Method: ^(1)H MRS studies were performed in thirty five patients with language developmental delay who resulted from mental retardation or autism without abnormal finidnings on radiological, metabolic and genetic studies. Image guided in vivo localized ^(1)H MRS were obtained from right parietal white matter, fontal gray matter, and cerebellar cortex. The [N-Acetylaspartate(NAA)/Creatine(Cr)], [Choline(Cho)/Cr], [Myoinositol(mI)/Cr] ratios were measured and compared with those of normal control group. Also the correlation between ^(1)H MRS findings and the degree of language developmental delay, which was evaluated by Receptive Expressive Emergent language (REEL) Scale were investigated. Result: A increased [Cho/Cr] ratio was shown in parietal white matter and frontal gray matter of patients group compared with normal control group. No significant differences were revealed in any metabolite ratio between patients with mental retardation and autism. There were also no significant correlations between the degree of language developmental delay and metabolite ratios. Conclusion: Although the degree of language developmental delay were not correlated with ^(1)H MRS findings, increased [Cho/Cr] ratio could be useful to diagnose the patients with language developmental delay.
