(A) Comparison of Percevied Importance of the contents of Nursing Education after Cardiac Surgery between Cardiac Surgical Patients and Nurses원문보기

Heart disease are chronic ones which put many physical and mental distress on patients and increase the prevalence. Cardiac Surgery which is one of the methods curing such disease have many risk factors. After the surgery, therefore, an effective education for ensuring prevention of any complication and rehabilitation are very important. To perform the education effective correspondence between nursing requirements by patients and purposes of educating nurses. However many previous studies showed that such correspondence had not been realized. Thus this study attempted to provide basic data for developing the nursing education program by comparing the perceived importance of nursing education between patients and nurses. The study was designed in the form of comparison survey. Data was collected from Aug. 6. to 31, 1999. The subjects were 127 patients who visited department of chest surgery to receive medical examination after taking heart operations and 82 nurses who taking care of cardiac surgery patients in chest surgery ward and ICU and other ward at one general hospital in Masan city and two university hospital in Pusan city. The instrument for the study is questionnaire which based on 12 previous studies using CPLNI(Cardiac Patient Learning Needs Inventory) and related literatures and a published health information for after cardiac surgery patients from cardiac center in a general hospital. And a questionnaire containing 42 items whose evaluation is made using a scale ranging from one to four points, which was directly developed by the researcher through tests of validity and reliability and factor analysis. The reliability of the instrument was found at Cronbach α = .9555. The data were anaylzed by using frequency, percentage ratio, mean and standard deviation, factor analysis, t-test, ANOVA and Scheffe test. The Results of this study can be summarized as follows ; 1. The comparison of perceived importances of nursing education after cardiac surgery between patients and nurses showed that nurses and patients were at 3.47(± .40) and 3.25(± .44) points in average, respectively, in the full mark of 4 points. This indicates that the importance perceived by nurses was significantly higher than that by 2. In the comparison of the importances between the two groups according to seven content aspects of nursing education, it was found that there were significant differences between the two groups all over the aspects(p< .05), p< .01, p< .001), except in the aspect of anatomy and physiology for which the importance perceived by patients(3.61 ± 71) were higher than that by nurses(3.39 ± .68). 3. Such comparison between patients and nurses in terms of 42 items as mentioned above indicated that 32 among the totaled 42 items showed significant difference between the two groups(p< .05), p< .01, p< .001). Except in the item titled as 'explanation of the shape and function of heart' for which patients(3.61± .71) were higher in perception of the importance of nursing education than nurses(3.39± .68), the latter showed higher levels of such perception than the former in the remaining items(p< .05). 4. When the contents of nursing education after heart operation were compared in terms of their priority in importances, patients perceived the content of 'anatomy and physiology' as most important(3.61 ± .71) while nurses, 'general information on heart disease'(3.73 ± .32). 5. When the 42 question items regarding nursing education after cardiac surgery were compared in terms of their priority in importance, patients perceived the item titled as 'explanation of the shape and function of heart'(3.61± .71) while nurses, explanation of complications which need urgent treatment'(3.90 ± .34). 6. In comparison of perceived importances of the educational content aspects among patients according to general characteristics of the patients, it was found that there was significant differences in the aspect of 'other information based on periods after cardiac surgery'(p<.05). Through Scheffe test, the aspect were found perceived as more important by patients whose period after heart operation is below six months than by those who are 2 to 4 years after cardic surgery. Considering all these results as above, it seems desirable that nursing education after cardiac surgery should be provided with its focus on things patients are not aware of in relation to their requirements and the importance of nursing. In addition, such education should be intensely performed within the first six months after the surgery since patients perceive the education as more important within the same period than in any other periods.
