Brugada syndrome, together with long-QT syndrome, is a representative inherited cardiac disorder causing cardiac sudden death. The patient with Brugada syndrome developes fatal types of ventricular arrhythmia although the sructure of heart is intact. About 25% of the patients is believed to have mutations in SCN5A gene that encodes for the alpha-subunit of the cardiac voltage-gated sodium channel. Brugada syndrome is inherited as an autosomal dominant trait and the reported endemic area is the countries in Southeast Asia and Japan. Brugada syndrome may also be found among Korean subpopulation with fatal ventricular arrhythmia and we previously experienced 15 patients with fatal ventricular arrhythmia and other clinical features suggesting Brugaa syndrome. Since the genetic characteristic of SCN5A gene in Koean patients with Brugada syndrome is largely unknown, we screened SCN5A gene to characterize the molecular defects in Korean patients with Brugada syndrome. Genomic DNA was isolated and purified from whole blood of the recruited 15 probands. Then 28 exons of SCN5A gene were amplified with PCR, and were directly sequenced with automatic sequencing. We identified five types of mutations and polymorphisms: three polymorphisms, A29A in exon 2, H558R in exon 12, D1819D in exon 28; two missense mutations, G292S in exon 7, and, W1094C in exon 18. Three polymorphisms (A29A, H558R, and D1819D) described in this study essentially were identical to the previous ones found in different population, and two missense mutaions(G292S and W1094C) described in this study were novel. We believe that those mutations and polymorphisms in SCN5A gene identified from 15 patients with Brugada syndrome could be used in the future to confirm clinical diagnosis on molecular levels. Moreover our data will provide basic data to develop strategies in early detection of Brugada syndrome in Korean population.
