoperation due to fibrosis accounts for many of the side effects of implants. The market demand for next-generation functional breast implants to effectively suppress these side effects is increasing rapidly. Therefore, in view of the continued increasing trend of the breast implant market and the frequency of side effects caused by fibrosis, if the development of an implant that has the same function as the previously approved product while obtaining fibrosis supressive properties, It will be able to preempt a very advantageous position in the medical device market.
Therefore, this project's aim is to develop a functional biomaterial and drug delivery technology that inhibits fibrosis, develop a surface-modified silicone implant based on this, and evaluate the effectiveness/safety using a preclinical animal model. Through this project, we developed a variety of methods such as drug delivery, surface micro shape control methodology, and biocompatible materials that can inhibit fibrosis of silicone implants to develop functional implants. This was confirmed by developing and evaluating a fibrosis evaluation model in preclinical small and large animal models to confirm the degree of inhibition of fibrosis superior to conventional implants. In order to optimize this, continuous evaluation and optimization of the physical/chemical properties of the silicone surface of the functional implant developed in close cooperation with industry-university-hospital were conducted.
The production and reproducibility confirmation evaluation was performed on a number of lots, and the actual production process was continuously improved.
For the developed functional implants, we secured a test report for accredited drug test items through accredited certification evaluation, and secured biological stability and multiple test reports. Based on this, it obtained IRB approval from Seoul National University Bundang Hospital and IND approval from the Ministry of Food and Drug Safety.

< Utilization plan and expected effect >
Based on the research results of this project, the application of technology to various implantable medical devices and evaluation methods thereof were presented, and the original technology was provided in developing various drug delivery and biocompatible biomaterial-based coating methods. By applying this to a variety of medical devices, it has provided the function of suppressing a number of side effects related to implantable implants, thereby laying the foundation for the development of functional medical devices that can effectively suppress them.
