국문초록 GC/MS로 니코틴과 그 대사물질의 분석을 이용한 ETS 노출지표 개발에 대한 연구 김진구 공주대학교 대학원 화학과 분석전공 (지도교수 박병빈) 본 연구는 생체시료인 혈액, 뇨 및 타액 중에서 니코틴과 코티닌을 동시에 분석하는 방법을 활용하여 효과적인 ETS 노출 지표 물질을 선정하고 더 나아가서 신속하고 간단하며 효과적인 시료를 결정함으로서 실제 ETS 모니터링을 위해 routine하에 사용될 수 있는 방법을 개발하는 데 그 목적을 갖고 있다. 연구 결과 다음과 같은 결과를 얻을 수 있었다. 1. 사람이 ETS에 노출된 정도를 알기 위해 혈액, 타액 및 뇨 시료로부터 nicotine과 cotinine을 정량분석하였다. 혈액과 타액 그리고 뇨 시료를 pH 12로 조절한 다음 니코틴과 코티닌에 모두 효과적인 methylene chlroide로 추출하였다. 내부표준물질로 diphenylamine을 사용하였고 검량선은 blank에 첨가한 후 추출후 작성하였다. 추출후 모든 시료는 GC-MS (SIM)으로 정량하였다. 이때 GC chromatogram상에 각각의 피이크들이 대칭적이고 sharp하였다. Nicotine과 cotinine이 위치하는 머무름 시간에서 방해 피이크가 존재하지 않았으며 검량선은 직선성이 0.99이상의 좋은 값을 보였다. 회수율은 모두 90% 이상의 값을 보였고 정밀도는 RSD로 나타내었을 때 6% 이하의 값을, 정확도는 90%이상을 나타내 높은 신뢰도를 보였다. 각각의 검출한계값 (MDL)은 모든 시료에서 1.0 ng/ml 이었다. 2. 학생 약 340명으로부터 각각의 혈액, 타액 및 뇨 시료를 받아 분석한 결과 혈중 nicotine의 농도는 6.3∼498 ng/ml의 분포를 보였고 혈중 cotinine은 4∼96 ng/ml의 분포를 보였으며 평균 51 ng/ml의 값을 나타냈다. 한편 타액에서는 nicotine의 농도가 0∼207 ng/ml의 분포를 보였고 cotinine은 0∼42 ng/ml의 분포를 보였으며 평균 24 ng/ml의 값을 나타냈다. 뇨 시료의 경우는 nicotine의 농도가 0∼1590 ng/ml의 분포를 보였고 cotinine은 0∼2986 ng/ml의 분포를 보였으며 평균 136 ng/ml의 값을 나타냈다. 이 결과를 기초로 시료 중 nicotine 및 cotinine의 농도간의 상관성을 조사하였다. Nicotine의 경우는 어떤 시료간에도 유의한 상관성은 발견할 수 없었다. 한편 cotinine의 경우는 뇨와 혈액간에 R값은 0.148을 나타내었고 P값은 0.264로 유의성이 적었으나 타액과 혈액간에는 R값이 0.961이었고 P값은 0.000으로 유의성이 매우 높았다. 이는 혈 중 cotinine이 일정비율로 타액으로 분비되고 있음을 나타내는 것으로서 타액은 혈액을 대표하여 사용할 수 있는 생체시료임을 보이고 있다. 타액 중 cotinine의 농도를 알면 y=2.31x+4.76 (여기서 x는 타액에서의 cotinine의 농도, y는 혈 중 cotinine의 농도)의 식으로부터 혈 중 cotinine의 농도를 알 수 있으며 생체 노출 지표물질로는 cotinine이 매우 효과적임을 알 수 있었다. 따라서 시료 취하기가 어려운 혈액대신 타액으로서 그리고 지표물질은 cotinine을 사용하여 ETS의 노출 정도를 평가할 수 있었다. 3. 1차 분석법에서 노출된 시료를 추출하는 과정에서 하루에 약 50-60개 정도 분석할 수밖에 없었던 문제점을 보완하여 타액시료만으로 신속하고 간단하게 ETS를 monitoring기법을 개발하였다. 타액 시료에 methlene chloride 첨가하고 내부표준물질 d3-cotinine을 넣은 다음 원심분리한 후 분리된 유기층을 자동시료주입기에 의해 주입되어 GC/MS(SIM)로 정량하였다. 그 결과 정밀도는 RSD로 나타내었을 때 2.3%이하의 값을, 정확도는 95%이상을 보였고, 타액의 코티닌 검출 한계는 1.0ng/mL이었다. 또한 이 분석법은 농축단계가 필요 없고, 간단하고 신속하며 하루에 1대의 분석장비로 150개의 시료를 분석할 수 있는 특징을 가지고 있다.

Indeed these biochemical markers have been used to estimate active smoking behaviors, to validate abstinence smoking, to evaluate exposure to environmental tobacco smoke (ETS). Many analytical methods have been described for the analysis of nicotinine and cotinine. In this study, The determination of nicotine and cotinine in biological samples (plasma, saliva and urine) was performed. Nicotine and cotinine were extracted with methylene chloride at pH 12 and concentrate to minimum volume with nitrogen stream. The quantization was performed with gas chromatography-mass spectrometry (GC-MS)-selected ion monitoring (SIM). Peak shapes and quantization of nicotine and cotinine are excellent, with linear calibration curves over a wide range of 1-3,000 ng/mL. The detection limits of nicotine and cotinine are 0.2 ng/mL in urine and 1.0 ng/mL in plasma and saliva. The intra-day precision of nicotine and cotinine in all samples was <5% relative standard deviation (RSD). Urine, plasma and saliva samples of 344 volunteers from a girls high school in Korea were quantified. As a results, the concentrations of nicotine and cotinine in plasma ranged from 6~498 ng/mL and 4~96 ng/mL. Otherwise, those of nicotine and cotinine in saliva ranged from 0~207 ng/mL and 0~42 ng/mL, and those of nicotine and cotinine in urine ranged from 0~1,590 ng/mL and 0~2,986 ng/mL, respectively. The primary metabolite cotinine of nicotine was good indicator of ETS exposure in nonsmokers. Salivary cotinine was found to be highly correlated to the concentration of cotinine in plasma. It appeared that cotinine is on average excreted in fixed relationships from plasma into saliva. We also found that the concentration of cotinine in plasma was successfully predicted from the salivary cotinine concentration by a following regression equation. y = 2.31 x + 4.76 x; the concentration of cotinine in saliva y; the concentration of cotinine in plasma The advantages of this procedure are good accuracy, reduced volumes of sample, rapid extraction periods of less than 20 min, and simultaneous analysis of nicotine and cotinine. For the rapid monitoring of ETS exposure, a simple analytical method of cotinine in saliva utilizing GC-MS was developed. Cotinine was extracted from 0.5mL of saliva with 100㎕ of methylene chloride and injected directly GC-MS(SIM). d3-Cotinine was used as internal standard. The new analytical procedure of cotinine in saliva showed the detection limit of 1.0 ng/mL and the intra-day precision of <2.3% (with relative standard deviation). The extraction method needs not any additional concentration step, and then is not affected by evaporation losses of cotinine. Turnaround time for up to about 150 samples was one day. The developed method is extremely simple, rapid and can be used easily for the monitoring of active and passive smoking. Through the accurate determination of cotinine in saliva, the risk of ETS-exposed human can be predicted.
