After controlling the effects of mental workload and job stress this study was conducted to determine the detailed mechanism which physical workload affects cardiovascular system by evaluating the effect of physical workload on heart rate variability. Methods: Two hundred and twenty four workers from foundry, ship building, car engine assembly, and precision machine assembly industry were study participants. Exclusion criteria were past history of diabetes and hypertension which can affect heart rate variability (HRV). Test for HRV were conducted in the morning for 5 minute. Physical workload was evaluated by relative heart rate ratio (RHR) using heart rate monitor and by structured questionnaire. Job stress was evaluated by Karasek's job content questionnaire. Results: SDNN and rMSSD decreased significantly with aging. log(TP) decreased with aging with borderline significance. log(LF) decreased significantly in higher RHR group. log(HF) was significantly decreased with aging and shift work group, and increased with social support with borderline significance. Conclusion: Physical workload increases cardiovascular disease risk by reducing HRV, which might be caused by wear and tear of sympathetic nervous system. As HRV was reduced in higher RHR group, physical workload criteria bassed on RHR would be valid. So it is necessary for us to maintain and promote workers' health by periodic health check-up using HRV as well as to reduce physical work overload using physical workload criteria based on RHR.
