유전자 편집 기술은 유전질환뿐만 아니라 암, 감염증, 자가면역질환과 같은 난치성 질환 치료제 개발에 폭넓게 활용되고 있으며, 이와 관련한 임상 및 제품화 노력이 세계적으로 활발히 진행되고 있다. 특히 3세대 유전자가위 기술로 불리는 CRISPR/Cas9 system은 높은 특이성과 정확성으로 매우 유망한 유전자치료 기술의 하나로 주목받고 있으며, 가파른 세계시장 확대가 예상되고 있다. 최근 국내외 규제당국에서는 ‘유전자편집 기반 유전자치료제의 품질 및 안전성 평가 가이드라인’을 제시하고 있으나, 이를 합리적으로 적용하기 위한 과학적

본 연구에서 조사한 CRISPR/Cas9 기반 유전자치료제 개발 및 임상시험 해외동향 분석 결과를 토대로, 향후 제품화 가능성이 높은 유전자치료제 3종에 대하여 각 질환별(비소세포성폐암, 겸상적혈구 빈혈증, HIV감염증)로 표적유전자(PD-1, BCL11A, CCR5)를 선정하였다. CRISPR/Cas9 유전자편집기술을 이용하여 3종의 유전자가위 기반 유전자치료제 검체를 제작하였고, 범용적으로 적용 가능한 확인 및 역가시험을 실시하였다. 유전자 편집 후 표적 위치의 Indel (insertion and deletion)을 확인하기 위해 T7E1 assay와 차세대 염기서열분석법 (NGS)를 수행한 결과, 모두 재현성 있는 Indel(%) 결과를 나타내었다. NGS 결과분석은 세계적으로 널리 쓰이는 CRISPResso2와 국내 개발 Cas-Analyzer를 사용하였으며, 3종의 유전자가위 기반 유전자치료제 모두에서 두 분석 도구의 Indel(%) 결과가 유사함을 확인하였다. 또한, CRISPR/Cas9 기반 PD-1 표적 유전자치료제를 PD-1 단백질을 발현하는 세포 (MOLT-4)에 적용시켜 ELISA와 flow cytometry를 이용하여 역가를 확인하였으며, ICH 가이드라인에 따라 특이성, 정밀성, 직선성, 정확성 항목을 평가하여 검증하였다. 추가하여, 안전성 평가를 위해 PD-1 유전자치료제의 비표적 site를 in silico 방법을 통해 예측하였고, 이 중 off-target 가능성이 높은 2개의 site에 대해 Indel이 발생하지 않는 것을 NGS로 확인하였다.
본 연구에서는 CRISP/Cas9 기반 유전자치료제 검체를 제작하고 범용적으로 활용 가능한 확인 및 역가 평가법을 확립•검증 후, 각 시험법들에 대한 SOP 를 마련하였다. 또한 안전성 평가에 중요한 off-target 평가를 추가적으로 실시하여 CRISPR/Cas9 기반 유전자치료제의 품질 및 안전성 평가 기반을 선제적으로 마련하고자 하였다. 본 연구 결과는 향후 CRISPR/Cas9 기반 유전자치료제의 허가심사 자료 검토 시 고려사항 및 사후관리(수거검정) 시스템 확립, 관련 가이드라인(안) 마련 시 참고 자료로 활용하고자 한다.

The third-generation gene-editing technology, CRISPR/Cas9, has been applied for the development of gene therapies for various diseases such as cancer, metabolic disease, autoimmune diseases as well as genetic diseases due to its high specificity and accuracy, and versatility. Despite the remarkable advances in CRISPR/Cas9 over the past decade, the assessment methods for CRISPR/Cas9-based human gene-editing products remain to be validated from regulatory perspectives.
In this study, we assessed the quality and safety of CRISPR/Cas9-based gene-editing products by current ‘on-target’ and ‘off-target’ analysis methods. To examine gene-editing efficiency, we produced three CRISPR/Cas9-based products targeting programmed cell death 1 (PD-1), B-cell lymphoma/ leukemia 11A (BCL11A), and C-C chemokine receptor type 5 (CCR5), respectively. The gene-editing products were transfected into HEK293T for identification test. Insertion and Deletion (Indel) were identified by surveyor nuclease assay (T7 Endonuclease I, T7E1 assay) and NGS (next-generation sequencing) followed by an analysis with RGEN-tools and CRISPResso2 software. The NGS results of PD-1-, BCL11A-, and CCR5-edited products indicated that the Indel events occurred in 92%, 99%, and 73% of transfected HEK293T cells, respectively. The validation of the assessment method shows high precision, specificity, accuracy, robustness, and linearity according to ICH guidelines. Next, the potency assay was performed after transfection of CRISPR/Cas9-based PD-1 gene editing product in MOLT-4 cell which is a T lymphoblast cell line expressing PD-1. From the results of the ELISA assay, the expression level of PD-1 protein was decreased by 44% in PD-1 gene-edited MOLT-4 cells, demonstrating that the CRISPR/Cas9 system knockout the PD-1 gene in MOLT-4 cells. In addition, we assessed off-target effects by using Cas-OFFinder, a ‘in silico’ off-target site prediction tool. Among the predicted off-target site for PD-1 targeted CRISPR/Cas9, we selected two DNA sites for further confirmation of the sites with NGS analysis. From NGS results, the Indel was not detected on the two off-target sites (<0.1%), suggesting the use of multiple off-target site analysis tools for off-target prediction.
Taken together, we successfully generated CRISPR/Cas9-based gene-editing products and evaluated the on-target and off-target effects with validation and SOPs for the assessment methods. We found that the PD-1 targeted CRISPR/Cas9 products efficiently promote DNA Indel in HEK293T cells and decrease PD-1 protein level in MOLT-4 cells in vitro, suggesting these assessment methods would be useful to evaluate human gene-edited gene therapy products. It is expected that these results would contribute to regulatory issues and the revision of guidelines for CRISPR/Cas9-based gene editing products.
