본 연구는 요통과 골반통을 경험하고 있는 임부를 위해 요통과 골반통에 대한 이해, 신체기전과 올바른 자세에 대한 교육과 요·골반통완화 운동을 포함한 프로그램을 개발하여 요통과 골반통 완화에 효과가 있는지 검증하고자 시도하였다. 연구설계는 비동등성 대조군 전후실험설계인 유사실험연구이다. 연구기간은 2001년 2월 17일부터 2001년 7월 28일까지였으며, 대상자는 광주광역시와 전라남도 중소도시에 거주하는 재태기간 17∼22주 사이의 요통과 골반통이 있는 임부로 실험군 29명, 대조군 27명(총 56명)을 내원 순서대로 병원을 다르게 선정하여 표집하였다. 사전조사로는 동통부위, 통증정도, 자세안정성, 운동효능감, 기능수행정도, 불안정도를 측정하였다. 실험처치는 요통과 골반통 교육을 실시한 후 가정에서 비디오 테이프를 활용한 요·골반통완화 운동을 3∼5회/1주, 20∼30분/1회로 총 12주 동안 실시하였다. 사후조사는 임신 23∼28주(1차), 임신 29∼34주(2차)에 사전조사와 동일하게 측정하였다. ...

Lumbar or posterior pelvic pain is a commonly observed symptom during pregnancy Despite its high frequency, we do not often concern about nursing intervention. Thus, this study was conducted to develop a pain-reducing program for pregnant women experiencing lumbar or posterior pelvic pain and to test the effects of a program on the pain during pregnancy. The research design was a quasi-experiment design with non-equivalent control group pre-and post-test. The sample of 56 pregnant women between 17 and 22 weeks' gestation were participated in this study from February 17 through July 28, 2001. All pregnant women who visited an antenatal clinic and had experienced lumbar or posterior pelvic pain were included in an experimental group (N=29), the results were compared with those of women in a control group (N=27) from another antenatal clinic in G metropolitan city, Korea. The experimental and control groups were compared on measures of pain distribution with the instruments of pain drawing, pain intensity (VAS), postural stability, exercise self-efficacy, functional ability & daily functional limitation and anxiety scale. The variables were measured in both groups before the intervention and were compared on measures at 6 weeks (23-28 weeks' gestation) and 12 weeks (29-34 weeks' gestation) after the intervention. The program was designed to increase an understanding of the mechanism of pregnancy-related back pain and to practice an exercise to relieve lumbar or posterior pelvic pain. Women in the experimental group received an education and a demonstrated exercise once, and then practiced at home with videotapes for 12 weeks, 3-5 times for a week with 20-30 minutes at a time, and combined with telephone contacts weekly. There are wide range of exercises including pelvic tilting, knee pull, straight leg raising, curl up, lateral straight leg raising and Kegel exercise to loosen back tension and strengthen muscles Data were analyzed by chi-square test, Fisher's exact probability and t-test using SPSS/win 7.5 PC^+. The results were as follows; The intensity of lumbar or pelvic pain in the experimental group was significantly lower than that of the control group on the pain for a last week at 12 weeks after (t=-2.30, p=.006). Also, the variation of back pain drawing after an exercise in the experimental group was significantly larger than that of control group at 12 weeks after the intervention (t=3.570, p=.0002). Postural stability in the experimental group was significantly more stabilized than that of control group when they move horizontally with their eyes open (t=-2.005, p=.050) and closed at 12weeks after the intervention (t=-2.402, p=.020). Self-efficacy on exercise in the experimental group was significantly higher than that of control group at 12 weeks after the intervention (t=2.26, p=.004). There were no statistically significant differences in either functional ability or anxiety level between experimental and control groups at before and after the pain-reducing program. It was indicated that the most painful regions drew were sacroilieum, lumbar, and buttocks and the patterns expressed were the feelings of pressing, aching and numbness. The findings showed that this pain-reducing program for pregnant women experiencing lumbar or pelvic pain was an effective nursing intervention in relieving pain and in enhancing a neutral posture and an self-efficacy on exercise during pregnancy. The results also supported the need for nurses to use this program to manage and prevent a pregnancy-related back pain for women early in pregnancy.
