BACTEC배양을 기준으로 한 PCR의 민감도, 특이도, 양성 및 음성 예측도는 각각 50.9%, 97.3%, 52.5% 및 97.1%이었다 이 기간 동안에 호흡기 검체로 두가지 검사를 동시에 의뢰한 환자는 총 2,590명이었고 그 중에서 295명이 결핵으로 판정되었는데, 이때 결핵환자를 진단하기 위한 PCR의 민감도, 특이도, 양성 및 음성 예측도는 각각 64.7%, 98.5%, 84.8% 및 95.6%이었고, BACTEC배양의 경우는 각각 86.7%, 100%, 100% 및 98.3%이었다. 그리고 결핵을 보고하기까지 BACTEC TB system은 최소한 2주일이 걸린 반면, PCR은 평균 5시간으로 매우 짧은 시간이 소요되었다. PCR은 임상 검체에서 결핵균을 검출하는데 매우 신속하고 특이도가 높은 검사이지만, PCR의 위음성을 감안하여 결핵을 진단하기 위해서는 PCR과 함께 배양을 시행해야겠고, 여러 번의 반복 검사가 중요하다고 생각된다.

To evaluate the reliability of the PCR assay, the PCR results were compared with those of culture and clinical diagnosis. This study analyzed the results of BACTEC culture and PCR for detection of M. tuberculosis between Jan 1996 and Dec 1998. Of 7,430 samples for the PCR and 16,163 samples the TB BACTEC culture, 4,810 specimens (3,167 patients) submitted to both PCR and BACTEC culture at the same time were selected for the clinical evaluation of PCR. The sensitivity, specificity, positive and negative predictive values for the PCR assay were 64.7%, 98.5%, 84.8%, and 95.6%, respectively; the values for BACTEC culture were 86.7%, 100%, 100%, and 98.3%, respectively. The average detection times were 5 hours in PCR but 15.8 days in BACTEC culture. In this study, the PCR assay is very rapid and specific for the detection of M. tuberculosis in a clinical laboratory. The PCR assay should be run as an adjuvant to BACTEC culture for each patient to diagnosis accurately tuberculosis infection, and a single-sample PCR-negative result must be interpreted carefully because of potential false-negative.
