한국의 노인인구 증가속도는 세계에서 유례가 없을 정도로 빠르게 진행되고 있으며 이에 따라 장기요양보호를 요하는 노인의 수도 증가하고 있다.

In this thesis, I studied and compared the development of the long - term care system of the elderly in Japan and Korea as a solution to the demand for the long-term care policy, in terms of the economic and cultural background as well as the political background. Furthermore, I examined the commonness and the differences of the long-term care policies of the two countries which resulted from different backgrounds. The goal of this thesis is to suggest the future direction of the long-term care policy in the economic, political and social condition of Korea, in relation to the recent introduction of ‘the long - term care system of elderly', which is still under discussion.
For this purpose, I analyzed the economic, social, cultural and political backgrounds of public long - term care insurance in Japan and the care system of elderly in Korea, with a focus on the introduction and early development of the systems.
For the analysis of the economic background, I examined the economic condition based upon the macroscopic economic index such as the ratio of the social security expense to GDP and GDP as well as the economic situation of the elderly. For the analysis of the social and cultural background, I studied the ratios of aging, female participation in economic activities and cohabitant with children, which decide the basic demand of long-term care policy for the elderly. Finally, for the analysis of the political background, I examined the transformation of the ruling ideology and the political system, in addition to the history of elderly welfare including its enactment and development.
As a result of these analyses, I reached a conclusion that the introduction of the care system of the elderly is a consequence of complex economic and social factors including not only the political circumstance but also the financial bankruptcy according to the increase of the elderly population who need long-term care and medical expense for themselves.
From this viewpoint, I can realize that the decisive factor of the recent introduction of the long-term care system is the economic and social demand rather than the political factor such as the ruling ideology or development strategy of the State under the authoritarian government of the past or the distribution-oriented policy of the democratized government.
The comparative analysis of the long-term care policies of Korea and Japan suggests some points. First, the care system for the elderly should be focused on ‘the home and community-based care' of which the basic requirement is the adequate level of care facilities. In order to construct ’the home and community-based care' smoothly, it is needed to support the main carer in families. Therefore, it is necessary to institutionalize on the public level the support of the long-term care service provider in families.
Second, the common consent among members of the nation should be formed. We should take notice that the introduction of the insurance system against senile infirmity, without the consent of the people, can cause social problems entangled with the unsettled matters of the existing system resulting in increase of distrust about the national policy in general as well as the social welfare system.
Third, the enactment of the elderly care system without thorough preliminary preparation can produce a dangerous effect, in the circumstance of Korea where the infrastructure of social welfare has not yet been constructed. In this sense, I argue that the elderly care system should be preceded by thorough preparation and construction of the welfare infrastructure.
Finally, the economic ability to pay for insurance and the service charge should be considered. The long-term care system can possibly be a burden to the lower-income bracket, especially to the disabled elderly, who are supposed to spend more expenses for the service. For this reason, it should be accompanied by a blueprint for the basic frame of further service for the future clients.
